THE EMPLOYEE WAS RIDING ON A RUBBER TIRED PERSONNEL TRANSPORTER HE SAT UP TO TIE HIS BOOTLACE AND STRUCK HIS HEAD ON THE HEAD OF A CABLE BOLT. THIS CAUSED HIS HARD HAT TO STRIKE HIM ON THE BRIDGE OF HIS NOSE RESULTING IN A FRACTURE.

The injured and another person were installing conveyor belt structure. The two men lifted a bottom roller and were putting it in position when the injured's left knee buckled.

A lift was bein mined from a pillar on the 2-left off 2 south panel when a piece of rock fell on the head of the mining machine and fouled the machine. A retriever was hooked to the machine and the machine was free in approximately 10 min. A rock fell inby the left row of roof bolts and no belts were damaged. This report is submitted in protest to satisfy a citation.

The individual was cleaning the water filter at the end of the hard line. While working on the filter, he caught his finger between the water filter and the conveyor belt tail piece resulting in a laceration to his left middle finger.

THIS AREA WAS MINED IN 1998 AND IS APPROXIMATELY 4 MILES OUTBY THE ACTIVE FACE. THE FALL CAUGHT THE BELT CONVEYOR FOR THE MINE AND WILL BE CLEANED UP. THERE WERE NO INJURIES INVOLVED.

EE WAS TRAMMING A RUBBER-TIRED UTILITY VEHICLE AND DROVE OVER A 4'-LONG METAL ROOF JACK. THE METAL ROOF JACK FLIPPED UP AND STRUCK THE EE ON THE HEAD, RESULTING IN A MILD CONCUSSION. An investigation revealed that the alleged injury was falsified by the employee.
