EE STATED WAS WORKING AT COMPUTER STOOD UP AND FELT PAIN IN BACK. DID NOT MISS WORK UNTIL 10-8-98. ONE DAY.
EE STATED HE WAS REMOVING PUMP BARREL ON A CART FROM PUMP BAY. HE ROLLED CART ACROSS THE GRATING AND INTO SUMP, WEDGING HIS HAND BETWEEN BARREL ON CART AND PUMP.
EE'S TRUCK WAS BEING LOADED, BOULDER FELL INTO BED OF TRUCK, THROWING EE TO THE RIGHT SIDE OF THE TRUCK
EMPLOYEE WAS CLIMBING ONTO DOZER, SLIPPED AND STRAINED BACK.
EMPLOYEE WAS WALKING DOWN LADDER ON BACK SIDE OF 80 YD MARION DRAGLINE BUCKET. HE WAS CUTTING SAFETY RAILING LOOSE SO THEY COULD TURN THE BUCKET FOR REBUILD WORK. WHEN HE STARTED DOWN LADDER H IS FOOT SLIPPED. HE FELL LANDING HIS WEIGHT ON HIS LEFT ELBOW, HITTIGN IT AGAINST DRAGLINE BUCKET.
FLASH BURNS TO BOTH EYES FROM WELDING PROCESS EE WAS WELDING WEAR BLOCK ON DRAGLINE BUCKET AND FELLOW EE . EE GOT EYES BURNED FROM OTHER PERSONS ARC RAYS
GRADER STARTED ROLLING, STRIKING EE ON LEFT ARM.
EE WAS WALKING INTO OFFICE TRAILER, TRIPPED AND FELL INJURING LOWER BACK.
EE WAS GETTING OFF MAHCINE AT END OF DAY, SLIPPED ON LADDER CAUGHT HIMSELF AND FELT SHARP PAIN IN SIDE.
EE WAS STEPPING DOWN OFF LUBE-RACK, SLIPPED AND FELL-SCRAPED RIGHT LEG AND TWISTED KNEE.
EE WAS TRAMMING DOZER IN SHOVEL PIT. DOZER RAN OVER ROCK. THIS CAUSED EE TO COME OUT OF THE OPERATOR'S SEAT STRIKING TOP OF HEAD CAUSING LACERATION REQUIRING 7 STAPLES FOR CLOSURE.
WAS ATTEMPTING TO PUT DECK BUSHING BACK INTO DECK. CAUGHT MIDDLE AND RING FINGER OF RIGHT HAND AND CUT MIDDLE FINGER.
BOOM CABLE STRUCK EMPLOYEE ON RIGHT CALF.
EMPLOYEE WAS USING A HAND GRINDER AND A PIECE OF METAL STRUCK LEFT EYE.
EMPLOYEE WAS STANDING ON A DIRT PILE WELDING CRACK ON TOP OF RAILING OF BUCKET.HE WENT TO STEP DOWN AND SLIPPED AND FELL LANDED ON HIS LEFT SHOULDER.
EE WAS DRIVING ROCK TRUCK-HIT HOLE IN HAUL ROAD-FELT PAIN IN LOWER BACK
EMPLOYEE WAS LIFTING A PULL CABLE TO LIFT A TRUCK WHEN HE FELT A PAIN IN LOWER BACK.
EE COMPLAINED OF CHEST PAIN AND PAIN IN LEFT ARM.
EE WAS USING A HOIST JACK WHEN JACK SLIPPED AND MASHED AND BROKE HAND.
EE WAS CLIMBING ONTO TRUCK AT BEGINNING OF SHIFT AND TWISTED HIS BACK.
EE WAS OPERATING LOADER WHEN LOADER JERKED AND EE FELT PAIN IN BACK AND BURNING SENSATION IN LEGS. EE DID NOT REPORT INCIDENT UNTIL 4-13-98.
EMPLOYEE OPENING HOOD WHILE PERFORMING PRE-TRIP AND STANDING ON BUMPER. HOOD OF TRUCK OPENED AND TRAPPED FEET BETWEEN HOOD AND BUMPER EMPLOYEE FELL BACKWARDS RESULTING INSPRAIN TO RIGHT ANKLE. INJURY OCCUREED IN PARKING LOT OUTSIDE DRIVERS OFFICE AREA.
