EE was going to free door latch on RR car, said he told another EE, who was on other side of car, that he was going to work on latch. Another EE had come into the Bldg and was talking to the other EE at same time, when other EE finished talking, having not heard injured EE, he shut car door with slate bar, pinching injured EE's finger in latch.

EMPLOYEE APPROACHED LOCOMOTIVE TO BOARD IT AS IT WAS MOVING, WHICH IS NORMAL. HE GRABBED THE HAND HOLDS AND PLACED LEFT FOOT ON STEP TREAD. HIS LEFT FOOT SLIPPED ON ICE ON THE TREAD, SLID TO T HE LEFT, LOST BALANCE AND FELL TO HIS RIGHT KNEE ON THE TREAD, CUTTING HIS RIGHT KNEE. HE HAD TO RELEASE HIS GRIP AND FALL AWAY FROM THE TRAIN.

EE WAS LYING ON TOP END PLATE ON LOOSE COAL STENCILING MARKINGS ONTO RAILCAR. HE LOST HIS FOOTING ON THE LOOSE COAL AND SLID DOWN A METAL SLOPE OF APPROX. 45 DEGREES FOR A DISTANCE OF ABOU T 6 TO 8 FEET. HE SUSTAINED A SPRAIN /STRAIN TO RIGHT KNEE AND SPINE ACCORDING TO MEDICAL EXAM. HE HAD BEEN INSTRUCTED TO ALWAYS USE A LADDER WHEN STENCILING.

EE was welding the side of a rairoad car on ladder. he came down ladder to start another task. he removed his welding hood, hard hat and safety glasses to wipe sweat from forehead. while wiping sweat, a hard shelled bug flew into eye. he worked until 1130 am and then went home. approx. 6 pm. he went to er for eye pain dr's instructions were to take off work on 4-24.
