Employee was cutting out a steel donut to weld onto the dump ropes that attach to the dragline bucket. While cutting he noticed the oxygen hose on the torch had a pin hole. He attempted to taped the hose instead of replacing or fixing the hose. Employee continued to use cutting torch and his gloves became saturated with oxygen gas and a spark set his gloves on fire.

Driving his rock truck, employee saw a 930 rock truck in the same pit was having problems out of the retarter system in which he saw smoke and sparks. He then went to help with his fire extinguisher. He was exposed to smoke and fire extinguisher dust about 10 - 15 minutes before smoke was gone.
