IE was walking across the supply yard toward the warehouse storage building. While walking across the snow covered ground, the IE turned EE's body to speak with another employee located at the fuel pad. During this time, the IE stated EE took a step and slipped, falling to the ground on EE's left side, fracturing EE's left hip.

The employee was using a 6-inch electric handheld grinder with a cutoff wheel. The employee was cutting a steel bar grading walkway when the grinder kicked back and struck EE's right thumb. The grinder caused a laceration just above EE's right-hand thumb knuckle that required 5 sutures to close.

The IE was descending stairs from the second floor in the Prep Plant. When the IE reached the landing between the 2nd and 1st floors, EE turned right with EE's left leg to start descending the next flight of stairs. As EE stepped down with EE's left leg, EE twisted the left knee.

IE had kneeled to tie IE's boot lace, when IE was struck by a section of coal that came loose sliding down the coal rib on the Unit 3 working face in Entry #7 at XC 43 on the 3L submain. The IE suffered several small fractures including the left fibula and a portion of the lumbar vertebrae

Employees were unloading metal undercast panels from a 5th wheel trailer into the bed of a coal hauler. The top two panels slid partially off of the stack, a panel got stuck, EE's hand slipped across the end of the metal panel, causing a laceration to EE's left hand. IE was transported out of the mine and taken to a local medical facility where EE received five sutures.

A mantrip containing 8 employees was traveling outby on the 9W roadway. While turning onto the 8W roadway the driver turned the mantrip too wide, struck the outby corner of the intersection, resulting in one employee contacting the metal frame resulting in a fractured tibia.

IE stated that EE was entering into the rear seat of a mantrip located behind the front passenger seat with EE's hand on the upright canopy support post. EE stated that another employee closed the door to prepare for departure smashing EE's right ring finger between the door and post. The IE was transported to the local ER for treatment where EE received four sutures in the finger

IE stated EE was backing a tractor with an attached trailer down hill. EE stated EE put EE's hand on the frame on the frame of the machine, outside of the cab, to help turn EE's body toward the direction of the trailer. EE accidentally depressed the throttle causing the tractor to turn slightly, smashing EE's finger that was resting on the machine, causing a fracture.

While tasked to fuel pit equipment, the employee crested ramp going to 5 pit when EE lost the front brakes. This caused the truck to descend ramp faster than normal with just the rear brakes working. The employee experience neck pain later in the shift.

On 01/02/23 a prep plant employee was descending a (3) step walkway over the Magnetite Chute. The IE slipped on the second step twisting the left knee. The injured employee continued to work until 08/28/23 where IE underwent surgery to repair a tear in the knee.

IE stated was exiting the operators compartment of the diesel scoop and stepped on a rock. IE stated that when did this, it caused IE's right knee to twist. IE stated heard a popping sound and instantly felt like knee was on fire. IE began missing work on 12/21/2022 when MRI results revealed a torn meniscus.

IE was drilling a hole to install a roof bolt. The drill steel became damaged and struck the IE in the right hand between the thumb and index finger causing a laceration. The IE received 5 sutures to close the laceration.

Employee was preparing a piece of plate for installation; Plate was clamped down on saw horses and when clamps were released the plate slide off the saw horses which came down on employee's lower right leg and made a laceration.

A qualified surface electrician was working inside of an electrical cabinet to close a hole temporarily until the cabinet could be replaced. EE took out a utility knife to cut a piece of plastic. While EE was making the cut, the blade slipped, slicing through the tip of EE's left thumb. The employee was taken to the local ER, where sutures were added to close the laceration.

IE was loading a spool of cable into a scoop bucket and felt a strain in their right shoulder. IE continued to work doing therapy on the shoulder. On 12/9/2022 the employee had an arthroscopic procedure completed to clean up a micro tear and bone spurs and began missing work.

An employee from Contractor BB0 was in the process of removing the outer jacket from an electrical cable at Peabody's Columbia Rebuild Center. The next day the employee presented with pain in left wrist...contractor was advised to see a doctor. Was initially given steroids to reduce pain and swelling and no lost time injury occurred.

IE was walking belts with a co-worker as part of the underground training program. While working on the opposite side of the bottom belt and EE's co-worker, the IE had turned EE's head to watch the belt, striking EE's left ear with a piece of hog panel/wire mesh. The IE was and taken to the emergency medical facility and received nine sutures to close up the laceration

IE was removing jack stands from a mantrip using right leg to slide the jack stand out from underneath the mantrip. When doing this, IE stated that knee popped with no immediate pain. Over time IE states that pain has progressively gotten worse causing IE to not be able to bear full weight on right leg. IE was taken off work by a medical doctor on 11/3/2021

An unexpected roof fall occurred on the 2W Main in entry #3 (primary escapeway) at cross-cut #5. The fall measured approximately 18'x 18'x 6+' in thickness.

