Employee had just finished walking along a path spraying CSS-1h Emulsion and was noticed to be out of breath. Shortly after, employee was noticed on their hands and knees on the ground and then collapsed face down. CPR was started and continued until an ambulance arrived and employee was transported to the hospital.

EE had finished eating lunch & was walking when EE got a sharp pain to left groin. EE was taken to ER via ambulance, taken off work & referred. EE was seen 9/20/19 & released with restrictions. Follow-up on 9/27/19 OCC Med provider determined there was no mechanism of injury & was released to work full duty & given a referral to PCP for personal illness.

EE was installing some bolts on a pipe flange. As EE repositioned self, EE felt a pop in the right knee with instant pain. EE was taken to the ER. EE was taken off work and referred to orthopedic doctor.

EE was walking on North cell line when he felt a pain in his knee. EE requested to go in and was seen at the ER. EE was taken off work and referred to Ortho.

While working side by side other employees, employee collapsed. CPR efforts were unsuccessful. ALS intercept personnel declared death at 4:05 p.m. Following investigation, determination was made that nothing work related contributed to his passing.

The wind had picked up in the afternoon of the 15th, sometime after that EE felt some discomfort in his left eye. He washed it out with water and it seemed to be okay the rest of the shift. He awoke the next morning and his eye was swollen and irritated.

EE was getting out of haul truck when the door blew shut smashing his fingers. EE was taken to the ER, he was diagnosed with three fractured finger tips. He was placed on restrictions and released back to work.

This is an 'Alleged Injury", this report may be amended upon further investigation. EE bent down to lift a roll of felt with a coworker, before lifting felt a strain in his lower back, he took a break and went and ran a rock truck for a while. Around noon his back was still hurting and that's when the foreman noticed and EE asked to be taken to a doctor.

Ee was checking the voltage on the batteries for the TDD when a gust of wind caused the battery box cover to fall on ee's right hand cutting both sides of his finger. He was taken to the front gate to be checked out by the first responders and then transfered to the ER he received stitches and was released back to work.

Discussing the work to be done with another employee, placed hand on handle of trackhoe turned body pointing at trench felt shoulder pop out.

EE was working in the guard shack when he suffered what appeared to be a massive heart attack. EE was transported to CRMC via the mine ambulance. Employee deceased.

Employee felt something in his eye. He was taken to the front gate to have his eye flushed. Employee felt no relief. He was taken into the ER where his eye was flushed. He was given eye drops, a prescription and was taken off work for one night.

Employee was stung by a bee. He was taken into the ER, given medication and taken off work. On going.

Employee's knee locked when trying to step out of ditch on the tailing dam after cleaning out the end of a 6" drainage line. He was taken to ER and was taken off work pending further diagnosis. Ongoing.

While washing the outside of the cable shop with a water hose, he felt something crawling on his head below the rim of the hard hat. Attempting to remove the bug, he felt something "get him". He was stung by a wasp in the webbing next to the pinky finger on his right hand. He was taken to the ER, given meds for swelling & infection. Unable to return his next shift due to swelling.

EMPLOYEE HAD PARKED FOR SHIFT CHANGE & WAS GETTING READY TO GET OFF THE TRUCK. WHEN HE REACHED IN THE CAB TO GET HIS PULLOVER & LUNCH BOX, HE FELT A TWINGE IN HIS LOWER BACK. EE WAS TAKEN OFF WORK BY HIS PERSONAL DR. AN ESTIMATED 30 DAYS OFF IS EXPECTED.

DUE TO HIGH WINDS, LIME WAS BLOWING EVERYWHERE.
