Showing all 32EE was removing a nylon strap (metal hook on both ends) The strap twisted imbedding the metal hook in the left nostril (nose).
While extracting partial pillar recovery #6 entry the 2nd right cut out of the pillar an unintentional roof fall trapping our continuous miner under approximately 4' to 5' of roof. This area appeared to have laminated roof condition.
While handling wooden crib blocks EE mashed his middle finger on left hand injuring the fingernail of his middle finger.
Employee stated the brakes locked up on #2 shuttle car, the car slid in to the rib corner.

Employee was operating the roof drill, he alledged that he pulled the wrong lever and the drill head hit his right leg.
Belt foreman was jacking up tailpiece to put a header board under it. He was cleaning out under the tailpiece foot when the jsck kicked out, let the tailpiece drop down.
Employee was operating the fletcher roof drill he let the drill head down on his right ankle.
EE was sitting in the deck of his shuttle car, he had his left foot outside the deck. A piece of rock rolled over on his foot, his shuttle car was not in use at the time.
Employee was trying to move a rock that was hung in the section tailpiece with a piece of pinner steel. The steel slipped & one end hit the employee in the chest.
Employee was setting a hydraulic tailpiece jack at the section tailpiece. He alledged that a piece of rock broke loose from the roof when pressure was applied to the jack, stiking him on the left shoulder.
Ee was rolling a wheel unit over to the shuttle car when lifting and pulling on unit felt pain in lower back.
EE SAID HE WAS UNLOADING BELT STRUCTURE FROM THE SCOOP BUCKET AND HE PULLED SOMETHING IN HIS BACK.
EMPLOYEE WAS INSTALLING BELT HANGERS. HE SWUNG THE HEAD OF THE ROOF BOLTER OVER TOO FALL STRIKING HIMSELF IN THE RIBS WITH THE HEAD TRAY.
EMPLOYEE WAS HOOKING UP CHAIN TO LIFT ROOF BOLTS UP WITH THE ENDLOADER, THE OPERATOR WAS GOING TO BACK THE LOADER UP, WHEN THE LOADER WENT FORWARD STRIKING EMPLOYEE ON THE RIGHT LEG.
AT SPAD #1486 ROCK FALL IN #6 ENTRY ON MAINS EXTENDED INBY FOR A DISTANCE OF APPROX 80' - 20' WIDE AND 5' THICK. AREA HAD ALREADY BEN TIMBERED AND DANGERED OFF PRIOR TO FALL. APPROX 650' OUTBY WORKING SECTION - SECTION IS ON RETREAT MINING.
THE SECTION FOREMAN WAS HELPING MOVE THE CONT. MINER TO ANOTHER WORKING PLACE WHILE WATCHING THE MINER CABLE A PIECE OF ROCK FELL FROM BETWEEN THE ROOF BOLTS STRIKING HIM ON THE RIGHT SHOULDER AND ARM.
ROOF BOLTER OPER. WAS INSTALLING A ROOF BOLT IN THE ROOF-HE PUSHED IT UP ABOUT 2/3 OF THE WAY THE BOLT BENT. HE SET THE MACHINE OVER TO PUT UP ANOTHER BOLT HE PUSHED THE LEVER THE WRONG WAY-TH E HEAD CAME OVER ON HIS LEFT KNEE.
APPROX 20' IN BY SPAD 1334 IN #5 ENTRY ON MAINS, A ROOF FALL OCCURRED, THE FALL LOOKS TO BE APPROX 0-5' THICK IN BY RANGES UP TO 5' THICK FOR A DISTANCE OF APPROX 230' IN LENGTH.
APPROX. 15' INBY SPAD #1359 IN #4 ENTRY ON MAINS(BELT) A ROOF FALL WAS DISCOVERED DURING THE PRESHIFT. FALL MEASURED ~20' WIDE; STARTS OUT ~3'ONONE END & ~5' THICK ON THE OTHER FOR A DISTANCE OF ~170' IN LENGTH. ROOF WAS BOLTED WITH 4' RESIN RODS.
CONT. MINR OPERATOR WAS CUTTING THE #4 RIGHT CROSS CUT THEN HE HAD HIS BACK TURNED TOWARD THE RIB. WHEN A PIECE OF ROCK ROLLED FROM THE RIB STRIKING HIM IN HIS LOWER BACK.
DISCOVERED A ROOF FALL ON THE INTAKE IN #3 ENTRY APPROX 400' UNDERGROUND FROM THE SURFACE AT SPAD #15. THE FALL WAS IN AN AREA THAT HAS BEEN CRIBBED OFF FOR APPROX 2 MONTHS OR LONGER. THE FALL IS APPROX 12'W, 5'H, 15'L. THIS ENTRY WAS NOT BEING USED AS A TRAVELWAY.
EE SAW THE CIRCUIT BREAKER FOR THE ROOF BOLTER HAD DROPPED OUT-WHEN HE WENT TO RESET IT-IT WENT PHASE TO PHASE AND BURNT HIS FINGERS.