HE WAS PULLING ON A #4 SHOVEL WITH A SMALL MOTOR ON THE SHOVEL (STATED HE KNEW HE SHOULD NOT DO THAT). SHOVE SLIPPED OUT FROM UNDER THE MOTOR AND HE FELL ON A JUNCTION BOX COVER HE HAD LAID IN THE FLOOR. (USING WRONG TOOLS TO MOVE A MOTOR.)
EE WAS CHECKING FLUID LEVELS IN WELDING MACHINE. THE BED OF TRUCK WAS FROSTED OVER. A PIECE OF ;PLASTIC WAS LYING IN THE BED OF TRUCK. EE GOT TANLED UP IN IT. HE LOST HIS FOOTING A FELL OFF TR UCK LANDING WITH ALL HIS WEIGHT ON TO HIS RIGHT HAND.
EE STATED HE WORKED CHECKING COMPONENTS IN GRID BOX WHEN COVER FELL & MASHED HIS FINGER.
EMPLOYEE SAID: HE WAS GOING DOWN STEPSS FROM WAREHOUS #3 AND SLIPPED ON FROST THAT WAS ON STEPS AND TWISTED HIS LEFT KNEE.
EMPLOYEE WAS REPAIRING THE WIRING THAT OPERATES THE DECK LIGHTS ON A D-11 TRACK DOZER. AFTER COMPLETING THE REPAIRS HE WAS LEAVING THE DOZER AND HIS FOOT SLIPPED ON THE TRACK AND HE FELL OFF T HE TRACK. HE WAS HOLDING ON THE HANDRAILWITH HIS LEFT HAND AND TWISTED HIS WRIST ON THE WAY DOWN. (IT WAS RAINING).
HE WAS PUTTING EQUIPMENT BACK INTO COMPANY TRUCK, PREPARING TO LEAVE JOB SIT. HE STEPPED OFF THE REAR OF THE TRUCK TO A STEP. HIS RIGHT FOOT SLIPPED OFF THE STEP, AS HE FELL TO THE GROUND HIS LEFT KNEE HIT THE STEP, TWISTING THE LEFT KNEE.
EE WAS WELDING NUTS ON BROKEN BOLTS TO BE EXTRACTED AND REPALCED WITH NEW ONES. HE WAS WORKING ON THE POINT SHEAVES OF 8200 MARION DRAGLINE. THE BOLTS WERE SATURATED WITH GREASE. WHEN HE WELDE D THE NUTS ON THE BOLTS A PIECE OF METAL OR GREASE POPPED UP IN HIS LEFT EYE.
WAS DRIVING ROCK TRUCK APPARENTLY LOST CONTROL OF TRUCK COMING DOWN HILL TO PREP PLANTTRUCK HIT RETAINING WALL BEHIND PREP PLANT HOPPERS-TRUCK FLIPPED END OVER END OVER WALL TO ABOUT SIXTY FEE T BELOW WALL ONTO EMBANKMENT.
EMPLOYEE WAS ASSISTING WELDER IN POSITIONING TOOTH BASE ADAPTER FOR 992 CAT LOADER. THE PART WAS BEING POSITIONED FOR WELDING LOCATION.
EMP SAID HE WAS PUTING ENGINE OIL IN ROCK TRUCK.WHEN HE FINISHED HE CLIMBED DOWN LADDER TO BOTTOM STEP AND THEN JUMPED TO GROUND AND FELT SHARP PAIN IN GROIN AREA.THE DISTANCE FROM BOTTOM STEP TO GROUND IS APPROX 40".
EE HE WAS STANDING ON THE FLOOR WASHING OUT THE FINE REFUSE CHUTE. WATER AND COAL BROKE LOOSE AND KNOCKED DOWN DOWN AS IT CAME FROM THE CHUTE. EE KNEW HE WAS NOT SUPPOSE TO BE A FRONT OF CHUTE .
EMPLOYEE SAID HE WAS USING PRYBAR TO MOVE DIRLLS TEEL FROM BEHIND CABLE AND HE FELT SOMETHING POP IN HIS STOMACH AREA. A COUPLE OF DAYS LATER, A KNOT CAME UP ON STOMACH BESIDE NAVEL.
