ROOF FALL WAS FOUND DURING WALK AROUND INSPECTION WITH MSHA INSPECTOR. FALL WAS IN #6 CROSS CUT SUB SOUTH MAINS IN RMS DRIVEN EAST FALL WAS IN #2 ENTRY OF ROOMS IN FIRST INTERSECTION. FALL WAS 30 FT LONG 17 FT WIDE 5 FT HIGH. THIS IS A NONEACTIVE AREA OF THE MINE.
DOING PRE-SHIFT IN NOTCH OF MAIN INTAKE ROADWAY STEPPED BACK ONTO MAIN ROADWAY AND SLIPPED ON ICE. FELL AND TWISTED RIGHT KNEE.
ROOF FELL IN AN UNBOLTED FACE INBY THE ROOF BOLTS. ROOF FALL IS APPROX 15'WX20'LX8TO9'H.
INDIVIDUAL WAS SETTING TIMBER NEXT TO CONTINUOUS MINER THAN WAS DOWN. HE JUMPED OFF MINER TO THE GROUND TWISTING HIS ANKLE ON A LUMP OF COAL AS HE LANDED.
WHLE HANGING SHUTTLE CAR CABLE INDIVIDUAL FELL OFF LADDER STRIKING HIS NECK AND CHEST ON TOP OF LADDER. EE WAS KEPT OVERNIGHT IN HOSPITAL FOR OBSERVATION.
ROOF FALL IN NON-ACTIVE SECTION--FALL WAS IN NEUTRAL AND RETURN ENTRIES. FALL WAS 6-8' HIGH, 30' LONG, 20' WIDE. THE AREA HAD BEEN TIMBERED PREVIOUSLY BEFORE THE ROOF FALL OCCURRED.
ROOF FALL OF UNSUPPORTED ROOF IN CROSS CUT FROM 5 ENTRY TO 6 ENTRY CROSS CUT HAD BEEN CUT THROUGH 40 FEET AND NOT BOLTED YET WHEN IT FELL. THIS DID NOT INJURY PRODUCTION THIS WAS NOT AN ESCAPE WAY. THE FALL HEIGHT WAS 3-4 FEET FALL WAS 50 FEET LONG 18 FEET WIDE.
WHILE CUTTING BELTING FOR SKIRTING ON SURFACE DUMP POINT, KNIFE SLIPPED AND LACERATED RIGHT LEG JUST ABOVE KNEE RESULTING IN 5 STITCHES
INSTALLING GUARDS ON DRIVE CHAIN OF BELT DRIVE. COVER SLIPPED PINCHING FINGER BETWEEN COVER AND FRAME OF BELT DRIVE. THIS INJURY REQUIRED 10 STITCHES
ROOF FALL IN INTERSECTION OF CONNECTING ENTRY FROM MAIN SOUT TO 1ST SOUTH OFF 1ST WEST. ROOF FALL BLOCKED ENTRY AREA OF FALL WAS TIMBERED BEFORE FALL.
USING PORTA POWER TO REALIGN SCRUBBER ON CONTINUOUS MINER. PORTA POWER SLIPPED OFF CAP PIECE STRIKING EE ON THE LEFT CHEEK CREATING A LACERATION THAT REQUIRED THREE STITCHES TO CLOSE.
EE WAS DRAGGING CABLES WITH DIESEL SCOOP. HE GOT OUT OF SCOOP AND STRUCK KNEE ON HYDRAULIC TANK CAUSING A SMALL CUT ON RIGHT KNEE THAT REQUIRED TWO STITCHES.
FALL WAS IN CROSSCUT #12 #1 ENTRY SUB SOUTH MAINS #2. FALL IS 18' L 15'WX7-8'H.
THIS FALL WAS IN X CUT 19 NO 5 ENTRY 1 ST SOUTH SECTION. THE FALL WAS 20 FT WIDE 25 FT LONG AND 7 FT HIGH. THE MIER WAS OUT OF SEVICE AND INOPERABLE AT THE TIME OF THE ROOF FALL. THIS IS A WOR KED OUT AREA IN THE SECTION AND WAS BEHIND THE 2ND MINING AREA. THIS ENTRY WAS CORRECTED WITH MAIN SOUTH SECTION.
THIS ROOF FALL WAS IN #2 ENTRY X CUT 19 1ST SOUTH. THE FALL WAS 20' WIDE, 7' HIGH AND APPROX 40' LONG. THIS AREA HAD SHOWN SIGNS OF WEAKNESS AND CRIBS WERE BUILT AROUND AND IN THE FALL AREA BE FORE IT FELL. THIS AREA DID NOT FALL ABOVE THE ANCHORAGE ZONE UNTIL 9-23-97.
