While lifting pumps onto a pickup, employee felt a burning sensation in his groin area. Employee was examined on 01/26/2015 and was diagnosed with a hernia, and received a full release to return to work.

While cutting conveyor belt employee lost his balance and almost fell causing the knife to push into his leg.

While cutting a rope hanger with a utility knife he cut his hand, requiring stitches.

Employee was putting hydraulic oil in the scoop. He repositioned the can to better align it over the hole when his hand slipped off the can striking a hydraulic saw causing a laceration that required stitches.

A non-injury roof fall occurred in the Northwest Mains at break 33 of old #6 belt, 1 x-cut inby spad 2044 in the #1 entry. The fall was approximately 25'L X 19'W X 7'H in an intersection.

Employee was positioning a set of metal cribs under monorails, to load on a flatbed truck, when he felt a burning in his right knee. He was given a knee brace.

Employee was walking towards the rear of the roof bolter and did not see a roof strap that was damaged. Employee struck the roof strap with his shoulder and neck resulting in a laceration that required stitches to close the wound.

Employee was helping load a cache of SCSR's into the scoop bucket when he twisted his right knee.

EE was walking on offside of belt and hit his head on the roof knocking off his hat. He then bent down and got his hat and when he raised up he hit his head on a chain hanger causing a laceration. He received stitches to close the wound

Employee had stopped at airlock doors, just inside portal, when a second mantrip entered mine and struck first mantrip resulting in a strain to employee in 1st mantrip. Bright sunlight outside had not allowed eyes of 2nd mantrip operator to adjust.

Employee was in the process of dragging 48" top structure for the belt move they were making. He stepped on another piece of structure causing his knee to turn causing him to fall.

EE was walking across the section toward the roof bolter, he bent down to go through a line curtain and after he stepped through the curtain he twisted his ankle.

Employee was hanging line curtain and as he turned around to leave the area, he tripped over a rock and fell landing on his knee.

Operating a Hitachi 1700 Rock Truck traveling up an incline to the dump. As the truck approached the top section of the ramp, a large rock shifted causing the truck's front wheels to come off the ground and then back down, jarring the member. He returned to work, then on 4/1/2011 he was taken off work until 4/11/2011.

Employee was installing a man door in a previously built stopping. He placed his left hand on the bottom row of block and a block fell from the top row of block causing a laceration to his index finger.

He was taken to ER and was released with no restrictions but continued to experience pain. On Thursday December 16, 2010 he had surgery to repair a torn meniscus.

EE was tightening at the slack adjustment bolt on the Fletcher RRII roof bolter and the wrench slipped and caused a laceration on his middle finger. He received 3 stitches to close the wound.

Going down hill on New Road. Ran over something, tore out plug in bottom of fuel tank & lost all the fuel out. Motor died out, lost brakes. Truck was checked at the scene. Was driver's fault.

Employee was making a belt splice. After end of splice was complete, the two ends would not align, so employee was going to cut off a clip on belt when the knife slipped causing a laceration to his left palm.

He reached up with his left hand and torch fell off his leg and landed on his hand. He was taken to ER and treated for minor burns and was released to return to work on 10/11/10.

Employee was in the process of switching seats after dumping his loaded shuttle car when he stepped on a rock causing his knee to twist. He finished his shift and transported himself to hospital where x-rays were negative. He was given a brace and was released with no restrictions to return to work on 10/10/10.

EE was dismounting a DML 45 Rock Drill when he slipped and fell. He was taken off work on 10/15/10 after an MRI revealed he had a bulging disc and a sprained left knee.

Employee was in the process of bolting using a two piece steel. After he drilled his starter hole he started his two piece steel and had his index and middle fingers caught causing a laceration.

EE was using utility knife to open a box. He let knife slip, contacted his left hand, causing injury. He was taken to ER for treatment, stitches were used to close the wound.

