FALL STARTED BETWEEN X-CUT #3 & #4 ENTRY & F ELL THROUGH THE #4 INTERSECTION IN TO THE X-CUT BETWEEN #4 & #5 ENTRY. ROOF FALL WAS 10' HIGH, 18' WIDE & 50' LONG. THIS AREA WAS BOLTED WITH 42" FULLY GROUTED ROOF BOLTS.
INJURED PARTY WAS DRAGGING A TRAILOR WITH THE UNIT SCOOP WHEN IT CAME LOOSE HE STOPPED THE SCOOP AND PUT IT IN NEUTRAL BUT DID NOT TURN THE SCOOP OFF. AS HE WAS GETTING OUT OF THE HE NOTICED T HE SCOOP WAS MOVING, AS HE TURNED AROUND HE SEENTHAT THE SCOOP WAS JUST ABOUT THE HOT THE COAL RIB. HE SLID BACK INTO THE SCOOP RIGHT AS IT WAS HITTING THE RIB STRIKING HIS HEAD AGAINST THE C
HEADER CLEANER WENT TO THE 1ST SOUTH HEADER TO CLEAN UP HEADER & FOUND THE ROOF FALL IN 4 ENTRYAT SPAD #10+50. THE FALL WAS 40' LONG, 18' WIDE,ABOUT 35" TO 40" HIGH.
INJURED PARTY WAS RIDING THE MANTRIP 2-6-01 WHEN THE MANTRIP RAN OVER A ROCK IN THE ROAD CAUSING THE MANTRIP TO BOUNCE UP. AT THIS TIME THE INJURED PARTY SAID HE A PAIN IN HIS LOWER BACK. THIS INJURY WAS NOT TURNED IN TILL 2-12-01.

INJURED PARTY WAS IN WASHER PLANT FIXING SKIRT RUBBER ON A BELT. AS HE WAS STRIKING THE CHISEL PUNCH WITH A RUBBER MALLOT HE MISSED THE PUNCH &STRUCK HIS RIGHT INDEX FINGER.

INJURED PARTIES WERE DRIVING A PERSONNEL CARRIERDOWN THE MAIN EAST SLOPE WHEN ONE OF THE BRAKE PADS BROKE CAUSING THE BRAKES TO FAIL. AFTER THIS HAPPENED THE INJURED PARTIES PUT THE DESSEL RIDE IN REVERSE WHICH CAUSED THE ENGINE TO DIE. AT THIS TIME THE INJURED PARTIES HIT THE ARCHES ON THE MAIN EAST SLOPE.
INJURED PARTY WAS TRYING TO DRAG CABLE OUT OF ROADWAY WHEN HIS FEET SLIPPED OUT FROM UNDER HIM AND HE FELL BACKWARDS.
INJURED PARTY BENT DOWN TO PICK UP THE REMOTE CONTROL BOX ON THE MINER WHEN HE SAID HE FELT A SHARP PAIN IN HIS LOWER BACK.
INJURED PARTY IS A PUMPMAN BY OCCUPATION AND HE WAS LOADING A PUMP INTO THE BACK OF A RIDE WHEN THE PUMP ROLLED OVER AND SMASHED HIS LEFT RING FINGER.
INJURED PARTY REPORTED WHILE WALKING ACROSS THE SECTION HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL AND PULL SOMETHING IN HIS LOWER BACK.
