Employee was looking for the end of a hydraulic hose. He placed his hand on a pipe, his hand slipped and caused him to get against the hot hydraulic pump.

Employee was prying on one side of the belly pan(pan was hooked to the boom, holding the weight)He was pulling against it with a slate bar to line up the hinge pin holes. He felt a kink in his lower right back and he thinks he pulled a muscle. The muscle got tighter as the shift went on. This injury was not ruled work related until June 8th, 2013.

Injured was trimming a speaker mount with a razor knife, it slipped cutting wrist slightly, first aid was rendered. EE transferred to hospital for professional care.

EE was helping put a cutter chain back on the sprocket when the chain slipped and pinched his left hand between the links. This resulted in a fracture to the top of his hand behind his little finger.

Employee was exiting a truck when he slipped on the step and hit his right side rib area on the cab. He finished his shift, worked the next day and sought medical attention on 9/30/2012 and was diagnosed with a strained muscle in the rib area.

Employee was a passenger in the HD2500 chevy portal truck when the driver lost control. They were traveling to the pit. The truck veered off the road, striking a rock embankment. EE's head hit the windshield post on impact.. He was not wearing an seatbelt

Employee was operating a Chevrolet 2500 hd pickup enroute to the pit. He said he heard a noise, panicked and hit the gas pedal, veering into and striking the embankment. His head hit the steering wheel. He was not wearing a seat belt.

Employee stated that he hurt his back while walking away from his truck.He went to the hospital and was released. He came back to work and worked two days then went back to hospital and was put off work with a strain. We are disputing his comp claim due to the fact he told other employees that he hurt his back off of the job at home.

Employee was making a grease line for RL4510,when he used a razor knife to cut a zip tie. The razor slipped and cut his middle and ring finger on the right hand.

Employee was lifting old "h- bracket" with boom when brace got hung. He then lowered the brace back down when a chain came unhooked, dropping the brace down approx. 12 inches on his left foot.

mine mechanic was assisting, Injured mechanic install a steel bushing in a link using a metal hammer, when mine mechanic struck the bushing it fragmented creating a projectile which penetrated EE's lower left calf. The fragment was to deep to be removed locally, required trip to emergency room for removal.

Employee was standing between two pump motors. He and another employee was attempting to move them. Each motor weighed approx. 800 pound each. As the first one was lifted, the second one became unstable and fell, pinning EE's leg under it.

A pin keeper that holds the tooth on the adapter was being removed when a piece of the adapter broke off and flew through the air approximately 5 to 6 feet. An employee was walking past and was struck in the nose.

HWM pad man was washing the walkway off when a piece of coal became dislodged and flew from the belt, striking him above the left eye. This cut required 4 stitches.

EE was coming to work when vehicle he was in slid into another vehicle. EE hit the rear view mirror with his head and caused a laceration to his head. EE was taken to Raliegh General Hospital where he received 11 stitches to the left side of his forehead.

As the 5500 excavator was pulling overburden there was an unplanned detonation of an explosive material. There were no injuries. There were no damages to the equipment.

As the 3600 excavator was pulling some wall down there was an unplanned detonation of an explosive material. There were no injuries. The damages were only to the windshield and paint of the excavator. This event is still under investigation.

EE was loading the powder truck, standing on top, when the ANFO at the top of the bin broke loose causing the bin to shift abruptly hitting him on the right shoulder. At the same time he felt pain in his lower back.

While doing his normal job position, employee started having symptoms of lower back pain. After extensive testing the results show he is suffering from the occupational illness of degenerative discs. Per 2/5 document 220102230004, EE was doing walk around his truck, when he slipped and fell on flat ground.

Employee was welding while working off of scaffolding at a height of 6 feet 3 inches. He fell between two scaffold boards and landed on his knees, resulting in a back injury.

Employee was following other employee up the "Woods Road" to the Twilight Mine. At the 31 curve. He stopped to let other employee go up the hill 1st, almost at the top he (other employee) spun out sliding down the hill striking his truck on the front drivers side(fender and door).The roads were snow covered and slick.

Employee was putting the hose back on the pulley of the repump truck. After finishing he went to climb down the ladder, when he twisted he felt a burn in back. He finished the shift and decided to seek medical attention on 2/4/10.

Installation of part (snubrite) on mining shovel bucket was being performed. Lifting sling failed, causing a component of the snubrite to strike the employees left finger. Employee was diagnosed with fractures to left index finger.

Employee was exiting the 495 shovel when the ladder shifted and he fell approx. 3 feet to the ground. His left knee struck a rock and caused a small laceration. He was taken to the Boone Memorial Hospital where he received 8 stitches to his knee.

Employee was spotting for another employee that was operating a forklift. The forks on the fork lift got in a bind on the load that was being picked up. The fork came off the lift and smashed the spotters finger between the fork and the load that was being moved.

Employee was assisting another employee with lifting a piece of plate (1"x6"x10'). He felt a sharp pain in his Rt. Shoulder and was sent to the doctor. The doctor released him back to work, but set up further testing. On 8/18/2009, the specialist put him on light duty restriction and set up an appt. with a surgeon.

On 7/24/09 the intial accident occured, employee was attempting to mount a D-9 Cat Dozer. As he was mounting, he heard a pop in his knee. He was taken to the hospital where he was referred to a specialist for an MRI. He was returned back to work full duty. On 8/7/09, he went for the MRI reading and found a tear in his meniscus and was taken off work.

Employee was walking on job site with welding equipment in hand and fell on a loose rock. When he fell, he hit his head & neck on a welded support on a service truck. Initially medical evaluation came back negative and he was referred to a specialist. On 6/2/2009 specialist prescribed physical therapy & meds making the injury reportable.

Employee was doing his preshift exam on his 930E haul Truck, where he found glass in the cab of his truck, due to a window installation. He then began to clean it up when he felt a slight burn in his right eye. He then reported to his foreman and was transported to Boone Hospital. He was treated with eye drops & returned to work on his next shift.
