On 7/9/2018 employee was pulling on a suction hose and hose was caught on something in the water resulting in a low back strain. First day missed was 7/16/2018.

Employee strained shoulder while tightening hose on brake valve.

Miner was exiting dozer using three points of contact. When stepping onto track, his knee "popped" causing knee injury.

Employee was removing a section of piping on a chemical delivery system to repair a leak. As employee made an incision into the pipe, material sprayed from the piping onto the employee's face, neck and right shoulder. The piping was not relieved of pressure or drained prior to cutting

Miner was inspecting beltline when coal build up was noticed on a roller. Using a wrench to free the roller from debris his glove and finger were caught into roller and belt.

While performing maintenance on truck, the truck fell from hydraulic jack onto the miner.

While mounting a dozer the miner's knee collapsed.

The miner was traveling on a downhill slope and lost control of tractor and collided into berm at the bottom of the slope resulting in his head striking the air conditioner vent inside the cab.

While installing equalizer bar pin in dozer with a hydraulic porta-power the victim hit pin with sledge hammer dislodging porta-power which struck victim in face.

While lifting track frame with overhead crane, the miner's finger was pinched by a roller on the track frame.

While climbing on to a step ladder to clean air conditioner core, the step ladder collapsed resulting in the miner falling to the ground striking head and back.

Preparing to unload drill steels from truck when peg came out causing one side of drill steel to fall off truck onto steel toe of boot and roll back onto foot causing broken tip of big toe and laceration on bottom of foot due to pressure of steel on foot.

While dismounting lube truck, the miner's right leg came in contact with step on truck resulting in a laceration onto lower leg.

While driving a pin into equalizer bar with a hammer, a fragment of pin entered into right leg of miner.

The miner raised the bed of the truck he was operating under high voltage electric line creating an electrical arc from the line to the truck. The operator dismounted truck, while on the ground two right rear tires blew out send a projectile into right leg.

While operating dozer, miner struck elbow against arm rest requiring only first aid at this time. Due to lack of improvement of bruised area a return visit to doctor on 9/6/11 required draining of fluid from elbow.

While placing a metal plate onto coal hopper to be welded the miners left little finger was pinned between metal plate and coal hopper.

While mounting truck the miner lost the point of contact by missing handrail with his left hand resulting in him falling to the ground.

While dismounting truck, the operator slipped on top step loosing footing and fell to the ground.

EE was striking track roller on D-11 dozer with a hammer when a fragment of steel splintered off and lodged in his left arm.

While using a wrench to remove filter housing the bolt broke loose causing victims elbow to hit hydraulic pump.

Victim reached to remove gloves from heater when his hand came in contact with fan blades resulting in laceration of right ring finger.

While installing track roller on dozer the roller rolled onto EE's right hand, breaking his thumb.

Miner was performing preshift inspection. While walking around truck he slipped, falling onto his right ankle and foot. Resulting in broken foot.

Driver was traveling down ramp reforming to shovel when he made contact with highwall with right front tire of truck overturning on left side.

While positioning chain breakout wrench onto drill steel the employee's middle finger on his left hand was pinched between wrench & drill steel.

While mounting haul truck the miner missed the top handhold on the ladder causing him to fall to the ground.

Miner was holding metal pin while another mechanic was striking pin with a sledge hammer. A small piece of metal splintered from pin striking miner in chin area of the face.

While using a cutting torch, victim's left hand was burned when hose on torch failed.

1 1/2" piece of angle iron (which supports walkway sections) failed allowing miner to fall through walkway to concrete floor below (approx 14'6" distance).

EE cut finger while closing sliding glass window.

While changing right rear tire on Dodge pickup truck, sidewall blew out injuring right wrist of employee.

Injured person was carrying grease pump to remove it from the bed of service truck. When stepping down he felt pain in lower back.

Victim was changing air bag on back tandem of wheels on Kenworth tractor. While loosening bolts on clamp of brake chamber, the back section separated from chamber, striking victim above & below left eye. Safety glasses were worn.

Victim was loading brush pile with shovel onto end dump. A tree limb fell off truck stirking left side cab window on shovel causing the glass to shatter. He suffered cuts on his left eye.

Victim rolled truck on its side while returning on haul road to shovel after experiencing steering failure, incurring bruising on left side of body and head.

During a previous incident, victim was standing on a ladder working on front of dozer. Victim felt discomfort in right shoulder. On 4/16/07 victim visited doctor for x-ray and MRI and physical therapy was ordered.

Victim was greasing boom of the loader when he stepped backward, missing his step point and fell to the ground approximately 4 feet.

Prior incident occurred when operator was dismounting truck and leg became lodged between door & catwalk. First aid only required with no lost time. Medical attention was received on 3/12/07 resulting in scheduled surgery.

Victim stepped from stepladder onto steel beam under coal breaker while working on belt line, losing balance after step and fell to the ground.

Victim was changing hose on D-11 blade hoist cylinder, slipped and fell to the ground.

Victim was exiting mechanics service truck onto rough road surface and stepped on large rock twisting right ankle.

While driving on haul road, truck slid into berm turning wheels and steering wheel rapidly. Victim's hand became entangled in steering wheel breaking wrist.

Grinding on steel beam with electric grinder and foreign object flew to the side or under safety glasses and lodged in left eye.

Traveling down haul road ramp into pit 5 and failed to negotiate curve at the bottom of the ramp. Proceeded through berm adn over approximately 12 foot high spoil wall over turning truck on it's side causing injury to head and chest. Appeared to have fallen asleep. During investigation marijuana & methamphetamine were found.

Climbing onto Service Truck bumper striking head on truck crane causing pain to the neck.

Pulling hose to pump when felt pain in shoulder.

Lifting pad and replacing it on dozer when lost footing and twisted knee.

Dismounting equipment, he stepped onto the ground; his foot slipped on a rock twisting his knee.

RETURNING DOWN RAMP WHEN APPLIED BRAKES CAUSING HIM TO LOSE CONTROL AND STRIKE LOADED 785 END-DUMP.
