Member was pulling 5'roof bolts out of a bundle to load onto his tray and his back started hurting .He was seen by a specialist and told to remain away from work because of a sprain or a herniated disc in lower back.

Member was bolting in 2 left putting up the 2nd bolt he was putting glue and bolt into hole when a small rock hit the canopy and then causing a small cut to his face. He worked his next shift and then missed 2 days because he sprained his neck when he seen small rock falling

Roof Fall, Parrell 2nd northwest mains area was already dangered off prior to it falling in fall was Apprx 100'l x 5' H in between #4 and #5 enrty . MSHA has checked the area it won't be cleaned up.

EE had been shoveling on LF 1 belt as he twisted to put material on the belt with a shovel he felt pain in lower back and his left leg went numb. EE had back surgery in the past and his symptoms were the same as before the back surgery. I ask this be classified as an occupational illness because of his past history with his back.

EE was in the process of taking the wheel unit bolts out of a shuttle car he was using a impact wrench and 3/4" Allen plug to loosen the bolts someone had placed a nail were the dow pin went to hold the plug in the socket and he started the impact wrench and it caught his glove on his right hand and caused a laceration requiring 4 sutures to close the wound.

EE was standing beside the forklift as they were loading the scoop bucket with a bag of bulk dust. He reached down to get the pallet and had his right foot there also as the forklift pulled up, when he did this, he ran over his right foot causing a contusion with the forklift.

EE was placing a wrecked flatcar back on track and had hooked a chain ratchet into the eye of a roof bolt plate and the eye broke out allowing the chain ratchet to fall and hit EE in the head causing a mild concussion. He had asked off prior to the accident for a funeral on 1-28-11 and he wasn't scheduled on 1-29-11 He returned on 1-31-21.

Employee was removing a 60" top roller 2 bks out by #3 tailpiece. He had raised the belt with a chain ratchet and was pulling the top structure out when his back started hurting. He finished his shift then called in the following day stating he was going to the doctor who prescribed him physical therapy for 2 weeks because of a strain.

EE wrecked the mantrip at bk-50 on Rh 1 track he was using a 10 ton railroad jack when the bar struck him on left side of chin at his jawline he lost consciousness for a few minutes. He was taken to hospital and x-rays were negative and he was released back to work on his next shift he did return to his normal job.

Fall was found on 11/30/10 MSHA was present and notified about roof fall. It was located #1 entry between bk 12&13 on the 4th west mains. Area will be dangered off, fall will not be cleaned up MSHA has completed their investigation of the area.

Member was taking down a roof bolter cable he was standing on top of the machine he lost his balance falling to the ground landing on his rt side fracturing his ribs. He never quit working went to hospital where he was diagnoised with 2 broken ribs and allowed to return back to his job

Cleaning walkway on 6 belt bk.26 and tripped on a rock and his finger went into a top roller pushing his finger nail back, he had 1 stitch to repair the injury. This is reported late because I was unaware that we were responsible for reporting contractor accidents and have been checking msha data base to see if it has been reported by the contractor company.

Hitting insert in the continuous miner with a hammer piece of metal went into index finger. Had to be removed by a doctor embedded object. The piece of metal was removed and he returned back on his next shift without restrictions.

EE was using slate bar to hold rail over, he had placed his hand under the rail tie to hold it up against the track. The slate bar slipped allowing the rail to move which caused a laceration to his finger requiring stitches.

EE was cleaning on tailpiece on his knees out of sight of foreman. EE told foreman to throw him a crib block when he threw the crib block it struck ee in the lower back. EE wasn't scheduled to return back to rockhouse, accident occured on 7/31/09 he was to return back to solid his next scheduled shift. He suffered broken vertebra tips and hasn't returned to work.

Employee was putting up the outside bolt in the 2 left cross cut. As he swung his drill pot to install the inside bolt a piece of drawrock fell scraping his left leg and foot. He went to a specialist and was told to remain away from work. He was diagnosed with strained ligaments.

EE and another co-worker were moving the miner cable out of the roadway at Bk 11 on 8 belt in the track entry when coal rib rolled off, striking him on the left ankle, causing a bruning sensation. He was taken to the hospital where no fractures were found and he was given light duty work for 10 days. He had no laceration or markings on his ankle.

Roof fall was found by JD Adams it is on the North east mains at Bk-48 and is appr. 6 high and goes from number 1 entry to number 7 it is in aircourse that is required to be checked weekly. It's not close to any working sections.

Injury aggravated a old injury that he had prior to being employee at Rockhouse he had suffered a herniated disc at another time. He had been seeking treatment since 2005 for back problems. He started missing work on 6/2/09 after doing an investigation of this incident we feel that His continued back problems is why he became unable to work. cause him to start missing work.

The roof fall was dicovered by MSHA inspector during his weekly exam the fall was a continuation of an existing fall that had occured some time ago. The fall will be re-supported and cleaned up to extablish a travel way. The area around the fall will be dangered off to prevent access to the fall area 7 head at spad no 30063.

Member states he was helping to build a stoppings when he injured his back. He never reported the accident to the foreman nor were any of his coworkers told that he had hurt himself. He left the mine without filling out an accident report. I'm reporting this to prevent a citation. I don't think he was injured at this mine after the investigation was completed.

On April 22nd due to occupational illness, as substantiated by medical records and reports, ee became unable to work because of the ailments described in the medical reports. This is not due to one single event at the mine, but repeated trauma over an extended period of time.
