The repair was on the K pin location of the machine. During the install, the bearing seized roughly a quarter of the way in. To continue, EE used a brass hammer to drive it in. During one of the hammer swings, EE accidentally missed. The left finger got caught between the hammer handle and the bearing causing a fracture.

Showing 50 of 197I was getting my toolbox on the bed of my truck after picking up the toolbox I turned to get down when I tripped on a jack which caused me to fall off my truck and I landed on the concrete surface head-first and I tried to break my fall with my left arm but I still hit my head. I received a cut on my head.
Employee was sorting 5 dragline cables. Employee had just started separating cable in the area where there was snow covered approx 6" - 8" of snow.
Employee was adjusting the two lengths of poly water pipe into the fusing machine when EE's left index and middle fingers got caught between the poly water pipes ends, which pinched the tips of the two fingers.
Smoke was noticed at 11:45 at N-8 hopper and found a small spontaneous combustion fire due to location the fire burnt for more than 30 minutes before it was extinguished.
Ford Truck collided with Powder Truck at N-14 Facilities intersection. Ford was headed east and Powder Truck was pulling out of a stop sign when they collided. Ford driver was transported to hospital treated and release with bruises. Powder truck driver was treated by paramedics and released.
Operator & Oiler were in process of moving power cable to maneuver Machine. Oiler hooked up 2" sling around power cable & using 1" sling hooking to fan tail hook. Before Oiler was ready Machine started to swing while Oiler's hand & fingers were still under sling on power cable side. Resulted in four fingers getting injured from the pressure of the sling when Machine swung.
Operator was dismounting a 785B End Dump, off a ladder when incident occurred. Operator missed the last rung, fell backwards to the ground. Put out left hand to break fall, fell on left hand. Fracturing left distal radius.
While employee was lifting 4 foot cribbing. EE felt a sharp pain in right shoulder. IE reported it to Supervisor and was taken to and seen at the Mine Clinic. IE was referred to another Clinic for further evaluation where Doctor determined it was a rotator cuff tear.
After employee finished replacing troughing idlers on beltline, ee was in the process of removing used idlers off the platform on the catwalk. To clear the walkways and got left hand caught in between handrail and idler frame, pinching the side of left thumb, receiving a laceration which required 7 stitches.
EE was assisting in installing bucket cylinder to linkage. A crane and sling were being used to pull cylinder forward to align. EE noticed a piece of plastic covering bore of cylinder and tried to remove it. This caused the sling to slide up, swinging the cylinder and EE's hand was caught in pinch point.
Trainee was moving cable at 2570. Trainee twisted as they were lifting cable and felt lower back pain. Reported to supervisor.
IE severed the left middle finger tip while installing a push arm assembly on a D-11 dozer.
Walking cam on 2570 dragline was being installed and was being set into place by 992 cat loader. Cam seized and was rammed by the loader and jarred the operator, causing whip lash to the operator.
A non miner suffered a heart attack at the public coal load out. While waiting for Navajo EMS to arrive, CPR was started. Once Navajo EMS arrived, the victim was transported by their ambulance to the mine helipad. Efforts continued to treat the victim with no response. Efforts were terminated by the flight nurse per hospital protocol.
Employee was exposed to unburned diesel fuel vapors.
The IE was lowering a tow pin weighing 25 lb. on the bumper of a water truck. As the IE was trying to lower the pin, it got hung up and wouldn't move. As the IE applied more force, the 25 lb. pin fell 3" to 4" inches and struck the IE's hand, causing a fracture to the left thumb.
As a scraper operator was running in the spoil cut area, was looking back at the apron and bowl and hit a large partially covered boulder. As operator made contact with boulder, the scraper bounced hard and the seat bottomed out causing operator to strain lower back.
The EE was in the process of removing a hydraulic filter from drill 812 when EE injured a middle finger. EE first removed the bracket that secures the filter housing assembly. EE then held the filter housing assembly with left hand and began to remove four bolts located on top. When EE removed the last bolt, the filter housing dropped and crushed the left middle finger.
When troubling shooting a limit switch, on a load out chute and is 25 feet above ground level. The electrician was utilizing mobile scaffolding to access the limit switch. The electrician adjusted the switch and raise the chute. When the switch was activated, the chute began to rise and struck the scaffolding. EE fell, struck the scaffolding and fell on concrete floor.
As a dozer operator was dismounting dozer 327 at the end of EE's shift, EE failed to use the workable power ladder installed and used the iron grab irons to dismount. When EE got to the left push arm, EE jumped to the hard ground where there were small baseball size rocks. Upon landing EE twisted EE's left ankle and twisted EE's left knee falling on EE's right shoulder.
The dragline operator and oiler were in the process of inserting a shaft roller on top of the dragline house. The operator lost hold of the shaft roller and it slipped from hands, left middle finger was pinched between the shaft roller and housing causing a laceration. Over the weekend the wound became infected.
As backhoe 450 was loading Haul Truck 409, large chunky material was dumped into the bed of the truck. The shock load of the rocks being loaded caused the truck driver to suffer back and neck pain. The IE did not report to their next scheduled shift.
A serviceman injured his right shoulder while cutting a padlock off a truck's tool bin with bolt cutters. Afterwards he swung his arm to test and relieve the pain when his shoulder popped. This caused dizziness and numbness to his arm, forcing him to kneel. After regaining strength, he rose to report it. He was seen locally and in Kayenta where a strain was diagnosed.
