Security officer went to use porta potty located on site. While stepping in floor was covered in ice causing the officer to slip and fall, causing a sprain in EE's back and hip.
Unintentional roof fall in unused entry. Fall location is 1st West mains inby spad 325. Fall measured approx. 16'W x 70'L x 5.5' high. Area was supported by 48" resin bolt. No injuries, no equipment damaged & ventilation not interrupted. Fall was dangered off. No plan of moving the fall at this time. Time of the fall was approx. Fall was discovered later during day. Inactive mine.
An unintentional Roof Fall occurred in unused entry. The fall location is 1st West Mains, inby spad 311. The fall measured approximately 19' wide X 20' long X 5' high. Area was supported by 48" resin bolt. There was no injuries, no equipment damaged, and ventilation was not interrupted. Fall was dangered off. There is no plan of moving the fall at this time.
Welding on Metal and received Flash Burns to Eyes
EE attempted to mount, Cat 789C, Rock Truck, when the second step on the access ladder broke. EE held all his weight with his right arm, he felt a pull in his right shoulder and pain down his right hip & leg. EE felt at the time he didn't need medical attention. ON November 6, 2015 EE having difficulty with his neck, shoulder and lower back, sought medical treatment.

Upon release from his illness he was unable to RTW. It was determined to be related to the incident of 7/8/14.

At 11:45 am a roof fall was discovered at Break #50 on the track by the examiner and called outside. The fall measured approx. 2 ft thick, 8 ft wide, and 5 ft long. No employees, equipment or air were effected. No roof support was effected, fell between the row of bolts. Travel was available around the fall. The fall was cleaned up and the area re-supported.
