On 7/9/2018 employee was pulling on a suction hose and hose was caught on something in the water resulting in a low back strain. First day missed was 7/16/2018.

Miner was exiting dozer using three points of contact. When stepping onto track, his knee "popped" causing knee injury.
Employee was removing a section of piping on a chemical delivery system to repair a leak. As employee made an incision into the pipe, material sprayed from the piping onto the employee's face, neck and right shoulder. The piping was not relieved of pressure or drained prior to cutting
Miner was inspecting beltline when coal build up was noticed on a roller. Using a wrench to free the roller from debris his glove and finger were caught into roller and belt.
While performing maintenance on truck, the truck fell from hydraulic jack onto the miner.
While mounting a dozer the miner's knee collapsed.
The miner was traveling on a downhill slope and lost control of tractor and collided into berm at the bottom of the slope resulting in his head striking the air conditioner vent inside the cab.
While installing equalizer bar pin in dozer with a hydraulic porta-power the victim hit pin with sledge hammer dislodging porta-power which struck victim in face.

Victim was loading brush pile with shovel onto end dump. A tree limb fell off truck stirking left side cab window on shovel causing the glass to shatter. He suffered cuts on his left eye.
Victim rolled truck on its side while returning on haul road to shovel after experiencing steering failure, incurring bruising on left side of body and head.
During a previous incident, victim was standing on a ladder working on front of dozer. Victim felt discomfort in right shoulder. On 4/16/07 victim visited doctor for x-ray and MRI and physical therapy was ordered.
Victim was greasing boom of the loader when he stepped backward, missing his step point and fell to the ground approximately 4 feet.
Prior incident occurred when operator was dismounting truck and leg became lodged between door & catwalk. First aid only required with no lost time. Medical attention was received on 3/12/07 resulting in scheduled surgery.

During investigation marijuana & methamphetamine were found.
