A mechanic suffered a strain to right shoulder while working on the Scaler. The injured miner was attempting to tighten a fitting on a 2" hydraulic hose when the tool EE was using slipped and EE felt a strain in the shoulder.

Mostrando los 37The Plant Operator sustained a laceration to left wrist while cutting a rubber hose off a fitting. The injured was wearing gloves and long sleeves and using a retractable blade safety knife. The knife blade failed to retract as the cut was being attempted and resulted in a wound that needed to be closed with sutures.
A miner suffered a sprain of the right shoulder joint while pulling on a wrench. The miner was changing screens at the Chip Plant when the incident occurred. The miner reported the incident on May 8th however, did not request medical treatment until May 14th.
A miner sustained a contusion and laceration to the area below nose and above upper lip when EE was struck by a falling object. The miner was adjusting the v-belts on the crusher and while positioned under the motor loosening the mounting bolts to move the motor, a spacer fell about 6 to 8 inches and struck the miner causing the injury.
An Operator strained a muscle in left lower back while pulling on a wrench to remove a bolt securing a hydraulic hose in place on a Front End Loader. The employee reported the incident and denied medical treatment at that time. Later next week ee made the Leadman aware ee had gone to the Chiropractor on own for treatment without informing anyone at the Mine Site.
A laborer has a complaint of pain to right shoulder. The injured states EE was cleaning out the bottom of the fluid bed dryer at the Mineral Filler Plant using a hoe and felt a pain in shoulder. EE did not seek medical treatment until June 1, 2016.
A repairman sustained an injury to the top of his left hand while using a chipping hammer. A steel chip came off the tool and went through the cloth gloves being worn. He did not seek medical treatment until 2/27/2015, the chip was removed by a hand surgeon.
A Driller injured his left long finger while removing a bolt from the primary crusher. While removing the third bolt he lost control of the bar on the torque amplifier causing his finger to be caught between the bar and the side of the crusher. He sought medical treatment the following day, the wound had to drained and splint was provided for his finger to be worn while at work.
An operator sustained a laceration to his left thigh that required sutures to close the wound. The injured miner was kneeling on the ground attempting to cut larger slots in a skirt board rubber. While cutting across the top part of the slots the razor knife got hung up. When more force was applied he lost control of the knife causing it to strike him in the upper left thigh.
A Driller suffered a lumbar strain while cleaning a hopper with a long handle scraper. He did not request medical treatment until April 27th at which time first aid treatment was provided at the clinic. A follow up visit to the clinic on May 14th now makes the case reportable with physical therapy and Rx medications being started.
An operator suffered a strain to his right shoulder while using a scraper to clean the inside of a hopper. While using a repetitive up and down motion with the scraper to clean the hopper he strained his right shoulder. Additional medical treatment provided now has made this incident reportable.
The injured employee was chipping slag from a weld with a chipping hammer and had a foreign body entered his left eye behind his safety glasses.
A mechanic sustained a contusion to his right elbow while using a shovel to unplug a chute. While in the process of clearing the chute the injured miner struck his elbow on a nearby handrail. Additional medical testing positive EMG for cubital and carpal syndrome of the right elbow and wrist make this incident reportable.
A laborer suffered a strain to left shoulder while cleaning a screen hopper. The injured miner was using a scraper bar to remove built up material from the hopper of the north shaker in the Transfer building. While scraping out the hopper, the bar hit the bottom of the hopper, jarring his left shoulder. He did not seek medical treatment until June 15, 2010.
A mechanic strained his right shoulder while installing a steering cylinder on a haul truck. The injured miner was using a 36" pry bar to line up a pin and as he pulled on the bar he felt a pain in his right shoulder.
The injured miner was shoveling material when he felt a foreign object enter his left eye. Nothing was found or removed from the eye however, prescription medication was provided for the irritation.

The employee was using a hammer to strike the side of the Primary Shaker when his left little finger was caught between the hammer handle and side of the shaker.
The employee was cleaning a bin using a long steel tamping rod when the material inside the bin broke loose striking the steel rod. The rod fell down straining the employee's shoulder. This is now a lost time injury as of November 7, thus making it a recordable injury.
The injured was prying bolts down on bolt flange with a pry bar. His hands slipped and the pry bar struck him under his lip. This injury required sutures.
INJ WAS SPLICING A BELT WHEN HIS KNIFE SLIPPED & LACERATED HIS ARM. A RX FOR PAIN KILLER WAS PRESCRIBED THUS MAKING THIS REPORTABLE
THE EMPLOYEE WAS LOWERING A STEEL PLATE INTO A CHUTE. THE SLACK IN THE CHAIN HOLDING THE STEEL PLATE RELEASED ALLOWING THE STEEL PLATE TO FALL ON THE EMPLOYEE'S FINGER FRACTURING IT.
THE INJURED WAS HOLDING A STEEL LINER PLATE WHILE THE SECOND EMPLOYEE WAS STRIKING IT WITH A SLEDGEHAMMER. THE STEEL LINER SWUNG LOOSE MOVING THE INJURED'S HAND INTO THE LINE OF THE SLEDGE HAM MER. THE SECOND EMPLOYEE THEN STRUCK THE INJURED'S HAND FRACTURING HIS LEFT RING FINGER.
THE MINER WAS IN THE PROCESS OF ADJUSTING A DIVIDER GATE WHEN THE CHAIN HOIST SLIPPED ALLOWING THE GATE TO FALL BACK ONTO HIS ARM. THIS INJURY REQUIRED SUTURES MAKING THIS A RECORDABLE INJURY .
EMPLOYEE WAS CUTTING A CONVEYOR BELT WHEN HIS RAZOR KNIFE SLIPPED AND CUT HIS RIGHT WRIST. THIS INJURY REQUIRED SUTURES MAKING THIS A RECORDABLE INJURY.
THE INJURED WAS STRIPPING WIRE WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT THE TOP OF HIS LEFT INDEX FINGER REQUIRING SUTURES.
THE EE WAS USING A RATCHET WRENCH TO REMOVE THE OIL PLUG ON A R-35 TRUCK. THE WRENCH SLIPPED & STRUCK THE EE ON THE LEFT HAND. THE INJURY CONSISTED OF A LACERATION REQUIRING FIVE STITCHES.
THE EE WAS USING A LSEDGE HAMMER TO STRIKE A PIN ON THE CUTTER HEAD OF THE DREDGE WHEN HE STRUCKHIS RIGHT MIDDLE FINGER.
THE MINER WAS CUTTING AN AIR HOSE WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT THE MINER'S LEFT THUMB.
WHILE ASSISTING WITH FEEDER REPAIRS THE HOOK FOR A COME ALONG BROKE AND STRUCK THE EE ON THE HARD HAT AND NOSE AREA. THE INJURY CONSISTS OF A FRACTURE AND LACERATION TO THE NOSE.
EE WAS USING A HAMMER TO BLOCK CABLE AND HAMMER SLID AND STRUCK HIS RIGHT FINGER AGAINST TRUCK BED FRAME.
EE WAS ASSISTING A MECHAINC REMOVING A RIPPER ON DOZER. AS THE EE WAS USING A HAMMER TO REMOVE PIN, A PIECE OF THE RIPPER BROKE OFF A STRUCK THE EE IN THE FACE AND CHEST.
WHILE USING A UTILITY KNIFE CUTTING A PIECE OF CONVEYOR BELTING, THE KNIFE SLIPPED & CUT THE E E'S RIGHT THIGH.
WHILE USING A IKNIFE TO CUT A CONVEYOR BELT THE EE CUT HIS RIGHT LEG.