Employee was working onsite on the drill pad and turned to the right stepped on the edge of a rock and twisted knee.
EE in an attempt to remove broken bolts from a dozer track pad, lacerated the heel of his gloved left hand. EE had positioned his left hand along the outer edge of the pad, then manhandled the freed pad towards him, when it slid into the adjacent pad pinching his hand. The EE was taken to the Kayenta Health Clinic where the laceration was sutured.
Employee strained his back dismounting a dozer.
The 8200 dragline oiler dismounted the machine carrying a bucket of oil when he stepped on an unhinged extension ladder that was laying at the base of the catwalk. He twisted his right ankle. An x-ray revealed a right ankle fracture.
331 dozer pushing coal into N-8 hopper. Dozer went over concrete edge of hopper and slid forward into hopper. Angle of dozer cut fuel to engine. Dozer operator stayed in cab awaiting fuel to track out dozer. Operator exited cab shortly after shift change to allow other operator to track out dozer once fuel arrived.
Employee was resetting calibration limits on the 8750-1 dragline drum for a re-socket. When done, he was walking back to his work truck and twisted his left ankle on the ground transition from compact to soft and rock material beneath him. He refused Mine clinic treatment but went to the hospital later that night, where they diagnosed a sprained ankle.
Injured tripped over a 4" x 4" piece of cribbing in the motor shop while moving dragline house fans with the overhead crane resulting in a fracture to the right shoulder.
During re-socketing of 8750-1 dragline hoist rope, Employee was positioning the wedge into socket when the hoist rope loop being pulled by a dozer snapped and hit his right thumb. Employee reported at end of shift. He visited Mine clinic and had minimal bruising/swelling. The next day, a hospital doctor diagnosed a bruised thumb ligaments and tissues. He was excused for 3 days.
Employee slipped on gravel while loading cable on to a reel truck at the N-9 Cable Yard twisting his ankle and resulting in a left knee strain.
Injured employee, assisting a vendor in installing an A/C fan motor on the roof of the PCR room of a 2570 dragline, stepped on a 2" pad eye and twisted his right foot inward. He suffered a fracture to his 5th metarsal upon contact and was taken to the Kayenta, AZ clinic for treatment. Later,was referred to a Flagstaff, AZ orthopedist for further evaluation and treatment.
The serviceman started the light plant to service it when he noticed a cable lying underneath the plant. He knelt to move the cable when he received a shock from the damaged energized cable. He was able to free his hand but received burns to fingers.
The operator took a step on the push arm-step to mount the dozer when he heard a sound, a (pop) in left foot. Operator has experienced a chronic leg/foot ailments related to health and received treatments and compensations for days away from work.
The injured employee sustained laceration to his left pinky when a metal catwalk fell on his left hand after losing his grasp. He was initially seen at the mine clinic and later taken to Kayenta for further evaluation. The finger was not fractured but sutures were needed to close the cut.
Employee while assisting two other individuals in replacing a troughing idler, suffering injuries to his left wrist/hand. The employee's hand was pinned between the idler and the conveyor belt frame. The crew had managed to nearly upright the idler when a wrench used in process slipped off the idler causing it to fall on the injured limb, possible navicular fx.
Mechanic was removing and replacing bit and sub on drill steel when it shifted. Thumb got caught between steel and nylon sling. Mechanic sustained broken and laceration to his thumb.
Employee was removing crusher liners when he pinch his finger.
Employee was installing a conveyor belt clamp, and was holding the clamp when it got hang up. The clamp was being moved by a forklift, fork got hung up released and jarred him. This release jarred him and hurt his shoulder.
Employee was cleaning trash in scraper parking lot. Lifting a 55 gallon barrel full of trash. Was taken to hospital for further evaluation, and was diagonis with lower back injury. Was given one week off and to get more treatment.
Maintenace was being conducted on a dragline when incident occurred. Hoist rope was being changed out. Winch line was attached to hoist rope to remove it. Winch line got hung up on lug, released and struck worker above left ankle causing a fracture.
Public trash dumpster on mine site. Someone put hot ash in dumpster in a 55 gallon drum. Which cause the trash to ignite. Fire was put out with fire truck and dumpster was flooded with water. Fire was put out at 1300 hours. There were no injuries to any personnel.
Mechanic was observing another mechanic removing final drive of a dozer. Tool: bracket fell and mechanic was trying to get out of the way, trip and fell on his face. Causing a 1/2 inch laceration on his right forehead, which required three stitches. Mechanic was treated and released back to work.
The injured employee's left hand was caught in between the frame and the pivoting idler base in a scissor like fashion pinching the left ring finger between the 2 pieces of steel. The finger tip, about 1/4" was amputated. The IE was wearing gloves at the time of the incident.
Mechanic was in a sitting position when a co-worker walking by threw a piece of rock at him striking his right. Right hand sustained a simple fracture of a metacarpal of little finger.
Injured was trying to turn the swivel of a portable light plant into position and when it came loose, injured's right little finger struck the light plant mast stopper causing a cut to the finger tip.
Preparation laborer was cleaning with a water hose when his leg became wrapped by the hose. He took a step forward he lost his balance and fell backward striking the back of his skull against a concrete floor.
The Prep-laborer reached with his right hand to remove a steel choker while a splice house was lowered onto a lowboy trailer. The cross brace of the splice house and surface of the trailer pinched his right mid-finger. The finger sustained an avulsion requiring sutures to close the wound.
