A fall of the roof occurred on the 2nd SE Mains, #5 entry, crosscuts 38-39 (intersection - pillar line). The fall measured approximately 30ft in length, 20ft in width, 7-15ft height. Area has been dangered off, standing support installed around fall area, we do not plan to clean up the fall at this time.

EE was outby the last open crosscut reclaiming line curtains and as EE pulled on the curtain, which was spadded to the rib, unbeknownst to EE loose draw rock was resting on the curtain; the rock struck right shoulder. EE has a non-surgical fracture to right collar bone and began missing time on 04/11/2019.

EE was operating a winch unloading hydraulic cylinders off of ride, EE caught finger between the cylinder and the ride, pinching the tip of the right index finger, causing a laceration that EE received sutures for. EE did not miss time as a result of this injury, nor was EE placed on restrictions.

EE was at the charging station on the supply road and parked the scoop in position to change out the batteries. As EE exited the scoop and traveled toward the batteries, EE slipped and fell striking the right side of face against the scoop frame.

A fall of the roof occurred in the 1st NE Mains, #7 entry, crosscut 54, intersection/pillar. The fall measured approximately 40ft in length, 18ft-20ft width, & 7ft-15ft. Standing support has been installed in the area, and flagging on each accessible side of the fall.

Employee was operating a continuous miner (CM), turning a crosscut. As they engaged the cutting head, a rock fell from the mine roof striking the CM, causing another rock to slide off of the CM, which struck employee in the right hand. Employee suffered sutures and was placed on restrictions preventing them from performing their normal occupation.

EE was operating a continuous miner (CM). As ee engaged the cutting head, a rock fell from the mine roof and struck the top of the CM, which caused a rock to slide off of the CM. The rock that slid off the CM struck ee in the right hip, causing a hip dislocation.

EE was adding hydraulic oil to an outby bolter, as EE lifted the five (5) gallon can of oil EE felt pain/strain in lower stomach (groin area hernia). EE continued working without restrictions performing normal occupation until EE had surgery date of 2-19-19.

EE was servicing the batteries for the ram car. As EE bent down to lift a gallon jug of water, EE twisted causing pain to mid/lower back. EE's doctor gave EE a full release without restrictions until 11-20-18, at which time EE's doctor placed EE on restrictions.

EE was tightening the cats on a continuous miner (CM). As they were positioned on the mine floor beside the CM, a piece of loose draw rock that was resting on the top of the CM shifted and fell striking them on the right ankle/foot, causing a fracture. They saw the loose material prior to squatting down, but did not think that it would move. EE began missing time on 10-29-18.

EE was washing and servicing the continuous miner (CM), as ee was removing a rock from on top of the CM, pinching left hand between the rock and the top of the CM. EE suffered a fracture to index finger and was given a full release and ee did not miss work as a result of this injury. EE returned to work on 10-23-18 performing normal occupation without restrictions.

A fall of the roof occurred in the 3rd NE Mains, 4th North Intake, crosscut 46, from #3 entry toward #2 entry. The fall measured approximately 50ft. in length, by 20ft. in width, by 7-15ft. in height. This area has had additional standing support installed and the primary escape way has been re-routed.

EE was observing mechanics repair the mesh handling tray jacks on a walk-through bolter. As the mechanics re-positioned the tray, the safety chain broke causing the tray to strike EE on the right shoulder & upper back. EE suffered 2 fractured ribs and ee began missing time on 09/17/2018.

EE was operating a walk-through bolter installing rib bolts, as ee attempted to place a plate on the bolt, ee dropped the plate onto forearm causing a laceration. EE received 2 sutures, ee did not miss time and returned to work on next scheduled shift performing normally scheduled occupation.

Employee was exiting a diesel mega trac and twisted their left knee. Employee continued to work without restrictions performing normal occupation until surgery on 10-30-18. Employee began missing on 10-31-18, they worked the day of their surgery as well.

EE was operating a walk-through bolter installing roof bolts. While EE was pushing the bolting tray off of the bolter, EE felt pain in the lower back, and transported to a clinic x-rays were conducted with nothing conclusive. Being treated for a strain and placed on restrictions that prevented EE from performing normal occupation. EE began missing time on 08-20-18.

EE was stacking/storing wire mesh onto the walk-through bolters mesh handling slide. As EE was handling the mesh, the tip of EE's finger caught on the mesh causing a laceration to the tip of the finger. EE received 4 sutures to the tip of the finger. EE received a full release to perform normal occupation. EE did not miss time as a result of this injury.

EE was treated for a mild strain, given a full release to regular occupation. On 7-16-18 ee seen doctor and was placed on restrictions that prevented ee from performing normal occupation. EE began missing time on 7-17-18.

EE was removing the collar off of the mesh tray jack, a piece of draw rock came from the rib/roof striking EE in the head & right shoulder. The force of the rock drove EE's head and left side of EE's face against the dust box. EE suffered fractures to the left side of EE's face, below EE's eye. EE began missing time on 6-11-18.

EE was operating a walk-through bolter installing rib bolts. While EE was installing the bolt, EE applied boom pressure/rotation and EE's glove caught on the bolt causing left hand to twist. X-rays revealed fractures to the middle & ring fingers. EE has been placed on restrictions that prevented EE from performing normal occupation. EE began missing time on 06-05-18.

X-rays were conducted with nothing conclusive. EE is being treated for a strain and placed on restrictions that prevented EE from performing EE's normal occupation. EE began missing time on 06-01-18.

EE was operating a diesel getman on the supply road. EE encountered a loose concrete block on the blind side, which caused EE to be bounced in the seat. As EE bounced, they felt pain in their back. EE was given a full release to return to work without restrictions. On 05-08-18, EE had an appointment and the MRI revealed two herniated discs, EE was placed off work at this time.

A fall of the roof occurred on the 2nd SE Mains, #1 Unit, crosscut 148, entry #2 (intersection). The fall measured approximately 20' length, 18'-20' width, & 7'-15' thick. The fall has been cleaned up according to our approved rehab plan, additional support has been installed in the area, with the fall area being arched.

EE was a passenger on a 10-man mantrip. The driver encountered uneven bottom which caused EE to be bounced in EE's seat. As EE bounced, EE struck EE's head against the canopy of the mantrip, causing pain to EE's neck. EE continued working without restrictions until EE was taken off work on 04-04-18, at this time EE began missing time.

A fall of the roof occurred on the 1st NE Mains (1st 54) in the #5 entry, the pillar line fell between crosscuts 26-27. The fall measured approximately 65' in length, by 18'-20' in width, & 7'-14' in height. The fall was in a secondary escapeway entry, the lifeline was re-routed, flagged, mapped, and all persons were notified of the change.

EE was operating a walk-through bolter and was drilling a hole into the mine roof in preparation for installing a roof bolt. During this process, a piece of draw rock slid off of the wire mesh striking EE in the right hand. EE was transported to ER where EE had surgery and is expected to make a full recovery and return to normal occupation within 5-6 weeks.

EE was operating a walk-through bolter, installing roof bolts in the #2L face. During the process of drilling the hole, the drill steel became stopped up with dust and EE did not let off of the boom/rotation pressure quickly enough, resulting in the steel bending and striking EE in the left side of the face. EE began missing time on 11-21-17.
