While employee was performing pre-shift inspection of equipment he had climbed up truck ladder checking things. As he was coming back down the ladder, he put his foot down on uneven ground & rolled his Left Ankle falling to the ground.

While being loaded by an excavator, a high wall approx. 50 feet away, collapsed sending unconsolidated material into the haul truck. Employee exited his cab and surveyed the situation (while on the truck), when the material began moving, sending the truck over another high wall. The employee jumped off his truck and tumbled down the high wall sustaining multiple injuries.
Employee was checking water level on equipment. He unsecured the cap with his foot & it had no pressure on the cap so he removed the cap from the radiator with his right hand. About the time he raised up, the radiator began to blow hot water out & hit his right arm, face, back & right leg.

While EE was operating water truck, employee said brakes would not hold & collided with 100 ton rock truck head on. Damage to water truck with no damage to rock truck. Rock truck driver sustained NO injury.
Employee was digging hopper out on crusher & lost his grip which caused him to fall back against hand rail hitting his lower back. He did not seek medical attention until 3/16/2011.

Employee was helping other employees reattach a bull hose to a pump using a crow's foot which became unlodeged letting the bull hose swing around & knock employee to the ground hurting his side, shoulder & back.

Employee was steam cleaning truck # 212 while climbing front ladder on truck. He fell backwards approx. 4 feet landing on Tail & Back complaining of back pain.
Employee called foreman stating he had been jarred real good while being loaded and wanted to get down & walk around for a minute or two. 15 to 20 minutes later he called foreman stating his neck & back were hurting worse and wanted to go to the doctor.
While Employee was moving a pump, the suction hose hung in rocks under pressure. The hose flipped back, hitting employee in the back & back of the Head.

Employee pulled ladder down on equipment in order to climb up. Ladder did not go all the way down, when employee attempted to go up ladder, his foot slipped and caused his Left Knee to hit ladder.
