Employee has filed a hearing loss claim dated February 5, 2013, with the Virginia Workers' Compensation Commission. As per the audiologist's report, employee suffers from "moderate sloping to severe sensorineuria loss." Employee's last date of service with company was 10/21/2012. **THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.**
On, November 1, 2012 employee strained his right arm while lifting a 3-wheeler battery. He did not miss any work. On November 17, 2012, he was terminated as a result of company cut-backs. On February 14, 2013 employee had surgery on his right arm due to the injury on November 1, 2012
Drill steel was hung in drill pot, employee engaged rotation and drill steel struck employee on left shoulder and ear causing a laceration to his ear.
Employee was installing rib bolts when he stepped back and got foot caught between cinder blocks causing knee to twist. Location was MMU 001 60' outby SS #377
Walking to move miner cable was walking down small incline, slipped and fell landing on his back. Bottom was wet and muddy at time of incident.
A Roof Fall occurred on 3 Left Panel at SS# 1209 to SS# 1227 in the #1 return entry. The fall measured Length 105' x Width 18' x Height 7'.
Employee had finished safety talk with crew and two MSHA Inspectors and went to the power center. He called for the Shift Foreman to come to him and was holding his chest, saying he had pain in his chest and arm. Foreman checked pulse, stopped at Trauma Trailer got AED, O2. Foreman administered O2 and transported employee to surface where he was taken to the hospital by ambulance.
Employee was putting rub rail back on Continuous miner. Employee set it in place and went to push rail in when it fell striking him on top of left foot. Accident occurred at MMU 001, between #2 and #3 entries at Brk #9.
Employee was loading augaline, sliding line forward pinching finger between augaline and coupling causing a laceration to his finger.
Drill steel was hung in the drill chuck, employee struck drill steel with wrench and it came back striking employee above the left eye causing a laceration.
Changing diesel scoop battery, employee started to pull battery out and it got caught and jerked a battery cap off splashing acid in left nostril.
The weekly examination was being performed when the foreman, EE, found a roof fall in the (A LEFT)panel one break inby spad #552. The foreman timbered all sides of the fall and dangered off the area. Additional timbers were set in nearby entries around the fall.
While employee was installing roof bolt a piece of rock fell, hitting the pan and plate pinching left hand against wrench bruising middle and index fingers.
The Lo Trac operator was checking the fluids on his equipment. When the operator raised the engine hatch to check the fluids, he secured the hatch with a rope hanger. The rope hanger came loose causing the hatch to fall on the operator's left hand resulting in a fractured thumb.
Employee was beating foot shaft off miner. Piece of steel punctured left arm and lodged in it.
Employee had moved the refuge chambers and was hooking up lifelines. After crawling under the belt line he stood up and felt pain in his lower back and right leg felt cold.
The individual was trying to pull a piece of ventilation curtain on a mud hole. He was pulling with both hands when his left hand slipped and caused him to strain his right shoulder.
The employee was pushing a rock out of the way with the stab jack on the roof bolter. The rock either broke or the stab jack slipped over top of the rock, causing the drill head to move forward quickly catching his left thumb between the drill steel and the canopy post.
The individual was putting an end on an extension cord and cut his thumb resulting in a laceration that required stitches. This is a medical only report with no lost or restricted work days.
The EE was dropping the miner cable that was hung in the #7 heading and as he was dropping the cable he felt a pain in his lower back.
The EE was loading I-Beams into the scoop bucket when the back end of the I-Beam kicked around and caught his finger between the beams. The injury required sutures but there were no lost or restricted work days.
While the individual was tightening the wheel on the shuttle car, the ratchet slipped causing him to lose balance and fall hitting his mouth on the fender resulting in two chipped teeth. There were no lost or restricted days and he has not sought treatment.
As the individual was traveling underground on a Mac 8, his dinner started to move. He reached back to catch it as he started under an overcast, catching his left ear causing a laceration which required sutures.
The individual was traveling outside on a Lil Mac when he ran into an equipment door. He did not stop at the door switch. The ride was checked and was found to be in proper operating condition.
