Employee had finished safety talk with crew and two MSHA Inspectors and went to the power center. He called for the Shift Foreman to come to him and was holding his chest, saying he had pain in his chest and arm. Foreman checked pulse, stopped at Trauma Trailer got AED, O2. Foreman administered O2 and transported employee to surface where he was taken to the hospital by ambulance.

The weekly examination was being performed when the foreman, EE, found a roof fall in the (A LEFT)panel one break inby spad #552. The foreman timbered all sides of the fall and dangered off the area. Additional timbers were set in nearby entries around the fall.

The Lo Trac operator was checking the fluids on his equipment. When the operator raised the engine hatch to check the fluids, he secured the hatch with a rope hanger. The rope hanger came loose causing the hatch to fall on the operator's left hand resulting in a fractured thumb.

The employee was pushing a rock out of the way with the stab jack on the roof bolter. The rock either broke or the stab jack slipped over top of the rock, causing the drill head to move forward quickly catching his left thumb between the drill steel and the canopy post.

While the individual was tightening the wheel on the shuttle car, the ratchet slipped causing him to lose balance and fall hitting his mouth on the fender resulting in two chipped teeth. There were no lost or restricted days and he has not sought treatment.

The individual was raising a 3.2 hp submersible pump to adjust the hanger straps. The pump slipped and dropped. The individual experienced sharp pain in lower left abdomen. No lost or restricted days at this time. Surgery will be scheduled to repair the hernia.

The individual had been grinding in the shop area. When he finished he rubbed his eyes with his hands and got a sliver of metal in his right eye. He was wearing prescription safety glasses while he was grinding. The individual received prescription medication for his eye.

The employee was changing the operator's side feed cylinder on a roof bolting machine. A come-a-long hoist was hooked to the machine's boom and a roof support plate. The hook came loose from the roof support plate and the boom fell catching his arm.

The employee was releasing an electrical trailing cable which was hung to a roof bolt plate. When he released the cable, the cable hanger caught his hand resulting in a laceration between his forefinger and thumb. The laceration required sutures.

The employee alleges that he was hanging a miner cable and had it on his shoulder. The cable pulled him to the ground, resulting in a possible bruise to his hip. The alleged injury is still under investigation and we reserve the right to withdraw the report upon completion of the investigation.
