A "B" function employee was wiring a pump at the Craig Shaft, when EE stepped down off support beams for the pump, EE stepped on a the steel rope loop that is used to lift concrete slabs off a shaft and rolled EE's ankle. Cause of this injury was awareness. Compliance with rules/regs, PPE, mining systems/equipment was not a factor, miner proficiency could be a factor.

Showing 50 of 408EE was cutting grass with 7300 compact track loader w/attached brush hog. For some unknown reason the machine lost traction and slid down small embankment into water. Cause of incident unknown at this time. Compliance with rules/reg not a factor, mining equip/system unknown to be a factor, but was used, PPE not a factor, job skills/miner proficiency/training/attitude not a factor.
Upon slope exploration of this in-active mine a roof fall was found at 1220' mark in the slope. Approx. size of fall is 15' high, 19' wide and length is unknown at this time. Cause of this fall was deterioration due to being flooded with water for many years. This report is being requested by MSHA on 3-2-11.
While opening a 10" knife blade of the bin activator on a 50 ton lime silo, lime sparyed from around the opening and covered EE's head and face. The EE was wearing required PPE. While attempting to wash lime from his head, lime entered his eyes and caused irritationand minor burns.
Attempted to stop lime from spraying from the opening around the 10" knife blade valve by sealing the opening with tape. The EE was wearing a respirator, but no eye protection. The dry lime struck his repirator and entered behind it. After exiting the building, he rinsed the lime from his head and face with water. Caused irritation and minor burns to eyes and face.
Prep Plant has been idle for 18 months. A fire was started by a suspicious nature under an outside belt. No flames were seen, however, damage would indicate fire.
Rolling a roll of belt onto wagon, slipped and hit knee on wagon, felt pain but continued working.
Was wearing hip boots and was pulling 28 hp submissible pump into water and felt pain in lower back.
Loading belt rollers. Left heel caught belt and started to fall, right foot slipped and fell backwards onto ground. Felt pain in rt. hip, lower back & across shoulder. And right heel was sore.
Stepped off 20 ton motor on "A" track twisting right ankle. EE stated he heard & felt pain with snap in ankle area.
EE WAS WALKING FROM 18 X-CUT OUTBY. HE SLIPPED ON MUD AND STRUCK HIS LEFT KNEE ON CEMENT BLOCKS USED FOR SUMP.
HOOKING DRAW BAR UP, EE PINCHED HIS LITTLE FINGER BETWEEN CRIB AND DRAW BAR.
Employee was walking along side of right ribs which was outby face. A piece of slate/coal fell from between bolts striking his back & tail bone. Rock 5 1/2" thick x 6' long x 3 1/2' wide.
While installing a roof bolt, a piece of slate/roof coal 2' square x 1 1/2" thick fell out from between bolts struck the back of his calf near the ankle.
Employee was working on a conveyor belt move, installing belt structure. Top roller was being put into place when he felt pain in his back. He did not report the accident to management. On Saturday, 2/19 he called the clinic for treatment.
Removing 6" pipe off a mine car. Stringing out pipe, injured sat pipe down, other employee pushed pipe. Injured's rt middle finger smashed between 2 6" pipes.
CM was changing places - employee was helping to move miner cable. Felt pain in groin area.
EE WAS FOUND LYING ON THE TRACK, NOT BREATHING, NO PULSE. CPR PERFORMED. PRONOUNCED DEAD. HEART ATTACK ACCORDING MEDS. FED INVESTIGATION WAS DONE
Employee was using a spad gun to hang a piece of curtain, a small piece of rock broke off roof, fell and struck ee on upper lip.
Employee was roof bolting at the face, as he swung the boom to install a bolt a piece of roof rock 4 in thick by 1 ft long fell and struck ee on top of left foot.
Employee was hanging continuous miner cable. He complained of pain in his mid to low back.
EE was standing beside the continuous miner. As he went to move, he slipped on a piece of rock, falling backwards and to the right, landing on his right shoulder.
