Employee was hooking up 8" hose to pump, he was moving the hose around when he felt something pull in his back.

Employee slipped and fell down the steps on equipment coming out of the cab down to the engine compartments and sprained his right ankle.

Employee was changing the oil in rock truck. As he was removing the drain plug in the rear end, the wrench slipped & he split his knuckle on one of the other bolts. Right Index Finger Laceration.

While employee was performing pre-shift inspection of equipment he had climbed up truck ladder checking things. As he was coming back down the ladder, he put his foot down on uneven ground & rolled his Left Ankle falling to the ground.

Truck Driver was bending down in preparation to hook up hoses to unload and strained back.

While cleaning coal on a small block of coal, loader slid off hitting the bottom hard which caused employee to hurt his back.

While being loaded by an excavator, a high wall approx. 50 feet away, collapsed sending unconsolidated material into the haul truck. Employee exited his cab and surveyed the situation (while on the truck), when the material began moving, sending the truck over another high wall. The employee jumped off his truck and tumbled down the high wall sustaining multiple injuries.

Employee was checking water level on equipment. He unsecured the cap with his foot & it had no pressure on the cap so he removed the cap from the radiator with his right hand. About the time he raised up, the radiator began to blow hot water out & hit his right arm, face, back & right leg.

While EE was operating water truck, employee said brakes would not hold & collided with 100 ton rock truck head on. Damage to water truck with no damage to rock truck. Rock truck driver sustained NO injury.

Stemming holes when employee struck rock with shovel and strained back.

Employee was climing ladder onto equipment, slipped and fell to ground landing hard on left foot.

Employee reached over in the crusher to pull out a Rock. He felt his Left Shoulder pop which caused his arm to go numb.

Employee was digging hopper out on crusher & lost his grip which caused him to fall back against hand rail hitting his lower back. He did not seek medical attention until 3/16/2011.

It was muddy & icy from excessive rain and when employee went to step across Track, he slipped & fell landing on his right arm.

Employee was helping other employees reattach a bull hose to a pump using a crow's foot which became unlodeged letting the bull hose swing around & knock employee to the ground hurting his side, shoulder & back.

EE was climbing up on his boom truck to get blocks to crib up truck and he slipped and fell.

While operating Dozer, employee ran over rock with blade up and it threw him into door handle, where he hit his face. Contusion to nose & mouth.

Employee was pulling fuel hose out of truck & pulled something loose in Right Shoulder.

While loading coal trucks, employee backed loader his was driving into coal crusher.

Employee was climbing down off of Dozer equipment, slipped & fell down hitting his back on the push arm.

While walking to help with pump hose, employee's left foot slipped in mud causing his right foot and leg to fold under him breaking his right ankle and twisting his right knee.

While climbing up on equipment employee hurt his back.

Employee was helping set a pump, while pulling on the rope to get the suction hose in place, he slipped and fell and landed on his right wrist.

Employee was working with other truck drivers and pulling on hose. Caused him to complain about his back hurting.

Employee was steam cleaning truck # 212 while climbing front ladder on truck. He fell backwards approx. 4 feet landing on Tail & Back complaining of back pain.

Employee was getting out of Service Truck & got his foot hung between the Cab & step. He fell & twisted left knee in the process.

Employee called foreman stating he had been jarred real good while being loaded and wanted to get down & walk around for a minute or two. 15 to 20 minutes later he called foreman stating his neck & back were hurting worse and wanted to go to the doctor.

Employee was climbing off motor grader & his foot slipped off step. He complained of back pain, but did not want to go to doctor at that time.

Employee was helping another employee change oil in screen plate & got a piece of coal in his left eye.

While replacing bolts in Air Compressor, employee grabbed hot exhaust, burning his left hand.

While loading pump hose into loader bucket, a rock fell on his right foot.

While walking, slipped & fell in gravel on hillside, causing pain to hip, lower back & left elbow.

While pushing coal with Back Hoe, employee jarred his back, causing pain to radiate up to his neck.

While pushing overburden benching down to milldale, employee inadvertently trammed off aprox. 30' edge.

While repairing the light on his hard hat, the utility knife the employee was using slipped causing deep laceration to his left middle finger.

While employee was providing service to Backhoe equipment, when he was climbing off, he slipped causing a contusion to his right knee.

Employee fell while pulling on hose at pump.

Employee was climbing onto Dozer, slipped & fell hitting right ankle.

While Employee was moving a pump, the suction hose hung in rocks under pressure. The hose flipped back, hitting employee in the back & back of the Head.

Employee pulled ladder down on equipment in order to climb up. Ladder did not go all the way down, when employee attempted to go up ladder, his foot slipped and caused his Left Knee to hit ladder.

Employee was climbing off of equipment & fell off ladder landing to the ground. He complained of Lower hip & back pain.

Employee was stepping onto rock truck when he felt a sudden pain to his left calf leg area.

Employee backed into a truck causing jolt to neck.

Employee was welding when arch bounced off face shield going into eyes.

Employee was sitting in rock truck cab while being loaded. Cab of truck shook causing employee to claim back pain.

Employee was turning on equipment, slipped & fell, hurting his Back & R.Shoulder.

Employee was closing lid on tool box when lid fell on his right thumb causing laceration.
