On 8-20-07 Employee Reported to mine supervisor, that on 8-8-07, he was stretching to pick-up a 4x6x1/2 sheet of plate steel with a co-worker, in doing so, he strained his back. on 8-20-07 employee was seen by DR. at the Louisa Medical Clinic, in Louisa Kentucky,and was givin a prescription for pain. Employee did not fail to report to work.

Employee opened inspection door on mixer unit to examine a plugged condition. The inspection door was not secured in place and fell, striking the employee's right hand, causing swelling and pain. Employee was taken for X-rays, no fractures were identified, however; the employee was ordered off work for his next shift and given a Prescription for pain.

Employee was moving the portable stairway so he could grease the top jacks on a 988 F model loader. While rolling the stairway into position, the employee strained his lower back, resulting in a lost time accident.

Employee attempted to re-attach the bucket onto the Skid Steer. At this time employee placed himself between the bucket and the Skid Steer. The bucket was raised approximately 12 inches off the ground. The bucket fell off the boom striking the employee in the back of both heels. This resulted in bruises to his heels and the Dr. prescribing Vicodin for pain.

EE WAS MOWING THE GRASS IN AND AROUND THE FACILITY. WHILE USING A STRING TRIMMER A PIECE OF FLOATING DEBRIS PASSED BEHIND HIS EYE PROTECTION AND ENTERED THE LEFT EYE CAUSING PAIN & IRRITATION.

EE BECAME FRIGHTENED BY LOUD NOISE NEAR HIM HE THEN JUMPED FROM STEP LADDER PLACED NEAR COAL HOPPER & BELT LINE. HE WAS APPROX 21/2 FT FROM GROUND HIS RIGHT FOOT COMES IN CONTACT WITH A LUM P OF CAOL & SPRAINS ANKLE. THIS INCIDENT WAS NOT REPORTED TO MGMT UNTIL 10/29/01
