EE WAS AT NO. 2 BELTDRIVE RECOVERING WATER LINE WHEN HE REMOVED THE CLAMP THE END OF THE WATERLINE CAME UP, HITTING HIM IN THE MOUTH, KNOCKING 2 FRONT TEETH OUT. DR. PUT THEM BACK.
FALL IS LOCATED IN #7 ENTRY 1 BREAK ABOVE SPAD 369 ON THE RIGHT RETURN. MEASURED 30'L, 20'W, 5-6'THICK. WAS TIMBERED AND DANGERED OFF. INVESTIGATED BY MSHA.
EE WAS TAKEN SIDE RAIL OFF 14-5 CM WHEN THE RAILCAUGHT HIS LEFT KNEE CAUSING HIS INJURY.
EE WAS RIDING IN ON MANTRIP AT START OF SHIFT WHEN HIS HEAD CAUGHT CROSS BEAM AT THE AIR LOCK DOOR LOCTED BETWEEN THE 10TH & 11TH BREAK ON NEUTRAL ENTRY WHICH IS THE TRAVEL WAY IN & OUT EE WAS DRAGGED BACKWARDS OVER THE MANTRIP CAUSING HIS INJURY.
EMPLOYEE WAS TRAMMING 14-1CM TOWARD OUTSIDE. WHILE TOPPING AN INCLINE HILL, THE MINER LEVELED OFF AND HIS HEAD CAME IN CONTACT WITH THE ROOF CAUSING HIS NECK INJURY.
THE FALL MEASURED 45 FT LONG 20 FT WIDE 35 INCHES THICK IN 3 HEADING INBY SPAD NO. 733, THE MSHA INSPECTOR HOBERT WEBB INVESTIGATED THE FALL. CAUSE DETERMINED OF THE FALL WAS FAULTY AREA IN TH E ROOF.
EE WAS HELPING WORK ON MINER. WE WERE LIFTING ON A PLANETARY AND TRYING TO PUT IT BACK WHEN EE SPRAIN HIS BACK. HE WORKED THE REST OF THE SHIFT BEFORE HE DECIDED TO GO TO THE DR. DR PAGE WAS T HE DR HE SEEN IN PIKEVILLE.
EE STATED TO CO-WORKER, THAT HE WAS ON HIS KNEESCHANGING DIRECTIONS WITH HIS SEAT LEAVING THE MINER, TO GO TO THE FEEDER TO DUMP HIS CAR WHILE TURNING HIS BODY SWISTING HIS LEFT KNEE CAUSING H IS INJURY EE FINISHED HIS SHIFT BEFORE HE DECIDED TO GO TO THE DOCTOR.
EMPLOYEE WAS HELPING HIS PARTNER TO CLEAN HIS DUST HOSE OUT GOING TO HIS DUST COLLECTOR WHILE HIS PARTNER RAISED THE ARM BOOM OF THE BOLTER UPWARD CATCHING HIS LEFT INDEX FINGER BETWEEN THE AR M BOOM AND BOOM JACK CAUSING HIS INJURY.
THE SAID EE WAS ROCK DUSTING RIBS INBY LAST OPENCROSSCUT IN #6 ENTRY WHEN HE PICKED UP A BAG OF ROCK DUST IN BENT OVER POSITION AS HE TURNED HE COMPLAIN WITH LOWER BACK PAIN. HE WENT TO THE DR & WAS RELEASEAD ON TAHE NEXT DAY VISITED.
THE SAID EE WAS TURNING RIGHT BREAK IN #6 HEADING WHEN PARTICLES OF COAL AND ROCK CAME FROM CONVEYOR OF THE MINER CAUSING HIS INJURY.
THE ROOF FALL IS LOCATED ON THE MAINS IN THE #7 RETURN AT SPAD #315 OUTBY FACE AREA, 9 BREAKS FROM THE FACE. THE FALL HAS BEEN CRIBBED OFF WITH RETURN AIR STILL FLOWING ITS PROPER COURSE. THE FALL WAS SHELL TOP MEASURING APPROXIMATELY 42"H 18'W 60'L.
EE WAS INSTALLING ROOF BOLTS WITH DOUBLE HEAD FLTCHER, OFFISDE WHEN PUSHING UP A BOLT AND PLATE HE CAUGHT HIS FINGER BETWEEN THE DRILL POT ON THE BOLTER AND THE ROOF BOLT PLATE.
INJURED WAS TAKING UP CENTERS IN NO. 6 HEADING WHILE 2"-3" OF SHELL ROCK FELL FROM THE ROOF CAUSING HIS BACK INJURY HE WAS TAKING TO OUR LADY OF THE WAY HOSPITAL IN MARTIN, KY. BY AMBUL ANCE E.R. DR. PUIG STATED THERE WAS NO BROKEN BONES ONLY BRUISED AND SORE.
EMPLOYEE STATED THAT HE TRAMMED HIS 21-SC SHUTTLE CAR DOWN AN INCLINE TOWARD THE BACK OF A 14-CM MINER,THAT HE COULD NOT STOP THE SHUTTLE CAR,AND WHEN IT WAS INVESTIGATED THE DAYSHIFT MECHANIC FOUND THE BRAKE PEDAL WAS GOBBED BENEATH IT WITH LOOSE COAL.
EE STATED THAT HE WAS SAWING A TIMBER TO BE INSTALLED ON #2 BELT CONVEYOR LINE WHEN HE CUT HIS FINGER.
SAID EE STATED THAT HE WAS COMING AROUND CORNER OF #6RB RIGHT BREAK WHEN HIS STEERING CAUGHT CAUSING THE SHUTTLE CAR TO RUN INTO RIB CAUSING HIS INJURY.
EMPLOYEE WAS MOVING TH EPERSONNEL CARRIER AWA FORM SHOP DOOR WHEN HE TURNED DIRECTIONAL SWITCH ON HE STATED THAT IT WS STUCK ON POINT AND WENT THROUGH THE SHOP DOOR WITH HIM BRUISING HIS BACK AND CAUSING LOWER BACK PAIN.
EMPLOYEE WAS LOADING 50 LB BAGS OF ROCKDUST BY HAND INTO THE BUCKET OF A SCOOP WHILE LIFTING IMPROPER HE TWISTED HIS BACK. THE ACCIDENT OCCUR3 BREAKS OUTBY FACE IN #5 ENTRY.
EMPLOYEE WAS INSTALLING ROOF BOLTS IN THE FACE OF #6 ENTRY, HIS GLOVE WAS CAUGHT BETWEEN THE STEEL & THE TOP OF THE ROTATION POT, CAUSING HIS INJURY.
EE WAS BOLTING TOP AND AS HE BROUGHT THE DRILL STEEL FROM THE HOLE. HE LEFT THE POT ROTATION RUNNING AND THE DRILL STEEL GOT AWAY FROM HIM. HITTING HIS RIGHT ARM AND BRUISING IT.
