A Grove RT765E-2 crane was in use to install a motor into a Caterpillar 773B haul truck on level 3 in the quarry, when the auxiliary line broke, dropping the ball/hook onto a miner below, resulting in a fatal injury
A customer's truck became caught in overhead wires when the driver deployed the automatic tarp in driveway while exiting the mine. The driver exited vehicle safely and was electrocuted when the driver attempted to climb back in to the truck while the tarp arms were still in contact with overhead power lines. The accident resulted in a fatality.
An employee was attempting to clean out a previously blasted vertical drill hole with an air hose. A sudden release of pressure from an unknown source inside of the hole forced material to be ejected from the hole and struck the employee. The employee passed away from the incident.
About 3:15 pm., employee went to a storage building to get some planks. He climbed a stack of loaded pallets to get to the planks that were near the rafters and fell approximately 8 feet to the floor below and was found dead the next morning at approximately 6:02 am.
A miner become buried from material sliding down the face of a bank.
Employee was looking into Pegson stone hopper when the missing excavator tooth was ejected striking employee in the head.
The driller apparently attempted to load drill pipe into the carousel while the drill head was rotating. It appears a strap the driller was holding became entangled around the drill pipe after the drill pipe threaded into the drill head and rotated.
Customer was driving loaded dump truck down a grade when the truck went out of control. He jumped from the cab and was run over by the truck. Truck involved in accident was owned by a customer, not a contractor or mine operator.
Employee was walking near Gypsum Storage Building to return to the Finish Mill Building. Employee may have been struck by a piece of mobile equipment that was working in the area.
Employee was changing screen material in a triple deck screen when he fell over the hand rail. He fell approximately 56 feet to the ground below.
Employee was in dump truck waiting to be loaded with bulk salt from under 150 ton bin to transfer it to the stockpile pad on Cargill Deicing property. He was being loaded with salt when the brackets from the bin gave way and landed on the cab of his truck pinning him inside.
Miner was drilling a slab shot when the roof collapsed in on him causing fatal injuries.
Drop ball on crane fell striking victim in the head.
DIGGING INTO A 10 FT FACE WALL WITH A FRONT END LOADER, THE OPERATOR DUG OR REMOVED MATERIAL UNTIL THE BERM ABOVE FELL IN. THEN THE OPERATOR TRAVELED AROUND TO THE TOP OF THE WALL AND BACKED O VER IT.