THIS ROOF FALL WAS IN 2 ENTYR X CUT 19 1ST SOUTH THE FALL WAS 20' WIDE 7' HIGH AND APPROX. 40 FT LONG. THIS AREA HAD SHOWN SIGNS OF WEAKNESS AND CRIBS WERE BUILT AROUND AND IN THE FALL AREA BE FORE IT FELL. THIS AREA DID NOT FALL ABOVE THE ANCHORAGE ZONE UNTIL 9-23-97.
THIS IS A FAULT AREA. IT HAD FALLEN IN THE EARLY 1980'S. IT WAS SUPPORTED WITH 8" BEAMS, CRIBS, & TIMBERS. THROUGH THE YEARS ROCK HAD BUILT UP ON THE BEAMS CAUSING THE BEAMS TO SPREAD. THE BEA MS KICKED OUT ALLOWING THE MATERIAL TO COLLASPE TO THE GROUND. THIS IS NOT A FRESH ROOF FALL.
EMP WAS CUTTING BELT WITH A UTILITY KNIFE. HE WAS PULLING THE KNIFE TOWARDS HIS BODY WHEN THE KNIFE SLIPPED AND CUT HIS LEG. HE WAS NOT PRACTISING THE PROPER CUTTING METHOD.
APPROX. 100 PM 7-29-97 CONTINOUS MINER WAS CLENAING CROSS CUT 9 IN 1 ENTRY OF 1ST SOUTH SECTION SO 8 FT BOLTS AND BEAMS COULD BE INSTALLED WHEN APPROX. 4 FEET OF TOP COAL FELL ON THE CONTINUOU S MINER CAUSING OF RAGE STUCK IN THE SOFT BOTTOMAT APPROX 100 AM 7-30-97 ANOTHER APPROX. 2 FT OF TOP COAL AND APPROX. 2 FT OF ROCK FELL ON MINER.
THIS IS AN INACTIVE AREA IN THE MINE. THE WEEKLY EXAMINER FOUND THE FALL ON HIS WEEKLY EXAMINATION OF THIS AREA. THE AREA HAD SHOWN NO SIGN OF FALLING THE WEEK PRIOR. THE FALL WAS NOT IN HIS T RAVELWAY. THIS FALL OCCURRED IN 1ST RIGHT ROOM OFF. 3RD EAST.
ROOF FALL WAS BETWEEN X-CUT 3 & X-CUT 4 IN 1ST SOUTH SECTION. FALL WAS IN #4 ENTRY. FALL WAS APPROX 25'L, 17'W, & 7'H. FALL WAS OUTBY WORKING SECTION. THERE WAS SECONDARY ROOF SUPPORT INSTALLE D IN THIS AREA BEFORE ROOF FALL.
EE WORING IN TRAIN LOAD OUT SHOP. EE WAS USING A UTILITY KNIFE AS A SCREW DRIVER. THE KNIFE SLIPPED AND CUTHIS PALM REQUIRING 3 STITCHES.
ROOF BOLTING IN A DIFFICULT OP CONDITION. A SMALL PIECE OF TOP ROCK FELL OUT AND HIT THE EE IN THE MOUTH. INJURIES REQUIRED 3 SUTURES.
2 FACE IN THE 1ST WEST SECTION FELL. THIS AREA WAS DETERMINED TO BE A PROBLEM AREA. MINING HAD STOPPED IN THIS DIRECTION AND FACE WAS CRIBBED AND TIMBERED JUST INBY EDGE OF INTERSECTION.
EMPLOYEE WAS ROOF BOLTING. LOWERED DRILL BOOM AND STEEL XXX IN HOLE. STEEL CAME OUT OF HOLE AND STRUCK HIM ON THE MIDDLE FINGER OF RIGHT HAND HAND WAS IN A PINCH POINT.
EE & PARTNER WEREMAKING A BELT SPLICE. EE WAS FEELING NAILS TO SEE IF PROPERLY SEATED. PARTNER DIDN'T SEE EE'SHAND & HIT HIM WITH HAMMER. HURRYING TOO FAST TO GET BELT SPLICED.
EMP HAS HAD SORENESS FOR SEVERAL WEEKS. WAS THROWING SHUTTLE CAR CABLE WHEN HE AGGRAVATED THE BACK AND WAS TAKEN TO ER FOR PRECAUTIONARY REASONS. INJURY WAS MUSCULAR AND NOT DISC RELATED.
HOOKING COME-ALONG TO BELT HANGER. BELT CHAIN CAME LOOSE AND COME-ALONG HIT THE EMPLOYEE'S RIGHT HAND BREAKING INDEX FIGNER AT THE END METACARPAL BONE.
