INJURED WAS MOVING HIS CABLE ANCHOR FOR SHUTTLE CAR UP. THE CABLE REEL TURNED PULLING ANCHOR INTO THE BATWING WHICH CAUSED THE CHAIN ON THE ANCHOR TO STRIKE INJURED ACROSS FACE.

AN UNPLANNED FALL OF ROOF HAS OCCURRED 1 BRK INBY SPAD NO.7027 IN RIGHT RETURN OF NO. 6 WEST MAINS. THE FALL MEASURES 100 FEET LONG, 20 FEET WIDE, AND 10 FEET THICK. ALL ENTRANCES TO THE FALL HAVE BEEN CRIBBED AND DANGERED OFF. THE FALL DOES NOT AFFECT VENTILATION OR IMPEDE TRAVEL IN THIS AREA.

EE was trying to unloosen a packing nut from the starter box. EE was using an 18" pipe wrench and 3' bar to wedge box. While pushing down EE felt pain in his stomach. "THIS DID NOT BECOME REPORTABLE UNTIL 02/17/2014 WHEN EE BEGAN TO MISS WORK DUE TO THIS ACCIDENT"

An unplanned roof fall has occurred in the No. 5 entry intake escapeway the fall measures 22'long, 24' wide and 48" high the fall does not impede travel or affect ventilation. all entries around the fall has been cribbed and dangered off.

EE was putting straps together on conveyor chain. EE put a chisel between press and the conveyer chain. The chisel came out hitting EE in the left wrist.

EE was bolting top and the pin steel was caught/fouled in the drill pot. EE was removing the pin steel, the pin steel came lose and struck EE underneath the right eye causing a laceration.

A piece of rock fell out between ATRS and hit the operators canopy. A piece of the rock broke off and hit EE on the right wrist and arm.

Employee was changing a tire on a forklift at Rockhouse Creek #2 Mine. He stated he was pulling on the tire when he slipped (lost footing) and he stated he twisted his lower back. Safety dept administered first aid treatment until ambulance arrived

Several unplanned roof falls has occurred in the Nos. 2,5,6,7,and 8 intersections one crosscut inby survey spad No. 375 the mine is in BA status and no one working underground only weekly examinations being conducted. All access to the falls will be dangered off and cribs built to prevent entry of persons.

EE was getting out of scoop to unload a set of airlock doors. EE tripped while getting out of the scoop. EE twisted his back while falling out of scoop. EE proceeded to get up and go lift the doors. When he picked up the doors he felt a pain in his back.

Crew was traveling to section, pulled over to get out of the way of another ride. Crew pulled under cable the EE put his hand up to keep the cable away from his face, a rope hanger came loose and snagged EE pointer finger causing a laceration.

There has been a fall found in the primary escapeway. All approaches to the fall have been cribbed and dangered off to keep workers from entering the area of the fall. The primary escapeway has been re-routed. We have had safety meetings with all three shifts discussing and going over on the map the re-routing of this entry.

EE was duck walking looking for bad places in a piece of cable. The ee slipped on a wet piece of draw rock falling aprox 26" hitting his elbow.

EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.
