On 8/29/2011 a roof fall occurred at approx. 11:18 am. at approx. 100 ft. outby spad #6254 in #2 entry on the A-West Panel. the fall measured 20 ft. by 25 ft. by 7 ft. high.

Discovered roof fall at approx. 4:05p.m. on the A West Panel, 200' inby survey station 6254 in the #2 neutral entry. The Roof Fall was at or above anchorage zone of the roof bolts installed in that area and the fall did not impair ventilation nor impede passage or travel of personnel. The fall meas. 30'L. X 10'W. X 8'H. Currently there are no plans of cleaning up the fall.

Individual stated he was riding on top right rear fender of the mantrip. When the mantrip went around a curve, he slid and his right foot was caught between mantrip and rib.

The individual was spinning roof bolt and a piece of foreign matter struck him in the right eye. The individual was wearing safety glasses. The individual had to have the foreign matter removed by a physician. This is a medical treatment with no lost or restricted workdays.

The ee was riding with the owl shift crew from the 001 section to the surface. Along the #9 travel way, he failed to adequately lower himself for clearance and was struck in the right forehead by a cross beam installed across the travel way. The contact caused a laceration which required stitches to close.

The individual was standing in the center of the bolter as the ATRS was coming down. His foot was caught in the scissors part of the ATRS causing fracture to the toe area of the left foot.

A roof fall occurred on the #13 belt line in the #4 entry approx. 2000' outby the face. The fall occurred in the intersection at SS6526 and measured 40'x30'x7'. The area was initially bolted with 48" x 7/8" resin grouted rods and 6' cable bolts C2 between rows.

This fall was found at 12:15pm on 10/7/09. The fall was 40'x30'x7' 90' outby SS 6591 in B northeast in the #4 enty. The area had been initially supported by 48" cable bolts and supplemented with 8' cable bolts. The fall will be cleaned up.

The individual was installing a roof bolt when a piece of draw rock fell. The draw rock struck the edge of the tray of the roof bolting machine which caused the rock to break. A piece of the rock struck the individual's left wrist.

The employee was operating a continuous mining machine when a piece of draw rock fell out between roof bolts. The rock struck him on top of his head and bent his neck forward resulting in a strain to his neck.

The injured was operating a roof bolting machine and was raising the drill head up to the drill steel, when he caught his right hand btween the drill head chuch and the drill steel. This resulted in a laceration, which required sutures.

The fall apprears to be approximately 30' long by 8'+ high by 20' wide. There were no injuries or equipment involved. Ventilation was not affected. The fall will not be cleaned up.

The individual was opening an air lock door, which is located on the mine travelway, and felt a pull in his lower back. He did not miss work or go to a physician until 5/19/09. The physician recently determined that the individual has a hernia. The operator was not aware that this was job related until 6/25/09.

The employee was using a set of torches to remove wear strips from a continuous mining machine. He was positioned under the machine and a piece of slag fell in his ear. On January 5, 2009 a physician determined that medical treatment was necessary to treat infection of the ear.

Cable for roof bolter machine was hung against the mine roof with a rope hammer hooked into a roof bolt plate. EE was taking the cable loose when the hook , on the end of the rope hammer, struck EE on the nose, laceration and 1 stitch.

The employee was removing a step-down transformer offt the top of a distribution box. the transformer slid off the corner of the the distribution box & struck the injured's left leg. This resulted in a fracture of the small bone in his lower left leg. NOTE: The DB Box & transformer were stored and were not in service

The injured was preparing to cut a piece of metal using a band saw. He lost his balance and his gloved band contacted the moving band gladp. The injured suffered a laceration to his right index finger which requirely sutures.

The individual was preparing to install the 1st row of roof bolts in 2 left face of unit 3. He was visually examining the mine roof as he set the floor jack on the roof bolting machine. The floor jack contacted his right little toe, causing a fracture and laceration. He was positioned on his knees at the time of the injury.

The employee was replacing a coupling on the feeder. He was tightening a bolt with a socket & ratchet when the ratchet slipped off the bolt head causing his right index finger to strike the bed plate resulting in a fracture to the end of his finger.

The injured was operating a continuous mining machine in the #2 entry of the rooms mined left off (about 133' inby SS #5867) northwest mains. The rib rolled and a piece of the rib consisting of coal and rock (approx 4" thick X 9" wide X 8' long) struck him, resulting in fractured ribs and a fractured pelvis.

The fall was 2900 feet 0utby the MMU 004-0 and measured approximately 5' High x 18' Wide by 85' Long. There were no injuries or equipment damage involved. Ventilation and passage of men were not affected.

The fall is 1200 outby the mmu004-0. There was no equipment involved. There were no injuries. Fall did not affect ventilation or the passage of men.

Pillars were being mined on the MMU 001-0 section. As the MRS supports were being pulled up from the heading, a piece of rock fell (12'X15'X3') outby the MRS. The fall was in the #7 heading about 105' to the right of survey station #4987. There were no injuried involved.

The individual has been workng the past couple of weeks constructing mine ventilation seals. As a result of the repeated exposure to the concrete being used in the Seal construction process, he was diagnosed with cellulitis.
