The EE was helping put the conveyor chain of a C/M together. EE was hitting a master link pin with a small sledge. When EE hit the pin a small splinter of metal hit EE in the right pinky finger.

An employee was transferring from a cricket to a mantrip. While the employee was exiting the cricket it rolled pinching the employees leg.

An employee was tramming a scoop loaded with supplies to load a roof bolter. As the employee trammed around the corner of the crosscut, EE grabbed the scoop canopy to pull self up and reposition self. The employee scrapped EE's finger between the scoop canopy and the coal rib, lacerating EE's finger. The employee received 7 sutures.

Am employee was releasing a chain binder on a flinger duster with a roof bolter wrench. When the binder came loose the wrench hit the employee in the forehead causing a laceration. The employee was taken to the Emergency Department and received 9 stitches, then was released to come back to work with no restrictions.

An employee was jerking on ventilation curtain that was tangled in miner cable. The employee stated ee went to take a step after retrieving the curtain and felt pain in back that caused ee to drop to ee knees.

An Examiner was traveling to MMU 001 section and found a fall in the #3 entry spad #12+60 on 5 belt. MSHA Hotline was called at 1:15 A.M. to report the fall. The fall damaged some belt structure and the outside structure of a VFD drive.

On 11/15/2017 an EE was separating sections of 4" waterline with a crib block. The crib block slipped off of the joint of water line and EE struck finger between the crib block and a fire valve. The EE fractured the tip of finger.

A buggy op was tramming S/C and leaned in to travel around a corner, EE accidentally hit the e-stop bar and stopped the car. it bounced the EE and EE struck EE's arm on the cab. EE was seen at the ER and released with a bruised arm. The EE went to a follow up appointment in the A.M. and EE's arm had swelled over night, the DR. made an incision to reduce swelling.

On 8/02/2017 a Roof Fall was found in the Main Return at the 1st North #2 Belt at 5x-cut. The fall was found in the #1 and #2 entries. The fall was called into the MSHA hot-line.

An EE parked EE's scoop and exited to wait for the shuttle car to tram by the EE was standing between the rib and the scoop bucket. The scoop bucket was sticking out past the corner of the rib, when the shuttle car was tramming through the entry and slid into the scoop bucket shoving it against the EE and pinning EE between the scoop and the rib.

A scoop operator, was dragging a structure tray behind the scoop tramming inby. The injured ee was walking behind the scoop and actually walked into the tray, the tray caught on a piece of rail and flipped the tray in the air causing the ee to go airborne. The ee hit the mine floor and the tray came down on top of EE's lower torso.

A Foreman called outside to verify the Intake phone was operating. The phone would not work so the employee walked the intake to find the problem. The employee found a roof fall at x-cut 26 in the #8 entry on the North East Mains #2belt. The fall was 4' to 6' High x 17'4 ' wide x 30' long.

The injured EE went to hang ventilation curtain, while his operator put up a corner bolt. The EE hanging curtain went back to get nails. The Operator started repositioned the bolter, not realizing his helper came back. When he started moving the bolter he bumped his helpers hip.

The Employee stated that he was replacing an area light on a continuous miner. The Employee started to cut the conduit that entered the area light, the Employee was cutting towards himself. The knife slipped and lacerated his left palm. The Employee received 6 stitches in his left palm.

The roof started working at spad #22+20 on MMU 006. shortly after the roof started working it fell in entry numb 8. The fall was approximately 18 feet wide x 100 feet long x 12 feet high. The fall was at 11:34 a.m. and was called in at 11:47 a.m.

The Employee stated he went to help the scoop operator dust the faces. The Employee stated he was gathering the dust hoses for the wet duster and didn't realize he placed his foot under the scoop bucket. The scoop operator lowered the scoop bucket and did not know the other Employee had his foot under the edge of the scoop bucket. The scoop bucket lowered onto the helpers foot.

The Employee was operating an off standard center drive shuttle car, after being loaded by the continuous miner, the employee swung around to the inby seat with out setting his parking brake. When the Employee was swinging around to the inby seat, the shuttle car started rolling and caught his right leg against the rib. The result was a fractured tibia.
