Changing conveyor pot on cont. miner, using slate bars to remove CLA disc from pot. Laid a slate bar on the ground after removal of disc. Disc slid off conveyor pan catching end of bar causing it to flip up and strike employee in cheek causing cut requiring 8 stitches.
Miner was operating a shuttle car with a hand resting on the side rail of the cab when a rock was dislodged from the floor of the mine, striking the miner's middle and ring finger on left hand.
While working on the C.O. Monitoring system the employee slipped forward. While off balance, reached for the C.O. cable pulling muscles in shoulders and back as well as thinking they may have torn rotator cups on both sides.
Employee was setting pump in water. As they turned after setting pump, they twisted lower back resulting in a strained muscle.
Employee picked up a belt rail to remove it from around the feeder dump area and pulled muscles in left shoulder.
Employee struck coal rib while operating a permissible scoop. Afterwards complained of pain in EE's neck. Employee worked until 7/12/19 then sought medical attention.
First intersection inby intake portal fell approx. 8 ft. high, 16 ft. wide, 23 ft. long. Fall did not affect ventilation, all employees were immediately evacuated from the mine. No injuries resulted from this event.
Roof bolter operator was swinging drill head to install a roof bolt when he caught his finger between the boom arm and drill pot of roof bolter.
Roof fall out-by, neutral entry , aprox 32 breaks out-by face area. Ventilation was not disrupted. Fall occurred during idle weekend.
Sometime during Christmas break a rock fall occurred in the left return at just inby SS# 2577. The fall was dangered off, timbered off and the travelway was re-routed around the fall. The fall was approx. 80' long, 20' wide, and 7' high.
Employee was tightening bolts on fan motor. Base moved and motor fell on left hand. Fractured left middle finger, cut ring finger, middle finger and pinky finger. Employee had to have stitches in all three fingers.
EE was going from feeder to miner. He put his hand under the shuttle car while moving.
EE said he picked rock up and strained back.
Employee claimed he was reaching for supplies when his knee slipped and slid down between tire and frame of the roof bolter he was operating resulting in his knee being twisted. The roof bolter was stationary at the time of injury.
Employee was cleaning #7 entry with scoop in low coal seam. While moving forward with scoop, he struck coal rib with bucket causing scoop to buckle somewhat in the center section causing him to strike his head on the mine roof resulting in employee complaining with pain in back of neck.
Employee started complaining of knee pain to the left knee.
Employee was loading parts and supplies, tripped and fell with a hydraulic boom jack in his hands. When he fell he caught his finger between the jack and the metal frame of a Mac 8 transport buggy resulting in a fractured fingertip and cut.
Injured EE and another EE were drilling an anchor hole in the mine floor to anchor belt tail piece with a hyd drill. The augar got entangled in a tool pouch, hanging from the other EE's belt, causing injured EE's fingers to become twisted around the handle of the drill. This resulted in a fracture of the middle and ring fingers of the right hand.
Employee was installing roof bolts and hit wrong lever to push bolt into place.
EE was installing roof support and puthand over plate while applying up pressure on machine.
Employee was hauling supplies on 001-0 section when he ran over rock in roadway causing bruise to lower back.
Miner operator was loading coal into shuttle car. When the car moved forward, the tire rolled into miner operators foot. (Remote miner) This resulted in bruising around the ankle and side of foot.
Employee and section foreman was attempting to move miner cable out of the haulageway. When He pulled on the cable, he sprained his back.
EMPLOYEE WAS INSTALLING ROOF BOLTS & AS HE STARTED TO DRILL A HOLE A PIECE OF DRAW ROCK APPROX. 1.5"THICK X 1'WIDE X 2'LONG, DISLODGED & FELL STRIKING HIM ON THE LEFT ELBOW RESULTING IN DEEP BRUISING.
Employee was adjusting belts on blower motor on the No. 2 Roofbolter and placed hand on belt to ck tension.
AFTER BEING LOADED BY THE CONTINUOUS MINER, THE S/C OPERATOR PROCEEDED OUT OF THE #7 HEADING TOWARD THE DUMPING POINT. AS HE MADE THE TURN AROUND THE CORNER THE REAR OF THE S/C STRUCK EE WHO WAS KNEELING WITH HIS BACK TURNED AWAY FROM THE OPERATOR & S/C HANGING LINE CURTAIN THAT HAD FALLEN DOWN.
Employee had foot under head boom of roof bolter and let roof drill down on foot causing bruise to hill area of his foot.
Employee had his hand in way when pushing steel in roof to drill for bolt.
EMPLOYEE WENT TO BATH ROOM AND WHILE RETURNNG STEPPED ON LOOSE ROCK AND STRAINED ANKLE.
EMPLOYEE WAS OPENING DORRS AT FAN HOUSE AND LATCH BROKE LETTING THE DOOR OPEN PREMATURELY.
EE WAS RIDING THE MANTRIP FROM THE FACE AREA TO THE SURFACE. HE ALLEDGES THAT THE MANTRIP HIT A HOLE IN THE TRAVELWAY AND BOUNCED CAUSING INJURY TO HIS BACK. THE INVESTIGATION REVEALED THAT TH E MANTRIP WAS NOT TRAVELING AT AN EXCESSIVE RATE OF SPEED.
