Walking by miner in 1 East #3 heading inby 7 block and a piece of loose rib (2' by 8-12") fell and struck him in the head and neck. X-rays were negative, given time off for possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.
Individual was rib pinning off the Alpine Miner when a rib rolled striking him on the right shoulder and leg.(sprain to right shoulder) This did not involve any compliance issues, no equipment involved. This did not involve proficiency or attitude. All personnel safety items were being utilized. Individual continued to work until 3/02/2015, removed from work by doctor.
Individual was bolting on the left side of the Alpine Voist Miner when struck by a rock from the top of rib line struck the left shoulder. Individual experienced pain to the shoulder.
The injured was installing a eye bolt for the messenger line in the No.3 entry 30'inby brk. No.66 when a piece of rib rolled off measuring 30"x24"x10" striking him on the left foot, causing a fracture to his left big toe. The accident happened on 21 Rt. development panel.
Individual was operating the shearer on the 18 w longwall when a rock blew out at the face area striking the individual on the right ankle.
Individual was a rib bolter on the 18w section fall #1 entry. The corner of coal pic rolled out striking the right side of face, pinned both legs thighs and right ankle. Individual suffered an abrasion to the right temple area, and bruises to the thigh area.
Individual was retrieving a roof strap towards the back of the miner when a piece of corner rib fell striking the outside of the left foot. Individual received a fracture to the fifth metatarsal.
EE was cleaning the walkway along the 15-W Longwall stage loader, when a large rock rolled from the top of rib striking EE and pining him beneath the rock. EE sustained fractures to both sides of the pelvis and a cracked scapula on the left side.
The employee was struck by a rib roll. As he fell, he struck the rib with his right arm resulting in a laceration requiring 9 sutures.
EE was operating left side of the roof bolter on the 12 Right Section in the # 1 entry approx. 50 ft. inby survey station 25484 & as he walked towards the end of the roof bolter a rib measuring about 76" high x 84" wide by 26" thick rolled off behind the line curtain striking the employee against the bolter resulting in fractures to left shouder, ribs, sternum & back.
This report is being filed to abate Citation 7101717. An ee suffered a sprained left ankle when he was struck by a rib roll. He went to 2 medical facilities with 2 conflicting reports. The operator chose to send the ee to an Orthopedic Specialist who could not see the ee for several days. The diagnosis was consistent with the 1st diagnosis of a left ankle sprain.
Employee was standing along side the miner when his left knee was struck by a piece of coal from the rib.
The injured employee was struck on the Lt. ear by a roof bolt strap laying on top of the miner. A rib rolled out contacting the strap, the end of the strap then pivoted striking Lt. ear. Employee received 10 stitches.
Emplyee started drlling rib holes and outburst occurred.
Employee was operating shearer on 8 right Longwall when a piece of rock measuring 5 ft. x 2 ft. x 9 inches fell from the face into the face chain then thrown out into the walkway striking employee's right ankle resulting in a fracture.
ROCK FELL OUT OF RIB AT FACE AREA WHILE EMPLOYEE WAS SHOVELING OUT LIFTER HOLES. ROCK STRUCK HIM IN THE HIP/LEG.
Located on the 10-A gate section, employee was walking from the mine (outby direction). While attempting to walk around the loading machine, the "berm" of the ribline collapsed, causing his knee to slide out from under him.
BECAME REPORTABLE ON 1/24/06 (1ST DAY OFF). ON 12/15/05 EE WAS REMOVING A METAL PANEL FROM A STOPPING WHEN A PIECE OF COAL RIB FELL, STRIKING HIM ON HIS RIGHT ANKLE AND TWISTING HIS KNEE (MUSCLE TEAR - RIGHT KNEE). EE CONTINUED WORKING UNTIL 1/24/06.
EMPLOYEE WAS ROOF BOLTING ON THE LEFT SIDE OF THE WEST ALPINE CONTINUOUS MINER/BOLTER, WHEN A PIECE OF ROOF ROCK/COAL FELL ALONG THE RIB LINE KNOCKING THE VENTILATION FIBERGLASS TUBING DOWN. THIS PUSHED THE EE AGAINST THE SIDE OF THE MACHINE RESULTING IN A LACERATION TO HIS CHEEK.