PUTTING CABLE ONTO MIDDLE HOOD OF MINER, STUCK FORWARD, TRIED TO GRAB & JERKED LOOSE. HOOK SWUNG AROUND & SMASHED FINGER.
EE was pulling conveyer belt off a mine car using channel locks to grip belt. The channel locks slipped, ee went backwards, tripped over 6 inch water line and fell against coal rib.
ROOF FALL IN THE RIVER HEADINGS AREA 102 CROSSCUT #7 ENTRY. THE FALL IS APPROX 40 FT LONG, 15 TO 20 FT WIDE, UP TO 18 FT HIGH. THIS AREA WAS ORIGINALLY DEVELOPED DEC 1959 AND SUPPORTED WITH 7 FT POINT ANCHOR ROOF BOLTS. THERE WAS A SLIP/CLEY VEIN RUNNING DIAGONALLY THROUGH THE AREA.
EE WAS ATTEMPTING TO MOVE A PORTABLE LIGHT PLANT. THE JACK FOLDED UNDER THE TONGUE. THE TONGUE STRUCK THE EE ON TOP OF THE RIGHT FOOT.
EMPLOYEE WAS SHOVELING RIB ROLLS ALONG A SUPPLY TRACK OFF THE SLOPE BOTTOM. HE COMPLAINED OF GROIN PAIN ON 10/19/04 AND ASKED TO SEEK MEDICAL ATTN. AT THAT TIME. FIRST DAY OFF WORK WAS 10/20/04.
EMPLOYEE WAS HANGING SCREEN ALONG THE LONGWALL FACE IN PREPARATION FOR RECOVERY OF LONGWALL. AS HE ROLLED OUT THE SCREEN HE TWISTED HIS RIGHT WRIST.
EE HAD BEEN CLEANING A FLOOR DRAIN IN THE PREP PLANT. HE STEPPED IN THE OPENING, TRIPPED AND FELL FORWARD. HIS HARD HAT FELL OFF AND HIS HEAD HIT AN UPRIGHT BEAM.
EE WAS THROWING A TRACK SWITCH AT THE TOP OF THE TRACK SLOPE. THE SWITCH THROW HANDLE SPRANG BACK AND TWISTED EE'S RIGHT WRIST.
EE stated he was crossing the walkway over the river heading 119 sump, he tripped over a water pipe, and fell landing on his right knee.
EMPLOYEE WAS BUILDING A STOPPING. AS HE WAS LIFTING A 6" SOLID BLOCK INTO PLACE HE SLIPPED, LOST HIS BALANCE AND THE BLOCK STRAINED HIS UPPER RIGHT ARM.
EMPLOYEE WAS SHOVELING LOOSE COAL OFF THE "TRIPPER CAR" OF THE BLENDING BIN. HE SAID HE FELT A SHARP PAIN IN HIS LOWER BACK AND NUMBNESS IN HIS LEFT LEG.
EE AND COWORKER WERE ATTEMPTING TO PRY A SECTION OF "EICO TRAC" ON LONGWALL FACE. THE SLATE BAR SLIPPED AND FELL INTO THE FACE CONVEYOR. IT HIT A FLIGHT AND SWUNG OUT, STRIKING EE IN LEFT RIBS.
Roof fall in the 5 seat longwall #2 entry track between band 14 x cuts. fall is @ 45 ft long, 17 ft wide, and 6-8 ft in height, area was developed 3-04 with continuous miner, and supported with 7 ft point anchor roof bolts. additionally a row of propsetter post was installed off the left rib thruout area due to a clay vein which ran lengthwise with the entry.
#6 shield was plowing into the mine bottom. another ee complressed the base lift jack s0 ee could put a crib underneath the base.t he other ee released the button for the base lift jack which caused the shield tip to come down. the shield canopy struck injured ee on top of head.
WHILE SERVICING A ROOF BOLTER, EMPLOYEE WAS HANDING A CAN OF HYDRAULIC OIL TO ANOTHER EMPLOYEE. A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS AND STRUCK EMPLOYEE ON TOP OF HEAD.
Employee was riding in a mantrip to the section. The mantrip derailed. The employee complained of back pain.
