He was operating shuttle car, and hit his head on the mine roof.
Roof fall anticipated due to visual observation of area, occurred in the area of the last open crosscuts and intersecting entries no. 1 through 5. The fall occurred as a result of the coal rider seam dipping toward the seam being mined in a low cover area.
Pre-shift examiner discovered a roof fall which had fallen sometime over the week-end. The roof fell in the face area of no.2 entry, across the intersection to the No.3 entry and was 5' thick.
EE WAS HAULING COAL WITH A SHUTTLE CAR AND A PIECE OF DRAW ROCK FELL OUT IN BETWEEN THE ROOF BOLTS, HITTING HIM IN THE LEFT SHOULDER AND SIDE.
EE WAS GETTING ON MANTRIP AND SLIPPED AND BUMPED HIS KNEE ON THE MANTRIP.
WHILE LIFTING SURVEY EQUIPMENT, EMPLOYEE FELT PAIN IN BACK. EMPLOYEE HAS BEEN WORKING LIGHT DUTY OUT OF ENGINEERING OFFICE SINCE DATE OF ACCIDENT. HE WILL BE OFF AS OF 2/3/03.
EE SLIPPED, CAUGHT HIMSELF WITH LEFT HAND CAUSING PAIN IN LEFT WRIST.
