The mechanic stated: was attempting to step up on a handrail to remove a chain on the drill. When EE raised EE's leg EE felt this knee pop. This resulted in a partially torn tendon.

A mechanic was attempting to install an equalizer bar on a dozer. While positioning the bar, the belly pan fell catching EE's left hand between the belly pan and the bar. This resulted on the loss of the left ring finger.

The contracted employee stated that "something" hit EE from behind after cutting a tree causing EE to roll down the hill EE was working on. This resulted in broken ribs, a fracture above the teeth, and cuts/bruises.

An employee broke both heels when EE jumped to the ground from the deck of a 785B rock hauler. EE jumped because a steering jack hose ruptured onto the turbo causing flames to come up to the outside of the cab area. The EE also received burns to the back of the neck and right arm.

EE climbed up a 3 step ladder to hand another technician a tool. EE was standing on the 2nd step of the ladder and holding onto the handholds on a 7858B Truck. The ladder twisted out from under EE and they slipped off the ladder and twisted an ankle when they hit the ground.

The employee was grinding on a push arm pin in order to weld the pin. The pin rolled on to their left pointer finger, while the grinding was being done, resulting in a broken bone.

Employee went to get on the push arm of a D10R dozer and his foot slipped causing him to fall and hit the push arm with his left rib cage causing cracks to two ribs. He returned to full duty the next shift. His regular job duty is an excavator.

As employee stepped off of the Lube truck his foot slipped and he fell into the ladder on the used oil tank causing a laceration to his left ear that took sutures to close the cut. He was in the process of emptying the used oil tank on the truck into the ground tank. He reported to full work on his next scheduled shift.

Employee was unloading supplies when cutting blades for a grader started shifting. He stepped to get in the clear and tripped over a hydraulic unit, falling to the ground catching his fall with his left hand causing a hair line fracture. He is continuing working his regular job.

Employee passed out as he exited truck.Worker saw him covered with mud.No injury occurred.No exposure to toxic hazards.Went to hospital for heart tests found nothing.Was admitted due to infection & given antibiotics.Was taking antibiotics prior to incident.Reporting this as occupational illness MSHA says we must.We do not agree this is reportable would like it removed from record.

Employee was helping load crib blocks onto a mechanic truck when he felt a pain in his stomach. He was diagnosed as having a hernia. He required surgery and has not returned to work as of today.

As the loader operator dumped into a 777 Cat. truck, a rock from the bucket hit in the truck bed and the driver backed into the loader bucket causing a strain to his back. He has been on restricted duty for four days and is working now at full duty.

Changing a wrench hydraulic unit out on a drill. The new unit was bolted up and when he stepped to the ground his stomach started hurting. He was diagnosed with a hernia that required surgery.

A fire was discovered in a day box on a powder truck as a shot bench was being loaded. A brush/tree fire was located near the loading site which may have been the ignition source. No injuries occurred in this incident.

Employee was tramming a D11N dozer out of a shot and when he got to the haulroad he noticed that the hydraulic gauge was showing hot. He stopped the dozer and trammed it backward to a wide spot (50 to 60 feet) to keep from blocking the haulroad. When he stopped, fire shot up beside both doors. The right side fire lesson and he exited the dozer. No injuries occurred.

Was decending a haul road with a loaded truck, he went to change gears and could not get the transmission in another gear. He turned the truck over onto the passenger side. On an approximately 8 percent grade. This accident is a contractor for Huffman Fork (LB Trucking #K193) but we are required to report the accident according to our MSHA district.

Employee was walking from a dozer to a hauler and tripped over a small berm created by the dozer pushing, he fell to the ground. He caught his fall with his right arm and stoved it up. He continued to work and started missing work on 9-7-2010 due to complications.

Employee entered the cab of the drill and mashed his thumb when he shut the door on it. He had to have sutures to close the wound on his thumb. Tip of bone was cracked. He missed no scheduled work.

EE was inserting a wire into a Bunch Block using a stainless steel razor knife, when it slipped and cut his right index finger. The cut required 5 stitches.

The employee was standing on top of a loader with a rope in his hand that was tied to a drive line underneath the loader. As he was pulling on the rope he felt pain in his back. He continued to work and on 11-30-09 he was taken off work to start 2-3 weeks of therapy and is still missing work.

The engine died on EE's truck, EE stepped out, he got back in and tried to start the engine and said the truck rolled off and went over the hill and the cab turned over. **EE was terminated 9/4/09 while still on restricted duty per Inspector**

turned 992 loader over into a pit. He was a contractor for Bates Contracting & Construction Inc. They reported the accident to MSHA. I got a violation issued on 7-9-09 to file an accident report on incident also.