ROOF FALL OCCURRED OUT BY FACE APP 1200 FT ON INTAKE HALLWAY, CAUSE IS BELIEVED TO BE OF MATERIAL FAILURE.
WHILE MOVING BELT, EMPLOYEE ALONG WITH ANOTHER EE ATTEMPTED TO LIFT A SECTION OF BOTTOM BELT IN ORDER TO ALLOW INSTALLATION OF BELT STRUCTURE, RESULTING IN A STRAINED MUSCLE IN HIS SHOULDER.
VICTIM WAS HELPING DRILL ANCHOR HOLES FOR SECTION TAILPIECE WHILE DRILL WAS IN MOTION HE REALLED DOWN TOCLEAN DUST FROM HOLE DRILL STEEL CAUGHT HIS SHIRT GLEEVE AND WRAPPED UP IN STEEL RESULTI NG IN FRACTURE TO RT FOREARM.
EE WAS MOVING ROOF BOLTER FROM #2 LEFT ENTRY TO #3 HEADING. HE PLACED HIS HAND ON DRILL POT AND BY MISTAKE, HIT THE FAST FEED UP LEVER ON THE CONTROLS. THE DRILL POT RAISED PINCHING HIS RT IND EX FINGER BETWEEN THE DRILL POT AND OPERATORE CANOPY.
THE EE WAS INSTALLING ROOF BOLTS. HE WAS USING THE DRILL HEAD OF THE MACHINE TO PUSH THE 42" FULLY GROUTED BOLT IN THE DRILLED HOLE IN THE MINE ROOF WHEN THE BOLT SLIPPED OFF THE DRILL HEAD ST RIKING HIM IN THE FINGER.
A METAL ROOF MAT OR STRAP HAD BEEN DISLODGED ON ONE END EVIDENTALY BY THE CONTINUOUS MINER OR SHUTTLE CAR. THE STRAP WAS HANGING DOWN FROM FOOR AND THE EMPLOYEE WALKED INTO IT.
EE WAS HELPING THE REST OF THE EES TO ADVANCE THE BELT WHEN HE TRIPPED & FELL STRIKING HIS KNEE ON THE MINE FLOOR.
EE WAS UNLOADING TIMBERS FROM SCOOP BUCKET @ DIFFERENT LOCATIONS IN FACE AREA. WHILE DOING SO, HE FELT PAIN IN LOWER BACK AREA AT 8:18 AM. HE INFORMED SECTION FOREMAN LATER & WAS TAKEN TO SEEK MEDICAL ATTN. THE EMERGENCY ROOM PHYSICIAN SAIDHE PULLED MUSCLE IN LOWER BACK.
EMPLOYEE WAS HANGING MINER CABLE AND WATERLINE IN #3 ENTRY WITH OTHER EE. HE SAID LATER HE FELT STOMACH AND BACK HURTING.
EMPLOYEE WAS RIDING MANTRIP TRANSPORT CART WHEN HIS HARD HAT FELL OFF. HE STRUCK HIS HEAD ON THE MINE ROOF CAUSING INJURY TO THE TOP OF HIS HEAD RESULTING IN 5 STITCHES OR CLAMPS.
VICTIM WAS INSTALLING ROOF BOLTS IN #4 ENTRY RT. XCUT WHEN HE SET PERSONAL CANOPY AGAINST MINE ROOF THE BOTTOM PLATE THAT SUPPORTS THE JACK BROKE, CAUSING IT BREAK LETTING THE CANOPY TO FALL S TRIKING HIM THE HEAD & STOVING HIS NECK UP.
VICTIM WAS CRAWLING WHEN SCOOP TRAMED BY RUNNING OVER LUMP OF COAL LUMP OF COAL FLIPPED OFF SCOOP TIRE HITTING HIM IN LEFT EYE.
EE WAS INSTALLING ROOF BOLTS IN THE LAST ROW OF #2 HEADING HE WAS USING SECTIONAL STEEL TO DRILLTHE LAST 18" OF THE HOLE & PINCHED THUMB OF RIGHT HAND BETWEEN THE 2 PIECES OF STEEL WHEN HE APP LIED UP PRESSURE ON THE DRILL HEAD OF THE BOLTERGLOVES WERE BEING WORN WHEN THE ACCIDENT HAPPENED
VICTIM PLACED HAND ON DRILL FRAME OF PINNER WHILE LETTING IT DOWN CAUGHT HIS RT. THUMB MASHING IT.
REMOVING TINE FROM ROOF BOLTER WITH RATCHET & SOCKET. LOOSENING LUG NUTS WHEN RATCHET SLIPPED & SOCKET BUSTED CAUSING VICTIM TO STRIKE BOLTER FACE FIRST CAUSING LACERATION ABOVE RIGHT EYEBROW REQUIRING 5 STITCHES.
EE ALLEDGED THAT HE TRIPPED OVER MINER CABLE AND FELL ON RT KNEE.