EE 1 break inby #2 belt head helping recover rail. Standing next to rail being lifted by fork-lift. Rail fell due to mud on it, struck EE's right foot. Mine worked out, no one scheduled to work there after 2/18/09.

Came around corner on 4-wheeler, bumped timber, timber fell onto hand causing injury. Taken to ER for treatment, stitches used on wound. Mine employee re-set timber.

Fall discovered 6:55 AM, break #18 at old 2A blet in return #2 entry. Fall 20' X 40' X 4-5' high; area was breakered off. Both MSHA and KY OMSL were notified, neither investigated. This was a continuation of a previous fall.

Fall discovered 12-02-2008 inby break 18 on 2A belt in main left return. The fall was 20 ft x 100 ft x 7>8 ft high. The area was already dangered, it was breakered off, and will not be rehabed. Both MSHA & OMSL were notified.

Had fall in face area, 4ft X 10ft X 7ft high; breakered area off, not to rehab area, moving section. MSHA & OMSL notified. No equipment involved, no people injured, no vent disturbed.

ROCK FALL; in main return on belt 2A, 20' X 70' X 5' high. No equipment involved, no ventilation affected. MSHA & OMSL notified, will rehab the area; clean up and install additional support.

Truck dropped off the road & went in the ditch.

Putting up strap, used hammer to adjust strap to drill next hole. Missed the strap and right wrist contacted strap, causing laceration. Employee was taken to the E.R. for treatment. Stitches were used to close the wound.

06-12-08, @8:05 A.M. a fall was discovered on #2A belt at break 21, in #1 entry, in the return. The fall measured 20' X 140' X 6' high. All entrances to the fall area was breakered off. The area will not be rehabilitated. Inspector inspected the area.

Fall was discovered at 2A belt, break #25 entry #2 about 11:00 A.M.. Fall was 20' X 70' X 6' high. Entrances to the fall were breakered off, and area will not be rehabilitated. Inspector investigated the fall.

Employee trimming plastic from cable hanger. Knife slipped, employee received injury to left forearm. Taken to E.R. for treatment, stitches used to close wound. RTW 5-2-08

Fall in the old Eastern Coal works area was discovered 11/20/07, #1 entry 4X-cuts inby portal. The fall measrued 20' X 60' X 8-10'. Area was breakered off, and MSHA and OMSL were notified, both agencies investigated. Return air had 50,000 CFM coming over the fall.

Fall discovered in #2 entry, #1 belt intake, break #6; Eastern's old works. Fall measured 20' x 60' x 6' - 7'. Area was breakered off, will be cleaned and additional support installed. Both MSHA and OMSL investigatied.

Operating SC, went under RB cable; flat rock on top of SC was pulled by RB cable, sliding under canopy into lap of employee. Taken to E.R. for treatment. RTW 10-17-07. 11-5-07 returned to doctor due to pain; removed from work for 2 weeks.

At end of track on section, man-trip slid on wet track (top sweating), employee holding onto bar behind seat, man-trip bumped flat car causing box with SCSRs to slide. SCSR box struck hand causing injury.

Fall on section 38' x 70' x 4' high. MSHA - OMSL notified. All entrances breakered off. Section not to be rehabed.

EE discovered a fall in the return at 3rd right panel, 1 x inby spad #2797. The fall measured 20' x 10'x 5' high, this area is in old works, MSHA inspector inspected the area 6/6/07 All entrances to the fall area had been breakered off, this area will not be rehabilitated.

Employee was replacing right cutter motor guard, the guard fell onto and cut the right ring finger. He was taken to the e.r. for treatment. He had stitches inserted to close the wound, and returned to work.

Employee stepped across tounge of mobile mine office, bumped head on air conditioning unit protruding from wall causing injury. He was taken to the E.R. for treatment and stitches were used to close the wound.

While taking down a belt conveyor structure the structure fell prematurely throwing two employees to the ground 25 ft below.