#3 ENTRY CROSSCUT 21 IN SECOND RIGHT BLEEDER SYSTEM FELL. THIS FALL MADE IT IMPOSSIBLE FOR WEEKLY EXAMINER TO TRAVEL TO END OF BLEEDER SYSTEM.
THE EMPLOYEE WAS MAKING A BELT SPLICE WITH SEVERAL OTHER EMPLOYEES. ONE EMPLOYEE WENT TO TAKE THE COCK OFF THE POWER. INJURED REMEMBERED THE BELT BLOCKS WERE STILL ON THE BELT. HE TRIED TO BEA T THE OTHER EMPLOHEE IN TAKING THE BLOCKS OFF BEFORE THE BELT STARTED. THE BELT STARTED BEFORE HE WAS FINISHED. HE FELL BACK AND HIT HIS HEAD.
EE WAS FILLIN A SECTION SUPERVISOR. WATER WAS OFF GOING TO THE CONT. MINER. HE GOT THE WATER FIXED & GAVE THE XXXX TO VENTILATE AIR. HE WAS FACING THE CONTINUOUS MINER. THE SHUTTLE CAR OPERATO R WASIN AN OFFSIDE CAR AND APPROACHED THE MINER TO LOAD. THE SC OPERATOR DID NOT SE EE AND PINNED HIM BETWEEN THE CAR & MINER. EE WAS IN THE INCORRECT OPERATING POSITION.
RIB BOUNCED OFF KNOCKING PANIC SWITCH ON OPERATOR SIDE OF MINER IN. WHEN ROOF STARTED TO FALL MINER WAS UNABLE TO TRAM AND ROCK FELL ON MINER. FALL WAS APPROX 6-8'H, 20'L, 18'W.
AREA WAS UNSTABLE. HAD ADDED SECONDARY ROOF SUPPORTS AND HAD DANGERED AREA OFF. ROOF FELL WHERE INTERSECTION WOULD HAVE BEEN.
INTERSECTION OF #4 ENTRY XCUT #41 IN MAIN SOUTH RETURN FALL ABOVE ANCHORAGE.
PIECE OF COAL FELL FROM RIB AND STRUCK THE EE. AT FIRST THERE WAS NO SIGN OF INJURY. LATER SWELLING DEVELOPED CAUSING EE TO BE OFF
IN #5 ENTRY CROSS CUT 65 IN OLD MAIN SOUTH SECITON ROOF HAS FALLL ABOVE THE ANCHORAGE. NO MINING ACTIVITY HAS TAKEN PLACE ON THIS AREA IN 15 MONTHS. ALL EQUIPMENT HAS BEEN REMOVED 15 MONTHS AGO.
WALKING ON SURFACE DOING EXAMINATION, SLIPPEDON ICE BOUNCED WITH SNOW AND STRAINED KNEE.
PUTTING STEPPING ON MINER AFTER SERVICING. SMASHED RIGHT RING FINGER WHEN STOPPER SLIPPED ON CUTTER HEAD.
EMP WAS OPERATING SHUTTLE CAR. TRAVELED AT RATE OF SPEED TOO HIGH FOR THE CONDITIONS. HIT A BUMPCAUSING HIM TO HIT HIS HEAD ON THE CAB. RESULT WAS A STRAIN TO THE NECK AND LOWER BACK.
EE STEPPED ON LOOSE PIECE OF COAL. RESULT WAS SWERVE SPRAIN ANKLE AND POTENTAL STRESS FRACTURE.
THE FALL WAS IN THE RETURN ENTRY OF THE FIRST RIGHT SECTION OUTBY. THE WORKING SECTION. THE FALL WAS IN AN AREA THAT WAS TIMBERED AND CRIBBED DOWN TO APPROX. 14' WIDE. THE ROOF FELL BETWEEN TH ESE EXTRA SUPPORTS.
ROOF BOLTING A CROSS-CUT. WHILE PUTTING IN A RIBPIN, THE RIB KICKED OUT. THE COAL HIT THE EE ABOVE THE ANKLE BREAKING THE BONE.
DRILL WRENCH GOT STUCK, EMPLOYEE SPUN ROTATION LEVER TO LOOSEN THE WRENCH. THE WRENCH SPUN OUT OF THE CHUCK AND HIT THE EMPLOYEE.
EMPLOYEE ALLEGES INJURY OCCURRED WHILE LOADING COAL. ACCIDENT NOT REPORTED UNTIL AUGUST 30,1996WHEN EMPLOYEE WENT TO EMERGENCY ROOM COMPLAININGOF BACK PAIN.
