On November 15, 2007 the employee completed an injury report for October 29, 2007 stating his knee hurt him at the end of his shift. He has been off work since 11-02-07. He had MRI read on Nov. 19 which indicated a medial meniscus tear and is scheduled for physical therapy for six weeks.
The employee slipped on a rock and twisted his right knee on 9/26/07. He continued working until 12/19/07 when he visited a surgeon that recommended time away from work.
When taking out drill steels from the roof, two 3" steels fell out striking the employee's hand against the bolter.
The employee injured himself on 8-21-07 moving a rock out of the way by hand. Continued working until 12-13-07 when he had surgery to repair a hernia.

ON 12/14/04 EE FELT SOMETHING IN HIS NECK WHILE TAKING A SHOWER. THE NEXT MORNING EE VISITED A PHYSICIAN AND WAS ADMITTED TO HOSPITAL (15TH, 16TH, 17TH) FOR POSSIBLE HEART PROBLEMS. ON 12/20 EE CALLED THE MINE SITE STATING HE HURT HIMSELF SHOVELING AND HAD A HERNIATED DISC IN HIS NECK AND WOULD BE OFF WORK. EE STARTED PHYSICAL THERAPY ON 12/20.
MAKING A SPLICE ON A BOLTER CABLE THE EMPLOYEE WAS HOLDING THE WIRES IN A CRIMPER TO BE CRIMPED. A FELLOW WORKER MISSED WITH A HAMMER AND HIT THE EMPLOYEES RIGHT INDEX FINGER. THE INJURY REQUIRED 7 STITCHES.