Employee was changing a headlight on the hoist brake car. As he was pulling the bulb out of socket it came apart in his hand.
The employee was drilling a roof bolt hole at the face. A piece of rock approx 5" x 5" x 4" fell from roof, glanced off bolting machine and struck employee on right elbow.
Employee was replacing bottom belt stands on the conveyor belt. as he lifted one he felt something pop in his left shoulder.
ROOF FALL ON THE 4 EAST LONGWALL RECOVERY FACE. FALL WAS APPROX. 6 FT WIDE, 16 FT LONG, AND 10 FT HIGH. THE FALL OCCURRED BETWEEN THE TIPS OF THE SHIELDS AND THE FACE. THIS AREA WAS SUPPORTED WITH 2 ROWS OF 7 FT POINT ANCHOR ROOF BOLTS.
EMPLOYEE WAS REPAIRING A DAMAGED RUBBER AIR HOSE. HE WAS TIGHTENING A BREEZE BAND WITH A SCREW DRIVER. THE SCREW DRIVER SLIPPED OUT OF SLOT IN BREEZE BAND AND PUNCTURED LEFT HAND BETWEEN THUMB AND FIRST FINGER.
ROOF FALL IN THE HIGH QUALITY 4 EAST GATE ENTRIES, #4 ENTRY FROM 45+50' TO APPROX. 46+10'. FALL IS APPROX. 60 FT LONG, 18 FT WIDE, 12 FT HIGH. AREA MINED FEB/MARCH 2004 WITH CONTINUOUS MINER AND SUPPORTED WITH 7 FT POINT ANCHOR ROOF BOLTS PLUS 6 - 100 TON BIG JOHN PROPS. THE AREA HAD A CLAY VEIN RUNNING DIAGONALLY THROUGH IT.
EE WAS LIFTING THE "BRETBY" CABLE HOLDER OUT OF THE PANLINE ON THE LONGWALL FACE. HE STATED HE FELT PAIN IN HIS LOWER BACK.
EE WAS REMOVING THE LID FROM A 5-GALLON OIL CAN TO USE TO MIX B-BOND. HE WAS PULLING LID WITH A PAIR OF CHANNELLOCKS. HIS GRIP SLIPPED AND EE CUT HIS LEFT HAND ON LID. EE RECEIVED 6 STITCHES IN LEFT HAND.
ROOF FALL IN THE 5 EAST (002-0) SECTION #2 ENTRY, 34 CROSSCUT. FALL IS APPROX. 35'LONG X 18'WIDE X 13'HIGH. AREA WAS MINED IN MAY 2004 WITH CONTINUOUS MINER AND SUPPORTED WITH 7'POINT ANCHOR ROOF BOLTS. THE AREA HAD A SLIP RUNNING DIAGONALLY THRU THE ENTRY & THE ROOF WAS "DRIPPING" WATER IN THIS AREA.
ROOF FALL IN THE 5 EAST DEVELOPING SECTION, #3 ENTRY FROM 25 1/2 TO INBY CORNER OF 26 CROSSCUTS. FALL IS APPROX. 80 FT LONG, 17 FT WIDE, 8 TO 10 FT HIGH. AREA WAS MINED LATE APRIL '04 WITH CONTINUOUS MINER & SUPPORTED WITH 7' POINT ANCHOR BOLTS SUPPLEMENTED WITH 12 & 16 FT. CABLE BOLTS. THERE WAS SOME ROOF TO GROUND SUPPORT INSTALLED(CRIBS). AREA HAS CLAY VEIN RUNNING WITH ENTRY.
EMPLOYEE WAS MAKING A BELT SPLICE. HE WAS USING A CHAIN COME-ALONG TO PULL THE SECTIONS OF BELT TOGETHER HE COMPLAINED OF PAIN IN GROIN AREA.
EMPLOYEE WAS USING A COME-ALING TO MOVE THE SHEAR CUTTER MOTOR OFF THE FACE. HE COMPLAINED OF PAIN IN HIS GROIN.
