Employee was moving a piece of plywood and smashed finger between it and concrete causing a small fracture and cut to little finger.

Operator was removing material from the Borden shot area. He pushed material off the highwall side of work area and traveled over the highwall coming to a rest on the Sewickley bench below. Emergency rescue personnel extracted the operator from the dozer and took for medical evaluation as a precautionary measure.

Injured employee mistepped and fell while dismounting a grader after turning on the equipment master switch. He fractured his left wrist in attempt to brace himself while falling.

Injured employee was struck by a hoist cylinder when it unexpectedly broke loose from lifting devices, as it was being removed from a 777D rock truck. The 630 lb. cylinder struck his left leg as he was kneeling in close proximity to the work area fracturing the femur and fibula in his left leg.

Injured employee was assisting maintenance department replace a pin on a CAT 365 excavator bucket. As injured employee and another employee were lowering the pin onto the back bumper of the welding truck, the pin slipped smashing the right finger of the injured employee. Incident resulted in a hand laceration, fractured finger tip and dislodge fingernail.

Worker was cleaning debris from roller on raw coal belt. Belt was locked out and tagged. As debris was removed, stored tension caused the belt to move trapping the workers right hand and arm between the roller and belt up to his shoulder. Initial injury report is a cracked ulna bone with possible torn ligaments. Further test and injury results pending.

Due to horseplay, injured EE was pushed, lost his balance & twisted his shoulder as he caught himself. Dr. determined EE had strained shoulder muscle which resulted in 1st aid incident. He was revaluated by an orthopedic surgeon & determined to have a torn rotator cuff. Surgery conducted on 1/17/13. At a follow up on 1/21/13 the dr wouldn't release back to modified work.

Afternoon shift arrived at equipment staging area,a D11 dozer was found to be fully engulfed in fire. No EE were involved. Dozer had been operated on day shift and parked at end of shift in the equipment staging area as normal. The local fire dept responded and extinguished the flames. No personal injuries occurred. The cause of the fire is still under investigation.

While scaling loose material from the corner of the #4 Entry at the #21 Block, employee was struck by loose material in the right lower leg. Resulted in a fracture to the second metatarsal on the right foot. X-Ray could not be confirmed until 4/30/2012 due to inflammation. Employee worked on modified job duty until 5/7/2012 then went out as LTA due to Corporate policy.

Injured was assisting coworkers to remove hydraulic hose on portable screener. Zip ties holding the hose had to be removed to take hose off. EE was holding the hose with his left hand and used his pocket knife to cut zip tie off. While cutting, his hand slipped, lacerating his left hand between the thumb and pointer finger. Resulted in four sutures to the laceration.

Employee reported on 1/25/2012 that while driving a CAT 785 Rock Truck he hit a bump in the haul road causing him to bottom out on his seat resulting in a lower back strain. Following a complete review of the incident, management was unable to determine validity of the report and is filing this 7000-1 according to Part 50 recommendations to report questionable injuries.

EE injured was working in the area around a D11R Dozer. He stepped on a rock that was covered with mud. On the next shift he reported incident to superintendent and denied medical treatment offered. That evening the EE visited a Dr. for evaluation. The EE injured deemed to have a level 2 sprained/strained ankle. Investigation pending by company.

EE heard a noise in the truck shop so he unlocked it & went inside to check it out & a man came full force at him. Employee pushed him away & then the man punched him in the nose. EE hit him 2 or 3 times in the face with his fist & the man landed on the floor. EE put his knee on his back & heard footsteps behind him then was hit in the head. He was unconscious for about 40 minutes

Employee was walking through the plant and stepped in a hole causing him to sprain his left ankle.

EE was proceeding down the haul road when he up shifted his truck from 1st gear to 2nd gear, when he shifted he shifted the transmission to the high side causing it not to go in gear. He proceeded down the steep grade out of gear and riding his brakes approximately 900ft before coming to a left hand turn and turning truck over on its side.

Employee was dismounting dozer after positioning the equipment for maintenance. Upon dismounting he raised the lid of the battery box to gain access for required task. In traveling down, he placed his hand on edge of battery box, the locking spring mechanism, which was supposed to hold lid open failed catching employee's hand. Resulting in a laceration to hand, needing 3 sutures.

EE felt sick at work and wanted to go home. His daughter came and picked him up and took him to the doctor. Drs at hospital thought he had a mild heart attack. At that time company reported to MSHA. The following day it was determined that it was not a heart attack but his gallbladder.

The bolter blower had been repaired and the lids were ready to be replaced. EE injured was assisting another EE to lift the lids back into place. He bent down to lift up the lids and while lifting turned slightly. Upon turning he felt a pull in the lower left side of his back and dropped down to one knee in pain. The EE was transported to area hospital for medical evaluation.

EE exited the cab of the CAT992G to remove debris from the equipment. While working to remove the debris EE lost his hard hat on the ground. Upon returning to the cab struck his head on the side mirror lacerating his scalp. The laceration required 4 staples to close.

Upon further investigation it was noted that the principal injured fell, striking his head on the batteries as a result of horseplay. The injured employee jumped on the back of another employee & he fell to the ground when the uninvolved employee leaned forward. When the principal injured fell forward this resulted in a small laceration above his left eye which required 2 sutures.

The employee was seriously injured when he was caught between the rib in the #9 heading and the #2 roof bolter machine being operated in the No. 1 North East Main Section. The bolter was hung up and was being pulled by a scoop. The roof bolter slid to the left pinning the employee. The employee sustained a fractured pelvis and required hospitalization as a result.

