Employee was walking down a sloped embankment near the fuel tank and slipped due to muddy conditions and fell onto right shoulder. Employee did not miss any scheduled work or receive medical treatment until 1/31/2025, when employee had the shoulder operated on for rotator cuff damage.

The employee was getting out of triaxle truck at the bed wash. The employee's foot slipped when employee stepped onto the ground. The employee fell backwards onto employee's shoulder. The employee did not seek medical attention until 12/27/23. Employee received an x-ray on 12/27/23 & it showed employee fractured left clavicle.

Driver was dumping the load of ash and ash stuck in the bed and caused the tractor/trailer to roll over onto the passenger side. Driver was having neck pain and was transported to the hospital as a precaution.

Employee was using an air chisel to remove hardened co-gen ash from around the pad bolts on a D-8T dozer. Employee was installing new track pads. A small piece of the air chisel bit broke off the bit and struck the employee in the forehead just below the brim of hardhat. The small piece of metal was removed at a hospital emergency room.

Employee notified employer that knee was sore on 11/1/2019. No medical attention was needed or received at that time. Employee sought medical attention on 11/5/2019 because knee was swollen and sore. Employee diagnosed with bursitis on 11/5/2019 and an infection in the knee on 11/6/2019.

The employee was helping to empty a water treatment sludge bag after it had been dewatered. EE was pulling open the bag with a pair of vice-grip pliers and the pliers slipped causing EE to fall backward onto the frozen ground. EE landed on the right side causing a contusion on EE's right elbow.

Employee was removing the bucket pins from a Volvo 250G loader. EE was swinging a sledge hammer and when the pin came free, EE twisted EE's body to the left and heard a popping sound and felt pain in EE's left knee.

Employee had just got a tri-axle truck loaded and had turned the truck around in the loading area. Started up the exit ramp from the loading area and drove through a 3 foot berm. This caused the truck to rollover onto the driver's side. Employee complained of pain in back/ribs on their left side.

Employee was cutting open a transmission filter to check for metal cuttings. EE's hand made contact with the sharp edge of the filter and caused a laceration to left thumb.

The employee was assessing the damage to the fan assembly of a Volvo L220E loader. The fan assembly suddenly fell and the employee put hand out to prevent the fan assembly from striking the employee. A sharp edge on the fan assembly caused a laceration to the back of right hand that required sutures.

Employee was installing a new tail roller on the fines collection belt of the McCloskey 733 trommel. The roller slipped while installing the bearing on the shaft. Employee's left thumb was pinched between the shaft of the roller and the belt alignment adjustment bolt.

Employee was flushing out the power washer water tank with a 2 inch water hose. EE was using bleach to clean the tank. When EE pulled the hose out of the tank, the pressure of the hose pushed EE back and caused EE to become unbalanced. EE fell approx. 3 feet from the top of the toolbox to the ground. EE fractured 2 ribs and pulled a groin muscle.

Employee was tightening a nozzle on the water cannon at the truck bed wash. EE was using a pipe wrench to tighten the nozzle. The pipe wrench slipped causing EE's finger to be caught between the handle of the wrench and the safety railing. This caused a laceration to EE's left little finger and required stitches.

Employee was lifting the pin on the rear of a Komatsu WD-600-3 wheel dozer to attach a tow cable. When lifting up the pin, employee felt discomfort in right side lower abdomen. Employee was diagnosed with an inguinal hernia on 1/12/2017. Employee is having surgery today, 1/20/2017 to repair the hernia. Employee did not have any lost time until today, 1/20/2017.

Mechanic was standing on the rear bumper area of a Euclid R40C water truck and was connecting air lines to the spray valves. He reached into his pocket to get a wrench and inadvertently stepped back and his foot slipped off the bumper area. He fell to the ground and landed on his right side, causing fractures to his ribs.

Mechanic was using a sledge hammer to remove a pin from the blade of a Komatsu WD-600-3 wheel dozer. He was holding a driving bar with one hand and hitting it with a sledge hammer with the other hand when he felt a strain in his lower back.

Employee lost footing while climbing up onto excavator to start the machine at the beginning of the shift. Site was wet and muddy. Employee twisted back when he slipped. This caused pain in lower back. Employee continued to work and did not have medical attention until 12/14/12. Doctor reported strained muscle. Employee did not miss any scheduled work.

Employee was spraying detergent on the boom of the PC400 excavator with a 1 1/2 gallon garden sprayer. Some of the detergent ran down his arm and got between his rubber gloves and Tyvek suit, causing redness and irritation to his right wrist and left forearm.

Employee was operating a wheel dozer and plowing snow from the roadways. Approx. 12 inches fell overnight. He ran over a large pot hole or low spot along the roadway, which caused the dozer to bounce suddenly. He felt pain in his lower back and later down his right leg.

Employee was helping load a 11R22.5 truck tire into the foreman' s pickup. Both the employee and the foreman were lifting the tire into the truck bed when the foreman lost his grip and the employee attempted to hold on to the tire by himself. He twisted his left knee while holding the tire by himself.

The injured employee was assisting on a repair of a hydraulically operated machine component, when the hydraulic line came apart. The employee was going for clean-up material when he heard a "pop" and felt a sharp pain in the rear of left leg/knee. After seeking medical treatment, he was diagnosed with Tendonitis. He has been released for duty and has returned to work.

Employee was excavating wet ash, mud and clay material when his bucket kicked full and sent the material through the front glass. He was struck in the right side of his face and head with the material. Emergency call to 911 was made. EMTs responded and employee was taken to Altoona Hospital. He returned to work at 3:00 with minor scratches & abrasions.

TWO EE WERE ATTACHING A SET OF DOLLY WHEELS TO THE PULL AREA A LOADER. BY USING A BACKHOE TO LIFT DOLLY WHEELS INTO PLACE. AFTER PULL PIN WAS SET, OPERATOR MOVED BOOM OF BACKHOE DOWN IN ORDER TO SLACK THE CHAIN TO UNHOOK. WHILE UNHOOKING, OPERATOR BUMPED BOOM LEVER, DROPPING THE BUCKET ON THE MANS RIGHT HAND.
