Employee was in the process of removing a hydraulic hand pump and jack from the top of the tub base and hung EE's foot on a hose and fell forward and EE's thumb on the left hand was caught between pump and ram which caused a fracture of the first joint of the left thumb.

Employee was using a pry bar to unplug a clogged bit on a blast hole drill. Once the clogged bit was free of material, the employee dropped the pry bar onto EE's foot which contacted EE's foot in between the safety toe and the met guard of the boot causing a superficial fracture to EE's big toe.

Employee experienced a medical emergency while at the mine site. The employee then fell to the floor from a leaning position. The employee was transported via county EMS for further care.
IE experienced left hip and back pain after loading drill on hauler. After loading drill, fell from desk of heavy haul trailer. IE had follow up visit on 4-22-26 and was diagnosed with a possible pinched nerve and was prescribed physical therapy and a steroid. IE was never restricted to do EE's job.

Employee began to experience pain in EE's shoulder after dismounting a haul truck. EE was seen by an ER doctor and referred to an ORTHO. An MRI reading on 03-11-2026 revealed significant chronic arthritis and was restricted for no work.

Employee twisted knee while dismounting a mobile crane. Employee was seen at an ORTHO on 01/07/2026 and was where an MRI revealed a torn ACL that will need surgery.
Employee was cleaning dirt, oil, and grease from a retainer cap. After removing all the debris, they ran their hand over the cap's surface. As the employee placed their hand on the cap, a small metal burr protruded from a bolt hole. The bur poked the employee in the hand, and they jerked it back, causing a laceration to their palm requiring three sutures to close the wound.

On 6-8-2025 IE felt knee pain while changing cutting edge on a scraper. On 6-16-2025 IE was seen at orthopedic and given a restriction after MRI reading.
Employee felt an irritated eye. Employee was taken to the optometrist for evaluation where a prescription of an antibiotic for a scratch on the cornea. There was no debris.

While changing out a left front tire on a haul truck, I.E. was trying to spread the lock ring and EE's hand slipped and was pinched in lock ring causing a laceration to EE's right wrist.
Employee stated that EE reached for EE's falling lunchbox and felt a pop in EE's shoulder. Upon an ORTHO appointment on 10/15/2-24, EE was issued a restriction for a shoulder sprain and scheduled an MRI.
Employee felt a pop in LH knee. Upon an ORTHO visit on 08-22-2024, employee was diagnosed with a MPFL tear on their kneecap and was scheduled elective surgery to repair.