EMP WAS CLEANING WELDING ROD STUBS OUT OF MARION DRAGLINE BUCKET. RAISED UP WITHOUT HARD HAT ON AND CUT HIS LEFT SIDE OF FOREHEAD ON A SCAFFOLDING USED TO WORK OFF TO REPAIR CRACKS IN BUCKET.
EMP WAS DOGGING A PIECE OF 1" T-1 PLATE DOWN ON THE INSIDE OF MARION DRAGLINE BUCKET. WELDED OSM 1" DOGS IN SLUG HOLES. WAS USING A PORTE POWER AND 5" REM TO TRY TO PULL PLATE DOWN. WAS OPERAT ING PORTE POWER TO PULL PLATE DOWN WHEN DOG BROKE AND RAM JUMPED UP AND HIT HIM OVER LEFT EYE.
EE SAID HE WAS AT DUMP SITE, CLEANING OUT BED OF SERVICE TRUCK. HE SAID HE PICKED UP SPACE TIRE AND RIM ASSEMBLY 10000X20" AND FELT SHARP PAIN IN LEFT MIDDLE OF BACK.
EE WAS CHANGING THE BRAKE ROTOR AND HUB ASSY ON A 170 TON REAR DUMP TRUCK. A LINE UP BOLT WAS BEING USED TO HAND ROTOR ASSY ON UNTIL IT CAN BE PUSHED INTO PLACE. THE ROTOR SLIPPED OFF LINE UP BOLT AND EE CAUGHT IT IN HIS HANDS AND IT PULLED HIM DOWNWARD. EE SAID HE THOUGHT IF WAS GOING TO FALL ON HIS FEET AND HE COULDN'T GET OUT OF THE WAY.
INJURED PERSON WAS HELPING OTHER EE MOVE A PIECE OF 1 1/2 THICK PIECE OF STEEL AROUND TO PUT A METAL CABLE AROUND. WHEN INJURED PERSON MOVED THE STEEL, HE CAUGHT HIS LEFT THUMB AGAINST ANOTHER PIECE OF METAL. THE METAL TORE A PIECE OF FLESH OFF HIS THUMB, IMBEDDING METAL UNDERNEATH THE WOUND.
EMPLOYEE WAS HELPING IN THE REMOVING OF A REAR SUSPENSION ON A 170 TON REAR DUMP TRUCK. EMPLOYEES WERE USING A FORK LIFT TO HELP IN REMOVING SUSPENSION. EMPLOYEE GOT HIS FINGER INBETWEEN ROLLE R AND BULGE ON FORK LIFT AND MASHED HIS FINGER.
EMPLOYEE WAS ADJUSTING A BELT SCRAPER. WHILE PULLING ON ADJUSTABLE PLIERS, THE JAM NUT ON SCRAPER ABRUBTLY LOOSENED. THE EMPLOYEE LOST HIS FOOTING AND FELL ONTO HIS BACKSIDE CAUSING A COMPRE SSION FRACTURE OF HIS L1 LUMBER.
EE SAID: HE WAS TIGHTENING BOLT ON FAN HUB OF A 16V149 DETROIT DIESEL ENG WHEN HIS RATCHET SLIPPED OFF, AND EE SLIPPED FORWARD, AND HIT HIS UPPER LIP AGAINST A HOSE CLAMP ON A RADIATOR HOSE, RESULTING IN A 14' DEEP CUT TO UPPER LIP
BACKING UP THE HILL IN A COAL TRUCK AND HE HEARD SOME THING POP AND HE PULL BACK UP AND IT TURN OVER.
WHILE TIGHTENING AXLE CAP ON TRUCK THE EXTENSION BURST ON AND ANKLE POPPED.
EMPLOYEE SAID HE WAS CLIMBING LADDER ON TRUCK AND FELT SOMETHING POP IN HIS LOWER BACK AND HAD A BURNING SENSATION.
EMPLOYEE WAS CHANGING A STARTER MOTOR ON AN 170 TON REAR DMP TRUCK. ACCORDING TO EMPLLOYEE, HE WAS IN AN AKWARD POSTION AND FELT A SHARP PAIN IN HIS LOWER BACK SHORTLEY AFTER JOB WAS COMPLETE D.
