Employee said, when EE was dismounting rock truck, EE slipped on the first rung of the ladder and fell on left side. Under Investigation.
THE EMPLOYEE WAS OPERATING A 785C ROCK TRUCK DOWN A HAUL ROAD WHEN EE RAN OVER A ROCK. THE IMPACT BETWEEN THE ROCK AND THE ROCK TRUCK JOLTED EE, RESULTING IN NECK AND BACK PAIN .
Security officer went to use porta potty located on site. While stepping in floor was covered in ice causing the officer to slip and fall, causing a sprain in EE's back and hip.
Unintentional roof fall in unused entry. Fall location is 1st West mains inby spad 325. Fall measured approx. 16'W x 70'L x 5.5' high. Area was supported by 48" resin bolt. No injuries, no equipment damaged & ventilation not interrupted. Fall was dangered off. No plan of moving the fall at this time. Time of the fall was approx. Fall was discovered later during day. Inactive mine.
An unintentional Roof Fall occurred in unused entry. The fall location is 1st West Mains, inby spad 311. The fall measured approximately 19' wide X 20' long X 5' high. Area was supported by 48" resin bolt. There was no injuries, no equipment damaged, and ventilation was not interrupted. Fall was dangered off. There is no plan of moving the fall at this time.
Welding on Metal and received Flash Burns to Eyes
EE attempted to mount, Cat 789C, Rock Truck, when the second step on the access ladder broke. EE held all his weight with his right arm, he felt a pull in his right shoulder and pain down his right hip & leg. EE felt at the time he didn't need medical attention. ON November 6, 2015 EE having difficulty with his neck, shoulder and lower back, sought medical treatment.
Walking toward equipment to services and stepped in a drill hole.
While adjusting rear brakes on Mack Lube truck, the air pressure dropped low enough to engage parking brake causing sudden shift of slack adjuster pinning right pinky finger between slack adjuster and rear axle mashing finger causing laceration.

He was holding a piece of metal for Guyan Heavy equipment employee when the crane operator made a sudden move and the plate shifted weight.
EE repairing the A/C on a Cat 789C rock truck when he attempted to ascend the equipment ladder he felt a pull in his left shoulder, no medical attention was given & EE finished his shift. *EE worked until 8/20/14 when he began missing due to a non-work related illness. Upon release from his illness he was unable to RTW. It was determined to be related to the incident of 7/8/14.
Employee was following a Tractor Trailer on the haul road and was temporarily blinded by sun glare causing him to contact the tractor trailer in the rear end.

The injured was behind mechanic truck #651 when the 854 G push-cat struck the mechanic truck and pushed it into 789 C rock truck #597 injuring his right forearm that was caught between the mechanic truck bumper and the wheel of the rock truck.

The continues miner was shearing a corner when the cutter head struck a rib bolt breaking the head off and sending it across the mining machine striking ee on his upper lip resulting in a laceration that required 9 sutures.

A rock fall was discovered in the Intersection behind the power center at #2 break in the #5 entry along the #2 belt the fall was estimated to be 5' thick X 18' wide X 20' long no injuries and no ventilation controls or equipment was affected. This fall will not be cleaned up and the area has been cribbed/timbered and dangered off.
A rock fall was discovered in a cross cut at 7 break of the 1st east mains between the #3 and #4 entries the fall was estimated to be 18' long X 15'wide X 5' high no injuries and no ventilation or equipment was effected. The fall will be cleaned up.
A rock fall was discovered at 11 break in the #2 and #3 entry (1 break outby 12A seal) the fall was estimated to be 75' long X 20' wide X 4' thick the area had been supported with 4' fully grouted bolts. no injuries, ventilation was not interrupted and no equipment was involved. the fall will be timbered and dangered off. the fall will not be cleaned up.
The driver was beginning to dump in the top truckdump at the Consol No. 1 plant and while dumping the truck turned over.

Rock fall was discovered in the intersection at 50 break in the #3 entry along the #1a belt. The fall was 5' high x 20' wide x 30' long. No injuries and no ventilation controls or equipment was effected. This fall will be not be cleaned up and the area has been dangered off.

An unintentional roof fall occurred on the #4 belt in the #4 entry 30 feet inby spad #1025between the blocks. estamated size of fall 15'L. X 18' W. X 4-5' H. Area had been supported with 4' fully grouted resin bolts. No injuries occurred, ventilation was not interrupted, conveyor belt was caught. Fall will be cleaned up.
An unintentional roof fall was discovered in the #2 entry (return) at #36 break spad #676 adjacent to the 1A belt. Size of fall was estimated to be 30'L. x 19'W. x 4-5'H. Fall area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught. Ventilation was not interrupted. Fall will not be cleaned up.
An unintentional roof fall occurred in the #1 entry inby the last open break in the face. Estimated size of fall 40'L x 18'H x 4'-5'. Area had been supported with 4 foot fully grouted resin bolts, no injuries, no equipment caught, ventilation not interrupted. Fall will be cleaned up/or dangered off and developed around the fall area.
An unintentional roof fall was discovered at the 105 break in the #4 entry south mains, fall was estimated to be 30'Lx3'-4'Hx18'-19'W. The area had been supported with 4' fully grouted resin bolts, no injuries, no equipment caught, ventilation was not interrupted. Fall will be cleaned up this mine is in a non-producing status.
A rock fall was discovered in the #6 entry between #28 and #29 crosscut one break inby spad #501. Size of fall was estimated to be 30"L x 18'W x 9'-10'H. The area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will not be cleaned up.
A rock fall occurred on the 1A belt between #29 and #30 break in the #4 entry inby spad #530. The size of the fall was estimated to be 20'-25'L x 18'-20'W x 5'-6'H. The area had been supported with 4' fully grouted resin bolts. No injuries, ventilation was not interrupted, belt line was caught. Fall will be cleaned up.
An unintentional roof fall was discovered at #40 break in the #1 entry off 002 section. Size was 5.5' H. X 25'L. X 25'W. Area had been supported with 4' fully grouted resin bolts, no injuries, no Eq. caught, ventilation was not interrupted.
At 11:45 am a roof fall was discovered at Break #50 on the track by the examiner and called outside. The fall measured approx. 2 ft thick, 8 ft wide, and 5 ft long. No employees, equipment or air were effected. No roof support was effected, fell between the row of bolts. Travel was available around the fall. The fall was cleaned up and the area re-supported.
