Showing 50 of 97Employee was shoveling material on 04/16/14 when he felt pain in his abdomen and noticed swelling later due to a hernia. His initial nature of medical treatment was Treated & Released. He did not begin missing work until 06/10/14 due to surgery for the hernia.
Employee was sawing timber when the saw jumped, hitting his left index finger.
Employee was removing left rear hub assembly on a rock truck. While he was removing the bolts he placed his hand on the back of the impact wrench and once it engaged, his left hand on injured.
Mechanic was removing "belly pan" on 320 tractor type dozer. Upon inspection pan was "cockeyed" requiring the use if a hammer and wedge. During the removal process the wedge became dislodged and struck EE on left side of face under left eye. EE sustained a deep laceration and glue was applied.

A roof fall was found by EE on his weekly air way examination. The fall was found 1 break inby spad #1129 (3000 feet outby). The fall is approx. 55 feet long - 18 feet wide and 8 feet high. This mine is idle and has been since Nov-12.
Employee was retrieving nails from roof bolter when a piece of rib material, 6'L x 3'W x 12" thick, fell, striking him from behind, injuring his left knee.

Employee was drilling on middle stockton pit. He went to dismount the drill on front and caught his left little finger on the mounting bolt to handrail.
Employee was taking down MRS cable to take outby when a piece of rock approx. 4" thick, 4" long, and 3" wide fell from top striking him in the back of his head, neck, shoulders and back, driving him to the ground. * Did not report prior because employee did not begin missing work until 01/06/2011.
Employee was standing on a rock truck ladder and reached out to clean off the headlights when his feet slipped out from the ladder causing him to fall to the ground, dislocating his right shoulder.

He was alone at the time of his death. He was not involved in the mining process. He only provided weekend security coverage at the site.

After replacing a 4" hose on a clean up pump two ee's were putting the grating over the sump. As the grating went into place the hose came off of the pump causing the water and mud to hit employee in face filling his eyes with dirt.
