Drill Steel rack tipped over while in the process of loading steel back onto the drill. The rack made contact with employees left leg creating a small laceration mid way on EE's calf. The employee was taken to the hospital. The employee received 4 stitches. The employee went back to the shop and finished the day there. No restrictions.

A 17'4" x 3' x 1/2" thick liner plate was being repositioned in the Pioneer feeder. The 5/8" grade 8 bolt holding the upper right corner in place sheered allowing the plate to fall to the pan. The plate struck the employee on the left foot.
Pulling lever on flop chute that is located on an impact crusher. Lever was jammed/stuck. The lever finally came loose & smashed his left hand on the stopper.
Cleaning out under the jaw so jaw could be moved off the blocks. A rock fell from bottom side of jaw and hit his left hand.

Putting on piece of metal with bolts, employee pushed bolt down while another employee was trying to spin nut onto bolt. Employee's finger slipped off the bolt causing his wrist to slide against the bolt, cutting it. He was wearing leather work gloves at the time of accident.
WHILE USING THE GRINDER SOME METAL SHAVINGS GOT INT0 HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES AT THE TIME OF ACCIDENT
HYDRAULIC CYLINGER CAME LOOSE AND IT HAD PRESSURE IN IT CAUSING OIL TO SQUIRT ON FACE. OIL SQUIRTED ON RIGHT SIDE OF FACE FROM CHIN TO HAIRLINE.
