INJURED WAS SHOVELING UNDERGROUND CONVEYOR BELT AND WAS ATTEMPTING TO DISLODGE SOME HARDENED MATERIAL FROM UNDER A RETURN IDLER, WHEN HE FELT A SHARP PAIN IN THE LOWER BACK AREA.
INJURED WAS REMOVING COAL SPILLAGE FROM ALONGSIDE, NO. 1 BELT CONVEYOR & CAUGHT LEFT FOOT IN A CONVEYOR RETURN ROLLER.
INJURED WAS LIFTING A COUPLING, PRIOR TO EMPLACING SAME INTO THE DRAW-HEAD OF A SUPPLY CAR, WHEN HE EXPERIENCED A LOWER BACK PAIN.
INJURED WAS INVOLVED IS ASSITING IN A BELT EXTENTION AND REPORTED HE FELT PAIN IN HIS LOWER ABDOMEN WHILE LIFTING TO PLACE TOP IDLER STRUCTURE ON CONVEYOR SIDEFRAME.
INJURED WA SCLOSING SWITCH A 480 VAC 200 AMP SWITHC BOX, SUPPLY POWER TO 100 HP FAN MOTOR CIRCUIT, WHEN A SHORT CUIRCUIT OCCURED IN SWITCH BOX, EJECTIN GMETAL FROM THE ENCLOSURE, CAUSING A LAC ERATION TO THE UNDERSIDE OF THE CHIN.
INJURED WAS SPRAYING PERMANENT STOPPINGS PER 75.333 WITH A SODIUM SILICATE PRODUCT. THE SPRAY MACHINE CLOGGED AND THE INJURED WAS PERFORMING REMEDIAL CHECKS AND ACTIVATED THE MACHINE, CAUSING PRODUCT TO BE EXPELLED FROM THE MACHINES HOPPER INTO BOTH OF HIS EYES. INJURED WAS NOT WEARING SAFETY GLASSES.
INJURED WAS ATTEMPTING TO BOARD THE TRACK MOUNTED PERSONNEL CARRIER & BUMPED HIS LEFT KNEE ON THE BUMPER OF THE VEHICLE INJURY DID NOT BECOME MANIFEST UNTIL END OF FOLLOWING SHIFT ON 03 MARCH 94
FALL OCCURRED IN NO. 6 ENTRY VIC SP 2865, APPROX 110'X18'X6'. ROOF MATERIAL WAS THINLY BEDDED SHALE STRATA AND APPEARED WET. COLLAPSED AREA ISADJACENT TO 100' COVER LINE. ENTRY WAS INITIALL Y REINFORCED WITH 36" MECHANICALLY ANCHORED BOLTS AND SUPPLEMENTALLY SUPPORTED WITH CRIBS PRIOR TO FAILURE.
INJURED WAS HOLDING 60# RAIL IN PLACAE WITH HIS RIGHT HAND, ON THE DRIFT END OF THE SUPPLY CAR, ATTEMPTING TO START A SECOND ROW OF RAILS. THE FOUR MAN SUPPLY CREW PICKED UP THE HIGHWAY END OF THE RAIL & PLACE ON SUPPLY CAR. RAIL ROTATED & CAUGHT THE TIP OF INJUREDS MIDDLE FINGER OF RIGHT HAND.
HE WAS PICKING UP AND CARRYING BELT STRUCTURE WHEN A PAIN STARTED IN HIS LOWER BACK.
HE WAS GATHERING BELT STRUCTURE WHEN A BOTTOM STAND FELL AND HIT HIM IN THE LOWER PART OF HIS LEG PUNCTURE WOUND.