EE stated that as they were unloading a trailer from a tractor using a ratchet strap. EE was pulling up on the tongue with right arm and pulling on the strap with left arm, when they felt a pull or strain in right shoulder. Employee continued to work until 9/8/2021, when they had surgery for a torn rotator cuff.

IE stated that while trying to unplug the charging cable from the battery for # 14 scoop, the charging cable was stretched over the battery lids. As the IE pulled on the cable to get some slack to unplug the charging cable, the cable pulled on the lid enough to slam closed on IE's left pinky finger as EE was disconnecting the charging cable plug, causing a laceration and fracture

IE was moving #31 remote controlled bolter forward from entry 6-7 to entry 5. When the bolter went through a curtain in 6-7, the cable got caught on the bolter. As the IE went to reach for the cable to move it, EE believes they hit one of the tram buttons causing the machine to shift and pin themselves between machine bumper and the coal rib. IE was treated for a fractured pelvis.

IE stated was operating a coal hauler. IE stated struck wrist on a sharp piece of wire, while removing their backpack from the equipment. When doing this, IE's wrist came in contact with the wire causing a laceration approximately 1.25" on the inside of IE's left wrist. On Friday April 23, 2021 IE was seen by local doctor and taken off work for sutures that was added.

#4 Roof Bolter was moving to 7R. The IE was helping move the bolter by hanging cable. One employee was tying the wire while the IE was holding the cable. IE stated the cable got tight and broke loose from the tie wire. IE stated the cable snapped IE's arm downward causing a sharp pain in shoulder.

IE stated was pushing a rock, sized approximately 4' in length by 1.5' in thickness, into the miner conveyor. The rock started to tip up into the conveyor, when the IE's hand was pinched between the edge of the rock and to top of the mine roof, causing a laceration to the inside part of IE's left hand index finger.

IE stated that while EE was operating a man trip, EE reached over to turn the lights off due to oncoming traffic. At that time, the IE ran over a large rock causing the IE to be lifted off EE's seat, striking EE's head on the top of man trip, then back in EE's seat. The IE stated EE did not feel immediate pain. On 4/19/2021 IE was taken off work for further evaluation.

Employee was kneeling down inspecting a roll of belt. A rock measuring 4.5"x3'8"x7" fell from approximately 6', striking the employee in the head, shoulder and back.

An employee was pinned against a coal rib by a coal hauler. The IE had walked to the #5 entry to assist another operator establish an alignment marker on a coal rib. As IE was walking back to IE's equipment, hauler (B) was traveling from the #4 entry towards the feeder and contacted the parked hauler (A) located in #5 entry, allowing hauler (A) to slide, pinning the IE to the rib.

IE stated was stepping up on a ladder to hang a line curtain. As IE reached the second rung of a 4' ladder, IE reached up to grab the hog panel to stabilize self, the ladder began to tip, causing IE to fall to the ground. IE began to complain of back pain. IE was taken to a medical facility for observation.

IE was installing a bent 6' bolt into the previously drilled hole. EE stated as they were pushing the bolt into the hole from the ground, they felt a sharp pain and popping sound in lower back. EE stated they finished the bolting the entry and moved to entry #9. EE continued to bolt 3-4 rows before coming outside to go the medical facility to be checked out.

IE was using pieces of concrete block to fill in the holes on the sides of the wall IE was building. As IE was using a rock hammer to fit a piece of block into place, IE struck left index finger between the block and the hammer, causing a laceration to the index finger. IE was taken to the medical facility for treatment where IE received sutures to close the wound.

IE stated that EE attempted to exit the mantrip, IE opened the door and stepped out with EE's left leg. IE then grabbed the top of the door frame with both hands to pull self out and up. IE's right leg was bent as EE was standing up. When EE stood up, EE felt a sharp pain in EE's right knee.

IE stated that EE was examining the belt line. IE slipped and began to fall forward. When EE tried to catch self from falling forward, EE grabbed the belt chain with EE's right hand and hog panel with EE's left hand and then felt pain in both shoulders. The IE had surgery on 5/15/2020 making this reportable.

IE was packing 60" top belt rollers to supply trailer when IE tripped over a stick of 6" waterline. IE fell into the supply trailer striking IE's left knee and thigh. IE received a 1" laceration on IE's inner left thigh, which required 6 sutures to close the laceration.

IE stated was outby the unit getting some metal straps for the bolter. IE grabbed 3 of them through the holes in the straps. As IE pulled on them, one of them got caught on something causing EE's hand to get caught between the strap holes causing a laceration to the right hand pinky. IE was brought out and for treatment and received sutures to close the wound.
