An unintentional roof fall has occurred in the no. 3 out by entry at break no. 79 in the secondary escapeway. The fall does not impede travel of miners or interupt ventilation.
Stepped out of truck onto gravel lot at mine site. Slipped and fell - felt back pain.
Employee was building cribs; had right hand on scoop bucket and crib rolled off on his right finger. Finger is broken. Splint on right pinkie and ring finger.
2,000' from section, entry 4, spad no. 604; no eqip. involved; Neutral-airway; Size 10' long, 19'wide, 4'-7' high; 48" resin, 3 cribs, type of roof support, 3 cribs prior to fall; Strata is shale, sandstone, and laminated; Mo coal rider seam; entrances are not breakered off, roof is being rehabilitated with clean timbers, floor the cribs than the roof.
Employee states, injury and other ee were loading the roof bolter, when he cut the bands on the bolts the band srung back striking him on the left hand. He was wearing gloves.
Was hammering studs in scoop tire and shoulder poped with sharp pain. Persons filed comp on 10/27. Started missing work on 10/28/08
EE stepped off back of roof bolter to hang bolter cable. While holding onto the bolter with his hand, it slipped and he fell against a pile of strap, cutting his arm.
While jacking up the jack it kicked out and stricking him on both knees and legs. Bar hit him in the chest.
Trying to pry belt clamp loose from top structure, structure belt broke. Belt and C clamp struck employee in the face, near the right eye.
Lifting blocks, to build brattice and pulled his groin. He began missing work on 10/10/08. He had surgery.
Putting conveyor chains together, smashed right ring and middle finger.
While carrying belt structure to track entry, EE got his foot stuck in mud and twisted his back.

While repairing a set of airlock doors, just inside drift mouth, employee was making a hole in the door with pick hammer to mount rubber seal. The tip of hammer broke off, impaling into lower arm. EMT dressed arm.
6/25/08 EE called to say he was unable to work. He said on 6/23 he was loading the TC sprayer (plaster machine) & liquid shot into face & eyes. I asked about safety glasses. He said he broke them, they were on his head but not on his eyes. His eyes were washed at work. He returned next day though his eyes were a little swollen. Today they're swollen worse & he is going to dr.
Employee was hanging cable inby feeder and his right hand slid across a pizza pan and sliced his fourth knuckle on right hand, causing laceration. Action to prevent recurrence is to watch the surroundings.
Prying on jack pin, slate bar slipped twisting his right shoulder. He fell to the ground on same shoulder. Began missing work on 10/12/08. He had surgery.
While laoding rock dust into scoop bucket; tied chain to metal straps turned around and the straps broke hitting EE on right side of head; making a one and a half inch cut on right side of head. Action taken in preventing reocurrence is do not hang rock dust from straps.
He was moving fly pads up and was walking back to the scoop when a rock fell out and struck his shoulder area.

Started bolting 8 right, put steel against top and the rock fell down the steel and smashed hand. Note: He said he was only bruised and took a couple of days off. He did not go to a doctor.

Employee was scattering belt structure for a belt move, threw a piece of structure under the belt. His hand got caught between roller and frame pulling something in right shoulder. Complained of pain going down arm. ***Employee did not start missing work until 07-13-2007.
