Employee was working to repair an emergency lowering and jammed EE's finger while removing the guard. EE reported but declined medical attention. Employee reported swelling and discomfort 8 days later and was taken for medical evaluation. Diagnosed with a sprain/contusion and a splint was put on the finger. **Did not become reportable until 7/9/2024**

Employee shoveling and their shovel struck the cardboard/rocks surrounding, causing the shovel to stop abruptly. Employee felt a sharp pain in right shoulder and informed supervisor. Medical was offered and turned down. Employee returned next day and requested medical evaluation. **Became reportable on 5/15/24 - Diagnosed with shoulder strain is currently on restricted duty

Employee was working on 1928 screw replacement. EE was pulling on the screw & felt pain in left shoulder. The employee was offered and declined medical attention. On 4/16/24 the employee stated that the shoulder was still bothering EE, and wanted to get it checked. After evaluation, employee was put on restricted duty as of 4/17/24 ***DID NOT BECOME REPORTABLE UNTIL 4/17/24***

EE was shoveling salt onto the belt while cleaning 22 skip tail. While raising the shovel to dump the salt onto the belt, the EE felt a sharp pain in their right shoulder/arm. EE noticed a large bruise the next morning & noted that the area was sore & was experiencing pain & difficulty when moving their right arm. *Didn't become reportable until 3/5/24. EE will require surgery.

EE was lifting a steel bar attempting to load it into a roll of belt. During this process, the EE strained their left shoulder.

EE suffered 1st degree burn on right arm while checking coolant on piece of equipment.

One of the hoist ABB drives was overheating needed to be replaced. While installing the new drive, the drive rocked back, pulling hard on the EE's left shoulder and striking EE's shin. The employee sustained an abrasion/bruise to the left shin, a cut on the right middle finger, and a sore left shoulder.

2 employees were lifting a 13' long section of 6" wide rectangular steel tubing in order to set it onto a dolly. Employee one lost their grip and dropped their end. The other end then pinched employees left middle finger between the edge of the steel and concrete floor causing a nondisplaced fracture of distal phalanx of left middle finger.

Employee was setting the parking brake on a rail car. As EE tightened the brake EE felt a pop in EE's left arm. Employee was diagnosed with a tricep strain.

On 8/8/22 an employee was attempting to open the landing gate at #4. They were trying to pull up on the latch with no success. They felt something pull/ strain in their back/shoulder area. The employee was sought medical treatment on 8/24/2022 and was prescribed physical therapy.

On 7/24 employee was helping with a belt move. Employee was pulling belt when they felt a pop in their left forearm near the elbow. On 8/6 the employee was pulling brattice curtain when they felt more pain in their forearm/elbow. On 9/6 during a visit to the clinic the employee was advised to begin physical therapy.

Employee was removing a guard when it swung down and cut their arm which required 6 stitches.

While putting equipment on the lowering slide, a drill cylinder fell off. The employee went to lift it back in place. When lifting it in place they hurt their bicep and shoulder.

Employee was installing a gate with another employee. The employee lifted one end of the gate and was holding it up while another employee was installing the hinge pins. The gate fell out of the hinges causing the gate to fall, this caused the employee holding it to twist their lower back.

An employee was in the process of shoveling some spilled salt beneath a conveyor belt transfer. While pushing a shovel through the salt, they felt a sharp pain in their lower back. The pain made it difficult to maneuver and the employee decided they needed to seek medical attention.

An employee was in a high lift pulling wire vertically from a spool located at ground level. After pulling the wire, the employee felt that their left shoulder was sore. A week later the pain had gotten worse, making the employee want to have their shoulder examined.

While entering the basket of a powder truck the employee reached over with left hand to close the lift arm. During this EE's hand slipped allowing for EE's left pinky finger to slide into a pinch point at the bottom of the lift arm. This resulted in the left pinky finger tip to become fractured and a laceration resulting in stitches.

Two maintenance employees were working on No 5 air door to remove the two bottom sections that had been previously damaged. While removing the rollers the door unexpectedly fell impacting the employees right index finger causing a laceration. The employee was taken for medical attention and received several stitches in the right index finger.

While performing repair work on a forklift, a maintenance employee pinched fingers on both hands between the first and second stages of the mast. The employee received 5 small (<1/2 inch) lacerations that required medical treatment. EE was released to return to work the same day.

Employee was mounting troughing rollers on belt structure. While tightening one side of a trougher, the opposite side slid off the structure causing it to swing down and hit the employee on the right leg causing a laceration.

Installing motor in fan housing, using scaling bar to align the shaft through the opening of the fan housing. ee dropped key from keyway, went to pick up, pulled hand through the output shaft, hand slid on greased shaft and ran fingers across keyway cutting 2 fingers

Employee was working on a scaler. At some point the employee's forearm contacted a sharp object on the machine. As the employee started to remove their gloves and suit, they discovered a cut on the inside of their forearm. The employee called for assistance and the cut was cleaned up, wrapped in gauze and sent for medical evaluation.

While working, an employee used left foot to slide a half full bucket of oil out of the way and felt pain in the left knee. Over the course of several months the employee worked through multiple diagnostic appointments with medical providers until a cause of the pain was identified. On Thursday March 21st treatment was administered that made the injury reportable to MSHA.

Louisiana Cat Techs were installing the tilt cylinder on the rear axle. While trying to line up the bores of the frame and the rod of the cylinder, the bearing on the frame was not lined up and was caught. While lining up EE's hand was on the bottom of the cylinder, when cylinder lined up it dropped 3" onto EE's finger. EE had gloves on at the time.

Initial injury occurred on 7/14/15 while employee was stacking 50lb salt blocks on a pallet. After visiting the doctor it was determined that it was just a strain and employee was sent back to work on full duty. On 5/31/16 Employee was shoveling salt at the screen plant. After taking EE to the doctor it was determined that EE has a hernia and was put on light duty.

