Pulling rock off of picking table and another rock fell on fingers.
Received a citation from the local MSHA office stating alleged accident occurred 6 months ago. No record of accident. Mine operator nor co-workers were aware an accident occurred. Attorney will be contacted to handle the denial of the claim. Employees are instructed to inform their supervisor immediately after the occurrance of an accident.
Rock caught in tailpiece. Employee used sledge hammer to try to loosen rock and another rock struck ring finger of right hand.

EE was maiking a belt splice. A clamp holding the belt came loose and struck him in the face and mouth, resulting in some stitches inside and outside of his mouth, chipped tooth and loose tooth.
Had a hoist hooked up to a speed reducer to a scoop. The arm of the hoist broke hitting employee on the arm.
Employee was tamping when he went to foreman stating this his side was hurting bad and was getting weak. He said he was going to have to go home and go to the doctor next day. He left at 1 am.
Was operating roof bolter. A rock 1 foot round and 1.5 inches thick fell hitting his finger and cutting it.
Employee was backing roof bolter out of section three left. He had his left arm out of deck, while holding cable out of way. His left arm got caught between the roof bolter and rib.
Injured was observing work being performed by co-worker on cutting machine. Grinding dust and metal went into injured's left eye.
EE was spinning steel out of top of bolter. His arm accidentally hit the up level on the equipment, pinning his index finger against the roof and the machine.
EE was using a hammer on a piece of metal, rust flew into his right eye.
Riding on a electric buggy when he hit his head on the roof causing injury to head, neck and back.
EE WAS OPERATING A BUGGY WHEN HE SAW A SCOOP STUCK. HE GOT OFF THE BUGGY AND HELPED GET THE SCOOP UNSTUCK. WHILE HE WAS CRAWLING BACK TO HIS BUGGY, A LARGE PIECE OF DRAW ROCK FELL ON HIM.
EE WAS SWINGING A SLEDGE HAMMER WHILE BUSTING UP ROCK AND HIS RIGHT SHOULDER CAME OUT OF THE SOCKET.
EE WAS TAMPING A FEEDER HOLE WHEN HE TWISTED AND FELT PAIN IN HIS LOWER RIGHT HIP THAT RADIATES DOWN HIS LEG AND CAUSES PAIN AND NUMBNESS IN HIS RIGHT FOOT.
EE TRAMMED A 482 SCOOP INTO THE #10 HEADING WHEN THE SCOOP QUIT. HE GOT OFF, TOOK THE PANEL BOARD OFF & BEGAN TO WORK ON CAPACITORS. HE STATES THERE WAS A FLASH OF LIGHT AND HE GOT A FACIAL BURN. EITHER DURING THAT TIME OR SOMETIME AFTER, AN IGNITION OCCURRED IN THE AREA TRAVELING ACROSS THE LAST OPEN CROSSCUT FROM #10 TO #8 & OUTBY THE OPEN CROSSCUT A DISTANCE OF 2 CROSSCUTS.
EE WAS BACKING CUTTER OUT OF #10 HEADING AND RAN OVER LOOSE COAL CAUSING HIM TO JAM HIS HEAD INTO THE ROOF INJURING HIS NECK AND SHOULDERS.
EMPLOYEE WAS TRAMMING ROOF BOLTER WHEN RIB FELL INJURING RIGHT LEG.
EMPLOYEE WAS REACHING TO PLUG SCOOP BATTERIES IN AND HURT HIS BACK.
