Employee reached behind a tool box to retrieve some welding rods. As employee pulled the welding rods out the employee lacerated left forearm on a sharp edge of the toolbox. Employee received 10 sutures to the wound.

The area was previously supported with 5' Hy-tech bolts and 12' cable bolts. Affected miners were removed from inby the location and the primary escapeway was re-routed.

While moving the roof bolter, a piece of roof rock measuring 13" x 15" by 2" in thickness fell striking employee's hard hat causing the hat to slide down employee's face. Employee sought medical attention on a later date and began missing work on 2/9/2016 when employee had surgery on their neck.

The fall measured 18' by 30' and was 10' in thickness. The fall area was dangered off. There were no persons affected. The area was previously supported with 6' torque tension bolts and 12' tensionable cable bolts.

An MRI was obtained and the results shows that Employee has degenerative disc disease from L3-L4 to L5-S1. Under the advise of the physician, employee began missing work on 2/21/2017 due to the condition.

The fall was originally supported with 5' hi-tech bolts and 8' cable bolts. The primary escape way was rerouted and all employees were notified of these changes.

While employee was hanging the miner cable a piece of draw rock fell striking right hand which was still on the cable. The employee received a laceration on right middle finger which required sutures. EE was wearing gloves at the time of the incident and returned to work next scheduled shift.

The area was originally supported with 36" resin bolts and timbers. All persons were moved outby until the escape way was re-routed. No persons were affected.

EE pulled harder and when the structure released, EE fell backwards with the structure hitting right leg below the knee causing a laceration requiring staples. As worker was helping align a feeder during a set up, went around the feeder to look at the alignment. As EE walked around the feeder, slipped and fell striking chest on a crib block that caused a contusion to chest. EE began missing work on 5/11/2016 when EE had the contusion drained by a physician.

While workers were cleaning up material that was removed from a new coal transfer borehole using a continuous miner, a section of rib/brow fell from the inby portion of the entry and continued outby pulling rib bolts. The material rolled down the rib and struck the miner operator causing fatal crushing injuries to chest and abdomen.

While employee was hanging a shuttle car cable, he missed the hanger causing he cable to pull his arms forward straining his right shoulder. He continued working but began missing work on 1/18/2016 when he had surgery to repair a tear in the shoulder.

Employee was pushing a ram car up a grade with a diesel scoop. The ram car gained traction allowing the bucket of the scoop to lose contact with the ram car. The front wheels of the scoop were raised approximately 10 inches off the mine floor. The scoop fell back to the floor causing the operator to hit his head on the canopy of the scoop causing a neck strain.

When employee lowered the boom on the roof bolter after drilling a hole, the steel came out of chuck and struck him on the left thumb. He later sought medical treatment and had surgery on the thumb to repair a torn ligament on 10/22/2015.

The area was originally supported with 5' Hi-Tech bolts, truss bolts, cribs and beams. There were no injuries and the escapeway was re-routed. The fall will be cleaned up and re-supported.

Employee was moving a sump pump. The pump line was tied off at the pump and lo-trac forks. While the operator was pulling the waterline with the lo-trac, the waterline got caught on the feeder causing the rope hangers to break allowing the pump to strike the operator on the left side of the face. He suffered fractures to the left side of his face.
