Showing all 40On 7-18-13 an employee was assisting in carrying a pump when he allegedly twisted an ankle. The employee completed that shift & all shifts leading up to 8-5-13, when he was placed off for non work related & pre scheduled surgery. The operator learned of the physical therapy for the ankle during an investigation & well after the employee was placed off. There were no days lost
Employee was striking the lipsaver of the PC3000 Bucket with a sledge hammer during maintenance procedures and a piece of metal sheared off the lipsaver and penetrated an area of his lower left leg.
Rock truck was being loaded by a PC 3000. Material was in bed of truck next pass a rock rolled from the bucket striking left front corner bed jarring operator. Mine operator has conducted a diligent investigation this time are reporting as a lost workdays (NFDL) accident, pending the review of additional med info. We reserve our right to amend, if necessary.
The EE began U.G. mining in 1975 and first filed for Coal Workers Pneumoconiosis in 1987 and was awarded 5%. The EE switched to surface mining in 1990. He was awarded a total of 20% and we were notified of that fact on 8/23/12. The EE has had no lost time or restricted duty.
Cut thumb on jagged sheet metal while installing engine enclosure on D11R. Required 8 stitches to close laceration.
EE & one person loaded a jack pad from a surface drill onto the walkway of the drill as the pad had came loose from the jack. The drill operator then moved the drill to wait on a mechanic. Then he pushed the pad from the walkway so it would fall to the ground. As the front edge of the pad fell, the rear raised & pinched the thumb between the pad & a hand rail. 5 sutures needed.
Right ring finger caught between 992G center drive shaft and steering jack boss on frame. Fractured right ring finger.
EE had been sick all morning and was vomiting, he had decided to go home if he vomited one more time. He pulled to the side of the road to vomit and when he leaned over the handrail, he apparently blacked out and fell 11 feet to the ground.
He was working on a dust collector. When he stepped onto the deck, his foot slipped, and knee made a popping sound. It was raining at the time, so deck was wet.
Fell backwards out of back of service truck while stepping to bumper.
Employee was stooping over D11R roller frame, cutting pins out of major boggle. He stood up and felt severe pain in lower back. Employee began missing work on 10/26/2009.
Employee was climbing off of powder truck when he stepped on frozen uneven ground causing him to fall on left ankle.

Hitting a piece of cold roll with sledge hammer, he missed the cold roll, hitting another piece of metal which caused the sledge hammer to "twist", which twisted the right wrist, causing injury.
Unloading oil filters into customers oil refuge bin, when one of the filters came loose smashing right pinky finger into the metal bin causing fracture.
Using pry bar to get brake slack adjuster off & got pain in groin area.
Technician was replacing bucket teeth on a 992G. Technician was using a sledge hammer on the pin driver when a small piece broke off the end of the pin driver and lodged in his right hand between the knuckles of his first and middle finger.
The truck that employee was operating overturned.
While dismounting an endloader, Employee alleges that he slipped & fell- thus injuring his back. Management questions this accident. This mine was idled the week of Thanksgiving.
Employee was coming down from off a machine when his foot slipped and he twisted his left knee.
EE WAS LOADING A LARGE ROCK WHEN THE ROCK FELL BACK INTO THE BUCKET, JARRING HIM. EMPLOYEE DID NOT MISS WORK UNTIL 8/8/05, WHEN HE SOUGHT MEDICAL ATTENTION.
Employee was climbing down off of a Caterpillar 992G and as he was climbing down he grabbed hold of a hose as a hand hold instead of the ladder. In doing so, he tore, possibly fractured, left shoulder.
Employee was repairing 988F that would not start due to the cole; in effort to start machine, had to use the hand priming pump and while pumping strained right shoulder.
Removing aftercoller fron engine, was in improper position for lifting while helping another mechanic, pulled muscle in back.
Employee was being loaded by loader when the loader dropped a large rock into the truck. The rock jarred employee, injuring shoulder.
Employee was walking down slope on valley fill when he slipped and fell, landing on his arms and shoulder. EE did not miss work until 11-11-04 when he had to get surgery.
EMPLOYEE WAS DRIVING TRUCK WHEN HE GOT IT OUT OF GEAR. EMPLOYEE COULD NOT GET TRUCK BACK IN GEAR & LOST CONTROL, TURNING TRUCK ON SIDE. EMPLOYEE STRUCK HEAD IN CAB.

WHILE CHANGING A STARTER, EMPLOYEE WAS ON HIS KNEES AND PICKED IT UP AND RESTED IT ON HIS LAP; AT THAT TIME HE NOTICED A BURNING SENSATION IN HIS BACK AND SHOULDERS.
EE WAS MOUNTING ROCK TRUCK WHEN HE CLAIMS TO HAVE FELT PAIN IN HIS CHEST.
EMPLOYEE WAS IN THE PROCESS OF LOADING THE POWDER TRUCK WITH ANFO. HE BENT DOWN TO CLOSE THE LIDON THE TRUCK BIN AND STRUCK HIS HEAD AGAINST A CROSS BEAM WHEN HE RAISED UP, HE RECEIVED A CUT O N HIS HEAD REQUIRING SEVEN SUTURES. HE RETURNED TO WORK AT APPROX 3:30 PM THE SAME DAY AFTER GOING TO THE OCEANA CLINIC.
VICTIM WAS INSTALLING THE REAR END IN A WESTERN STAR TANDEM WHEN IT ROLLED OVER ON HIS RIGHT ARM.
EE WAS PUSHING ROCK IN THE SHOT. AS HE WAS BACKING UP, HE STRUCK A ROCK, JARRING HIS BACK.
RAISE ARM, STRIKING TRUCK FRAME WITH ELBOW.
DRIVER STARTLED BY SHOT, FELL TO GROUND, STRAINED NECK. SHOT OVER 3/4 MILE AWAY. HE DID NOT REPORT ANYTHING TO THE MINE OR HIS SUPERVISOR. SOUGHT TREATMENT OVER A WEEK LATER THEN INFORMED SUPE RVISOR IT WAS AN ON THE JOB INJURY.
TRUCK WAS BROKEN DOWN AT BASE OF HAUL ROAD WITH TRUCKS STOPPED WAITING. WHEN OPERATOR APPLIED BRAKES THE TRUCK DIDN'T SLOW SO HE PULLED THE PARK BRAKE. WHEN THIS FAILED TO STOP THE TRUCK HE WE NT ALL THE WAY UP AN ESCAPE RAMP. AFTER A FEW SECONDS THE TRUCK TURNED ONTO THE DRIVER'S SIDE. THIS RESULTED IN MINOR BUMPS AND BRUISES TO THE DRIVER.
EMPLOYEE WAS TRAMMING TRUCK DIAGONALLY UP RAMP COMING FROM PIT WHEN HE TURNED THE TRUCK ON IT'SSIDE.
