EE was using chain hoist to lower cylinder from excavator, when the cylinder moved quicker than anticipated, pinning their right hand between the cylinder and the chain. Their fingers were cut when they pulled their hand away.

Employee was repairing a screen in a tight space. As a result of the tight space, employee's hard hat was knocked off and EE hit EE's head while exiting the screen. Employee's head was lacerated. EE received staples at WellNow Urgent Care.

Non employee and another non employee were tresspassing on quarry property. After repeated failed attempts, their vehicle mounted the berm, descended the interior, and fell to quarry floor. Driver died at the scene, passenger died after transport to local hospital.