Employees sending a switchgear underground in material cage. Switchgear was suspended in the cage while EE's tried to secure the bottom of the switchgear. EE stepped between the switchgear and forklift to attach the chains, the switchgear shifted pinching the EE's left leg between the forks & switchgear. EE received an abrasion/contusion to their shin.

Employee experienced a 25 db threshold shift in their right ear after being tested and retested. Employee stated that his regular doctor informed him that this ear has ear wax built up in it from a sinus infection that he needs to have cleaned out. We are waiting to hear back from audiologist on whether this is work related or not. Employee works in office setting underground.

While operating the walk behind Bobcat skid steer, performing clean up duties in the underground mill the employee pinched his hand near his pinky/ring finger between the piece of equipment and the surrounding steel structure causing a small laceration.

Employee was changing a bit on a roof bolter drill steel. He tripped and placed his right hand on the loaded bolt carousel to brace himself. His right pinky finger contacted the corner of a roof bolt plate causing a small laceration.

The employee noticed a piece of debris in his walking area at the gator pad. In an effort to prevent himself or someone else from tripping or injuring themselves he attempted to toss the chunk of salt to the nearest rib. In the process of throwing the chunk he contacted his right pinky finger on the lid of a nearby Knack box causing a laceration.

An EE was sprayed in the face with a 3% sulfuric acid, 97% water solution after a pipe broke in the ED Plant while testing rotation of a pump. The EEs eyes were examined and determined there was a small ulcer to the left eye. The doctor prescribed eye drops with an antibiotic to prevent infection. The employee was released to work without restriction.

Supervisor had stopped to take lunch at approx. 1am in U62A. Piece of hotdog became lodged just above the entrance of his stomach. Breathing was unaffected. Given the level of the employees discomfort, the decision was made to send him to the hospital. Employee returned to his next scheduled shift on 2/3/14.

Miner had his finger smashed by a bolt driven out of a guide and pinching his finger against the shaft wall.

During motion on lower a vertical bent section, injured worked contacted the edge of an exposed angle iron. Injured worker received a laceration on his left forearm.

The hand brake of a rail car was stuck so the employee tried to jerk the wheel, it did not move and he felt pain in his left shoulder. The employee was diagnosed with a minor tear in the left shoulder. No work restrictions were given and the employee was prescribed physical therapy.

Employee was descending a service loader with a filter assembly. The assembly got stuck on the steps and the employees right middle and ring finger tips were snipped off by a pinch point created between the handle and the housing.

While installing a bearing the pulley shifted causing the shell of the pulley to fall off the surface it was resting on. Resulting in the employee's right and left pinky fingers to be pinched between the bottom of the bearing and the steel bearing mount surface. The employee was taken to the hospital where he received stitches in both his left and right pinky fingers.

Employee was using a lift assist to stack 50# salt blocks on a pallet. The assist failed and a block was dropped onto the ground. The employee bent at the waist to pick up the block and then pivoted to place the block onto the pallet. While pivoting he felt a pain in his right lumbar. He did not report this until the next day when the pain persisted.

Had been installing returns on 22-skip and had removed PPE to get under belt and when he was getting redressed he started pulling on hardhat and walking at the same time and walked into structure. He received three stitches.

Performing maintenance on an "out of service" stamler unit. While installing flight chain bar, worker inadvertently placed left index finger in pinch point. Due to miscommunication, 2nd worker pulled chain causing chain to tighten catching workers left index finger. High noise levels contributed to accident. No equipment was damaged and mining operations were not impaired

The employee was attempting to remove a cylinder from an outrigger of a smag drill. When he loosened the cap screw holding the inner leg to the cylinder, the entire inner leg of the outrigger fell. The employees left ring finger was smashed between the ratchet he was using and the ground.

Employee was in the process of changing a cylinder on the face drill. Upon moving the cylinder to reposition it, the cylinder unexpectedlty swung pinching the employees finger between the cylinder and the boom of the drill.

employee and partner were changing the oil on a speed reducer for belt drive # 204. After completing task, they were cleaning up the work area. They attempted to lift a 50 lb Jabsco pump into the back of a Gator, as EE bent down and gripped the handle, he felt something pop in his lower back.

Employee was removing troughing rollers on # 210 belt in order to change the rails. While removing the rollers, he pinched his left ring finger between the roller and the rail.

Employee was moving a fireproof filing cabinet in the HR office. After moving the cabinet the employee stated he felt pain in his groin area.

Employee worked throughout the Mill Building, walking up and down the stairway replacing fire extinguishers, much of the shift. Near the end of the shift, employee was loading a large box of paper trash onto a pallet. While lifting the box, the load shifted and box started coming apart.

Employee was working on barge dock loading empty barges. Employee was bending over to pick up an accudraft box which weighs 5-10 lbs. He stated when he stood back up he felt a sharp pain in his back. Took employee to get examined on 11/16/07 doctor did not prescribe any medication but did place EE on light duty for a week. EE's personal doctor excused him from work on 11/21/07.

Mechanic needed to fill a Gator with hydraulic oil. He went to retrieve a 5-gallon pail of oil (weight 38.76lbs). During the lift he felt a sharp pain in his elbow. He experienced limited motion and swelling so was taken to the clinic for evaluation. X-rays were negative, no medications were prescribed, but he was returned to work with 25 pound lifting restriction.

3 employees were installing a 120lb stainless steel chute. After lifting the chute EE felt a twinge in his lower back. Following day 4/3/07 he was taken to the clinic, x-rays were negative, diagnosis was a Lumbosacral Strain, Mobic a nonsterodial anti-inflammatory drug was prescribed. The employee returned to work with restrictions.