The individual pulled a pallet off the Lo Trac fork lift and mashed the ring finger on his left hand. He developed staff infection and was hospitalized.
The individual was operating the continuous miner and positioned outby the boom of the miner wearing safety glasses, when an object was thrown by the cutter head, striking himi in the head above the left eye. This is a medical only report.
The EE was inserting a roof bolt in the mine roof and as he raised his arm, he felt pain in his right shoulder.
The employee was moving the cable to the continuous mining machine out of the roadway and felt pain in his upper abdomen. He continued to work his regular job. but during a dr's visit on june 29 2010 it was determined that he had a hernia of his upper abdomen.
EE slipped on a rock falling backwards and hitting his left shoulder on an anchor pin at the #4 belt drive.
The individual was tightening the steering arm bolts with a wrench when he felt pain in his left arm and shoulder. This is a medical only reportable accident as the indiviual performed his normal duties and did not have any lost or restricted work days.
The individual was walking to the rear of the roof hitting machine when he stepped on an area of irregular bottom and twisted his knee.
The individual was raising a 3.2 hp submersible pump to adjust the hanger straps. The pump slipped and dropped. The individual experienced sharp pain in lower left abdomen. No lost or restricted days at this time. Surgery will be scheduled to repair the hernia.
The individual was haying the miner cable and water line when a piece of draw rock fell striking him on top of the left foot.
The individual slipped on a ledge in the #5 heading and twisted his right ankle.
The individual was getting out of the operator's compartment of the roof drift and twisted around. He said he felt something pop in his chest.
The EE was observing the operation of the CM when a piece of rock and coal came off the head of the CM and struck him in the left eye. Some material was embedded in his eye and had to be removed. This is a medical only treatment. No lost or restricted work days.
The individual had been grinding in the shop area. When he finished he rubbed his eyes with his hands and got a sliver of metal in his right eye. He was wearing prescription safety glasses while he was grinding. The individual received prescription medication for his eye.
The conditions were wet had a light drizzle rain all day.
While cleaning rock off the drill head of the roof bolter a piece of draw rock fell from between the bolts hitting the individual on the right shoulder and lower back.
The individual had his back to roof bolter waiting on miner to exit #5 entry. When miner trammed out of the way, the roof bolter trammed forward with the T-bar of the roof bolter catching the toes on the right foot.
EE was operating roof bolter, rock fell from roof, striking EE on left hand. To correct the problem, EE was told to keep hand off of drill head and observe top before starting to bolt.
The employee was changing the operator's side feed cylinder on a roof bolting machine. A come-a-long hoist was hooked to the machine's boom and a roof support plate. The hook came loose from the roof support plate and the boom fell catching his arm.
The employee was releasing an electrical trailing cable which was hung to a roof bolt plate. When he released the cable, the cable hanger caught his hand resulting in a laceration between his forefinger and thumb. The laceration required sutures.
The employee, along with another person, was hanging the power cable to the continuous mining machine and felt pain in his lower back when he lifted the cable.
The employee alleges that he was hanging a miner cable and had it on his shoulder. The cable pulled him to the ground, resulting in a possible bruise to his hip. The alleged injury is still under investigation and we reserve the right to withdraw the report upon completion of the investigation.
Employee got truck out of gear on hill to slow truck down. He steered truck into drainage ditch; impact from going into ditch caused water tank to become loose and come into cab on driver.
The injured was putting roof bolting supplies on the roof bolting machine. He cut the band on a bundle of roof bolts. The band sprung loose and struck him on the left hand. This resulted in a laceration which required sutures.
Injured was stepping across the cable to the roof bolting machine. When the cable reel of the machine was turned on, the cable caught his foot, causing him to fall and bruise his ribs.
The EE was working on the feeder which had stalled out. The stalled out feeder caused the water line to heat up and rupture. The water sprayed the individual on the right leg resulting in a burn.
The injured was pushing a rock off of the top of the feeder and cut his middle finger on his right hand.