Principal injured was removing the service jack off the side of the #2 Shuttle Car. He took bolts out of the jack and was removing for maintenance. The jack fell pinching his hand against shuttle car and lacerating middle finger. Employee cleaned and bandaged the wound and completed his shift. He later visited a physician and received thirteen stitches to his middle finger.

Employee was working to remove the teeth from an excavator bucket. After cutting the teeth from the bucket EE used a sledge hammer to dislodge the teeth. While pounding the teeth off the bucket a piece of metal broke from the hammer striking his leg. EE was taken by ambulance to Western Maryland Health Systems where he received 5 stitches to repair the laceration.

While moving a light plant, ee was attempting to lower the tongue of the light plant on the hitch. The tongue slipped off the ball pinching the end of the thumb on his left hand. The injury resulted in a torn fingernail and fracture of the tip of the left thumb.

The employee was operating a Cat 777 rock truck headed to the dump site when he hit a bump and was jarred injuring his lower back.

The injured employee was exiting his vehicle to enter the office when he slipped and fractured his right ankle.

EE was attempting to climb onto the 850 John Deere dozer. He had placed one foot on the step. When he tried to place the 2nd foot on the step, that foot slipped in the snow. He was holding onto the handrail. When his foot slipped, it caused him to twist and fall against the tracks of the dozer, pulling a muscle and pinching a nerve in his back.

Employee fell while getting down from grader. Foot slipped, causing fall. Tried to break fall by putting hands out, causing strain on left hand. He reported to supervisor, refused medical treatment. Went to ER next morning 11/30/10, and had hand X-Rayed. Employee came to work that day, missed no time. Hospital notified him 12/5/2010 with the results, confirmed his hand was broken.

Employee went to trouble shoot a leveling sensor located at the top of a silo. He claimed up the ladder and open the hatch at the top of the silo. When he looked inside a gust of wind caught a hold of the hatch door (30" x 30" x 1/4" steel plate on hinges) striking him in the back of the head causing him to hit the bridge of his nose on the steel frame opening of the hatch.

The injured miner was attempting to remove a rock lodged between the dual tires on a fuel truck when one of the tires ruptured. The injured employee received foreign material to the eye and face region requiring embedded material to be removed from his eyes.

Employee was assisting in the removal of bearing and was hit in forearm with sledge hammer.

Checking density on truck and saw a cap falling in hole. Bent over to get it and felt a pain in back.

A blast was detonated causing material to slough off the outside of a hillside near the shot. The material displaced mud from an existing pond creating a wave of mud. EE's truck slid down an embankment, causing a contusion to his head.

After parking the Fuel/Lube Truck employee walked behind the truck where he slipped/tripped on the frozen ground resulting in a sprained ankle.

At approximately 8:30 pm Injury descended the fuel truck to re-fuel a rock truck in the pit area. While walking to the rear of the fuel truck he stepped on a rock and twisted his left ankle.

At the beginning of day shift on November 18, 2008 EE was attempting to enter his dozer to begin the pre-shift examination. The door of the dozer was frozen due to snow and below freezing temperatures. When he pulled on the door handle and it broke causing him to lose his balance and fall backward off of the dozer striking his head on the ground.

While descending a Cat 988F Loader, the miner slipped and caught himself, causing a strain to his right shoulder.

Lifitng bags of dirt and felt burning sensation in stomach and a lump in side.

Employee was using his finger to hold a keeper washer in place instead of a screw driver while putting a guard on a ripper shank. The guard slipped and cut the tip of his finger. Employee was taken to the hospital where he received 2 stitches to close the wound. He returned to work the next scheduled shift. No lost time.

Employee went to use a port-a-pot. As he opened the door he was attacked by bees. Employee ran from the area and slipped and fell landing on his right side.

Employee was working underneath a water truck and was stung by a bee. Shortly thereafter he started to exhibit anaphylaxix systems. He was taken to the hopital, treated with injection and intravenous medications. Following treatment he was released and sent home. He returned to work on his next scheduled shift.

Employee was attempting to lift and place a cylinder in the service truck. With the crane controls in right hand, he was attempting to spot the cylinder inside the service truck with his left hand. The load started to swing and smashed the tip of middle finger of left hand between cylinder and side of truck. ER doctor applied a splint and released him to return to work.

Employee was operating an impact air wrench in an overhead position. The injury was reported to his supervisor on 6-27-08 where upon he was taken to the hospital for evaluation. Employee was diagnosed as having pinched nerve in left hand. Employee was told to wear a provided wrist brace for 5 days and to return to work the next scheduled shift.

After pre-tripping my truck, I saw who I thought was the attending supervisor for J & W Excavating. I needed to tell him I had to leave at 2:00 pm that day. As I went between the truck & the building to tell him, I tripped & landed on my left knee which caught the corner of a metal pan filled with water that my right leg had fallen into (causing the cut to knee).

A contractor was reteiving some drag line parts from an abandon truck shop. One of the contrator employees was attempting to disengage a parts rack with a cutting torch. Sparks from the torch caught the back wall on fire. The fire quickly spread to the roof and was immediately out of control. Local fire departments were summoned, but the building was a complete loss.

Employee was holding hood door open while checking oil. A gust of wind blew hood out of his hand causing hood to fall hitting him in back of head & neck. He was wearing his hard hat.

As the employee got off the 385-b to go to the 400, he was about one step away from the 385, when his foot slipped on the ground. He reached out to catch himself, his hand caught a rock.

Employee was cleaning the belt take up roller on the plant feed belt with a hammer. His right hand and lower arm got caught and pulled into the roller. This event is under further investigation.
