THE EE REPORTEDLY INJURED HIS BACK ON 2-19, BUT DIDN'T REPORT THE INJURY UNT IL 2-23, CONTINUED WORK INTERMITTENTLY THROUGH 3-3-99. THE EE WAS ENCOURAGED TO SEE A PHYSICIAN ON 2-24, BUT HE DECLINED. ON 3-3 THE EE DECLARED HIMSELF UNABLE TO WORK ANY LONGER AT MIDSHIFT