EE WAS STRUCK BY A PIECE OF LOOSE COAL RIB AS HE FINISHED MINING AND WAS PLACING THE REMOTE CONTROL UNIT ON TOP OF THE MACHINE AT THE END OF HIS SHIFT. RESULTED IN A LEFT HAND INJURY (LACERATION/FRACTURE).
EE was in the process of prying down a loose coal rib with a prybar when additional rib also fell just out by the place he was prying, resulting in the material striking him on his left leg.
ON 2-N LONGWALL FACE EE WAS ASISTING HIS CREW IN RECOVERING SHIELD SUPPORTS DURING A LONGWALL SYSTEM DISASSEMBLY AND MOVE WHEN A PIECE OF FACE ROCK AND COAL 2 1/2 X 3' X 1' FELL FROM FACE. STR IKING HIS LOWER RT LEG.
EE WAS HELPING PUT WIRE SCREEN ON THE MINER WHEN A PIECE OF STONE POPPED OUT OF THE RIB AND STRUCK HIM ON THE FACE.
EMPLOYEE WAS HELPING REMOVE LEG JACK FROM HEADGATE AREA AND WAS STRUCK ON TOP OF LEFT FOOT BY A PIECE OF COAL FROM THE RIB.
HELPING EE TAKE DOWN TUBING. COAL RIB ROLLED OFF AND STRUCK HIM ON KNEE.
ATTEMPTING TO SCALE DOWN A LOOSE RIB AT THE CONTINUOUS MINER. MORE MATERIAL FELL THAN EXPECTED AND STRUCK HIS RIGHT FOOT.
EMPLOYEE WAS A PASSENGER IN AN OPEN TYPE TRACK VEHICLE (PERSONNEL CARRIER). A PIECE OF LOOSE COAL RIB APPROX 1 1/2' X 1 1/2' X 6"THICK FELL FROM THE TROLLEY WIRE SIDE RIB AS THE VEHICLE PASSED BY STRIKING THE PASSENGER ON THE SIDE OF HIS RT KNEE, CAUSING HIS KNEE TO BEND INWARD.
WHILE SCALING DOWN A LOOSE RIB A RPIECE OF STONE FELL FROM THE RIB BESIDE HIM, STRIKING HIM ONT HE FOOT. THE STONE STRUCK BEHIND THE STEEL TOE OF HIS BOOT.
EMPLOYEE WAS BARRING DOWN LOOSE RIB ROCK- APPROX. 1'X 2' X 2" THICK. WHEN THE PIECE HIT THE MINE FLOOR IT ROLLED TOWARDS HIM STRIKING HIS RT. HAND JUST BEHIND HIS THUMB.
1-A SECTION-EE WAS ADJUSTING VENTILATION TUBES AT THE MINER. COAL POPPED OFF OF THE RIB CAUSING HIM TO STUMBLE AND FALL INTO THE SIDE OF THE MINER.
EE WAS CHANGING BITS ON THE LONGWALL SHEARER DRUM AT THE FACE NEAR #9 SHIELD WHEN A PIECE OF FACE ROCK 28"LONG X 12"WIDE X 2"THICK FELL STRIKING HIM ON HARD HAT, NECK, BACK & SHOULDER, THEN DO WN LEFT LEG, RESULTING IN A SPINEOUS PROCESS FRACTURE OF C-7 VERTEBRAE-TIP.
EE WAS WALKING ALONG A COAL RIB IN THE 3-R LONGWALL SECTION, IN A CROSS-CUT ADJACENT TO THE STAGE LOADER, BETWEEN #3 AND #4 ENTRY, WHEN A PIECE OF COAL RIB APPROX 3'LONG X 1'WIDE FELL STRIKING HIM ON BACK OF RIGHT SHOULDER - CAUSED HIM TO FALL TWISTING HIS LOWER RT LEG.
EMPLOYEE GOT OUT OF CONTINUOES MINER TO SET CURTAIN JACK. RIB ROLLED HITTING EMPLOYEE FROM BEHIND ABOUT KNEE HIGH FORCING KNEES INTO MINER FRAME. RESULTING INJURY WAS A STRAINED RIGHT KNEE REQUIRING SURGERY.
