The employee was positioned at the inch control tram controls of the Fletcher HDDR Roof bolter and fell, striking the right side of EE's head, resulting in sutures to EE's right ear.

Positioning bolt into 6" wrench. Operator inadvertently bumped rotation and burrs from wrench caught glove and operators hand was wrapped around bolt causing laceration to palm of hand underneath ring finger extending to middle finger.

Employee was tightening lug nuts on EE's permissible 4 wheeler when the lug broke and caused EE to stumble and roll EE's ankle and jolted EE's right leg. EE hairline fractured EE's right fibula and was seen at hospital and was instructed to heal normal and is allowed weight bearing on that leg.

The injured employee and a 3rd Part Electrical Engineer were troubleshooting a permissible line splitter. A faulty monitor was identified, the 110 volt circuit was de-energized and the employee proceeded to remove the monitor. Both electricians believed the high voltage was de-energized and during the monitor removal, the employee contacted a phase of the energized high voltage.

Employee had a hand on the canopy of the shuttle car and the other hand holding a roof bolt plate while talking to the operator of the shuttle car. The operator started to pull away and employee allegedly felt a "shock". Employee continued to work for approximately 30 minutes after the incident at which time they started experiencing shortness of breath and sweating.

Employee was unhooking chain from buggy that was being pulled with a scoop. As they were unhooking the chain the scoop inadvertently rolled and as they pulled their hand out the cotter pin caused a laceration to their right index finger and thumb.

Workers were attempting to use the excavator bucket to install sheepsfoot attachment to the roller. EE then reached under the bucket to tighten a bolt, resulting in the bucket slipping off the sheepsfoot catching EE's arm between the bucket and the roller frame.

Worker was cutting with a torch. Employee had on safety glasses but was not wearing a face shield. When the metal was heated a piece of rust came loose and went under the workers safety glasses and into their left eye. The rust particle caused a corneal abrasion in their left eye.

Employee states water valve was leaking. When EE attempted to tighten the fire valve it blew off and struck EE in face and mouth area. EE was transported outside to an awaiting ambulance and transported to a hospital.

Employee stated that when they were tramming a rubber tired ride through a curtain that it knocked their safety glasses off and the curtain came in contact with their eye.

Employee was unloading a fork for an IT28 fork loader from the bed of a flatbed pickup truck. When EE slid the fork back and it cleared the end of the truck bed, the fork rolled and pinched the tip of EE's right pinky finger between the fork and the truck bed. Result was a simple fracture to the tip of EE's right pinky finger.

Employee was trimming a dam for a screen panel. The employee was cutting it with a utility knife. The knife slipped and cut employee's right leg.

Employee stated they were riding on a MAC8 on the left side near the rear. They rounded a corner from the scoop roadway into the power entry and a rock lying near the corner pinched their hand on the frame of the MAC8. This caused a puncture wound on the left hand between thumb and index finger which required 2 sutures.

The employee stated was jacking up a personnel carrier when it slid pinching EE's hand between the jack bar and the machine. This caused a laceration to EE's hand between the left thumb and index finger.

Unwitnessed, the employee reported was on knees and went to pull down curtain, felt pain in the left knee. The operator investigated and determined there was no mechanism of injury for the reported condition. This report is submitted to abate citation 9246538 issued on 12/28/2020. This is a Degree 10 incident.

Employee stated that EE tripped over a 4 inch pipe and fell on EE's right knee. EE was provided First-Aid treatment at a local medical facility and returned to work on EE's next scheduled shift. The EE's condition was diagnosed on 6/24/2020 as being a work related injury and EE is unable to return to work at this time.

The employee stated was pulling the drill steel out of the rib and accidentally bumped a control causing left pointer finger to be pinched between the drill chuck and drill steel. This resulted in a partial amputation of the left pointer finger (first knuckle from tip).

Employee stated that on 9/27/2019, while operating a grinder, a small sliver of metal struck EE's left eye. This incident wasn't reported until 10/07/2019. The employee received medical treatment for the left eye, also on 10/7/2019. The incident became reportable at the time of the treatment. The employee did not miss any days work and had no restrictions from the doctor.

Employee stated that while lifting a toolbox off of the battery lid of a rubber tired ride on 07/17/2019, they strained abdominal region. The employee reported this incident on 07/18/2019. The employee was later diagnosed with a hernia and was treated for this issue on 10/1/2019. The employee did not miss any scheduled work and has no restrictions from thier doctor.
