A ROOF FALL MEASURING 12' BY 15' BY 6' THICK OCCURED ALONG #7 BELT/ TRACK HDG (#5 HDG) IN THE XCUT #5-#6 AT SPAD #2297 1 BRK OUT BY SPAD #2337. THIS IS A NEUTRAL AIRCOURSE HOWEVER THE TRACK HA S BEEN CLOSED OFF UNTIL THE FALL AREA CAN BE CLEANED AND RE-BOLTED, THE AREA WAS BOLTED WITH 48 IN RESIN BOLTS. THERE WERE NO INJURIES.
EE WAS LIFTING THE COUPLER TO RAISE IT INTO POSITION TO COUPLE TO A MINE CAR WHEN HE STRAINED HIS BACK.
A ROOF FALL MEASURING 12' BY 6' BY 4' THICK OCCURED ALONG #3 TRACK BELT, 2 BLKS IN BY SPAD #636,THIS IS A NEUTRAL ENTRY AND AIRFLOW WAS NOT IMPAIRED. HOWEVER THE TRAVELWAY IS CLOSED TO RAI L TRAFFIC UNTIL THE FALL CAN BE CLEANED AND SUPPORTED. THE AREA WAS BOLTED WITH 48 IN RESIN. THERE WERE NO INJURIES OR OTHER LOSS.

A 6:10 PM THE GUARD NOTICED SMOKE COMING FROM THE MINE FAN. A FIRE WAS FOUND ON THE SLOPE BELT AT THE OUTSIDE AIR LOCK DOORS. FAN WAS SHUT DOWN AND POWER KNOCKED. FIRE WAS EXTINGUISHED BY 7:00 PM. WATERING DOWN AND CLEAN UP OF AREA CONTINUED UNTIL 12:30 AM. FAN RESTARTED @ 1:00AM. INSPECTED SLOPE UNTIL 2:15AM. RESTORED POWER 2:55AM A FTER PRESHIFT OF MINE. WELDING EARLIER, BELT BUR

A ROOF FALL MEASURING 18' X 40' X 5' THICK OCCURRED IN THE #1 ENTRY 1 ENTRY OVER FROM SPAD #1571. THIS IS A RETURN ENTRY BUT AIR FLOW WAS NOT OBSTRUCTED. AN ALTERNATE TRAVEL WAY WAS ESTABLISHE D AROUND FALL. AREA WAS BOLTED WITH 42" RESIN BOLTS.
A ROOF FALL MEASURING 20'X50'X5' THICK OCCURRED IN THE #6 RETURN ENTRY 1 BRK OUTBY SPAD 1570. RETURN AIRFLOW WAS ONLY MIDLY OBSTRUCTED, BUT TRAVELWAY WAS BLOCKED.
WHILE DRILLING BOTTOM WITH ANOTHER EMPLOYEE, INJURED EXPERIENCED LOW BACK HURTING. BOTTOM WA VERY HARD. DRILL SHOOK A LOT WHILE TRYING TO DRILL.
UNLOADING TOOL BOX AND FELT A BURNING DOWN LEFT LEG AND LEFT SIDE OF LOWER BACK. WORKED THROUGH 1-19-00 AND PAIN KEPT GETTING WORSE. WENT TO DR ON 1-24-00.
DRILLING HOLE FOR ROOF BOLT AND WRENCH SLIPPED OFF STEEL AND STRUCK OPERATOR ON FORHEAD.
