988 Loader caught fire, was unable to be extinguished by site personnel, 911 was called. Two fire crews arrived on site, one fireman approached the loader by the rear tire. The plant manager was yelling at them to get away from the tire. It exploded killing the fireman by the tire.

The IE was hauling overburden in a Cat 793 end dump to a fill area. The front area on the end dump caught fire. The IE parked the truck on the fill and exited the end dump. The IE sustained multiple burn injuries as a result of the exit. IE passed away on 09/12/18. Ultimate cause of death is pending the coroner's report.

EMPLOYEE WAS OPERATING A CRAWLER TRACTOR ON THE #1039 STOCKPILE IN THE MINE PIT. A FLASH FIRE OCCURRED. THE OPERATOR EXPERIENCED EXTENSIVE BURNS AS HE EXITED THE OPERATOR'S CAB. INVESTIGATION CONTINUES. HE WAS HOSPITALIZED FOR 2 1/2 WEEKS IN THE BURN UNIT. ON NOVEMBER 2, 2000, MSHA WAS NOTIFIED THAT HE EXPIRED AT HOME.
