Employees were pulling a vertical turbine pump, had placed a joint of column pipe on lay down timbers. As the pipe reached the last timber, it was slightly higher, so employee attempted to lift the pipe and exacerbated a recently repaired hernia. Surgical repair slated for 05/21/26. Currently ONGOING.

Mostrando 50 de 303Employees right-hand middle finger pinched between the drill pipe & lifting strap. Xray was negative for fracture/dislocation with minor bruising to finger. Medical provided performed fingernail trepanation & issued oral oxycodone & Ibuprofen. Second opinion requested -Stated that with conservative care such as O/C ibuprofen /ice packs would achieve the same desired outcome as FAI.
Employee in truck bed, properly bent down to place case of liners in bed. EE heard/felt a pop in hamstring. Placed ice on the leg and continued to walk. On 3/12/25 EE returned to work and complained of leg pain. EE was seen at clinic and taken off work for 2 days
EE was attempting to put an insulator on an anode which was about 4" off the ground. As the EE went to hammer the insulator it caused the opposite side of the steer horn to come off the strongback. This resulted in the employee's hand being pinched between the strongback hook and the steer horn. EE was seen at ER and GHR clinic and was released back to work with restrictions.
EE was helping to replace hoist ropes on Shovel. EE was trying to pry the hoist ropes to the next rung on the hoist drum. EE lost contact with the rope causing pry bar to break free, smashing EE's left pinky between the pry bar and drum. EE was seen at the ER diagnosed with a fracture to left pinky & released with restrictions. Restrictions accommodated.
Driller was attempting to remove a 4" core barrel from the Sonic drill head when driller left the drill controls & attempted to grab it while it was rotating to finish unthreading/removing it. When driller grabbed the barrel it continued to rotate & the driller began rotating with the core barrel, causing driller to sustain a broken left tibia & fibula, & a dislocated left ankle.
Employee had just finished walking along a path spraying CSS-1h Emulsion and was noticed to be out of breath. Shortly after, employee was noticed on their hands and knees on the ground and then collapsed face down. CPR was started and continued until an ambulance arrived and employee was transported to the hospital.
EE was working at the 24S welding and then went to help co-workers when EE was done. Upon return to the Shop employee began to feel nauseous and began vomiting. EE was taken to the ER to be seen. EE was treated for heat illness. EE was given a full release at the ER.
The 417HT was being loaded by the 24S at the 5950 bench of Little Rock. While swinging the bed of the truck the dipper made contact rubbing against the bed of the haul truck. Employee was taken in for observation to the ER and was placed on 1 day of restrictions.
EE was trying to loosen a bolt with a ratchet and socket when the ratchet slipped striking EE in the mouth. EE's front #8 tooth was chipped and EE had a small cut on lower lip. EE was taken to the dentist. EE had temp cap installed, 2 stitches to lip and given a full duty release to work.
Employees were pulling cable to a floating barge using an airboat, when the corner of the airboat dipped down. This resulted in the airboat continuing to dip until it went fully down placing employees into the pond. Employees were wearing all their PPE, including life jackets. Employee was taken to the hospital for observation.

Employee was using a pry bar to move a lower sheave wheel, when the pry bar slipped, and EE fell backwards onto the drill pipe container (on the buggy rig) and sustained 2 lacerations to the right forearm, as EE fell. EE was examined at the local hospital and received 5 staples and released to full duty.
Employee was exiting the rear stairs of a bulk truck when EE's right foot contacted with the ground EE's ankle bent over causing pain. Employee was taken to the local hospital for medical treatment.
EE working on dozer left door wiper. Linkage in a bind, placed left hand on wiper motor and wiggled linkage. Linkage freed with the wiggling returning to center; employee's left finger caught in the linkage. EE was taken to the ER, received 5 sutures, restrictions until 10/11/22 sutures were scheduled to be removed. Started scheduled days off 10/5-10/9, returned 10/10 restricted.
EE reported to EE's supervisor on 9/8/22 that EE had noticed a rash on 9/5/22 on EE's wrists that had since grown over the past three days. EE was taken to clinic and diagnosed with contact dermatitis. EE was released back to work with no restrictions. EE went on scheduled days off and returned to work 9/12/22.
Employee was working on shim on dipper they placed a bar between the latch lever and H-block. Either the bar, latch lever or H-block shifted creating a pinch point. The employees right ring finger was caught in the pinch point. Employee was taken to ER, they were released with restrictions and referred to Ortho for further evaluation.
Maintenance mechanic called out to work on the east starter sheet machine, as it had jumped a tooth on the up ender outside the chain sprocket. The mechanic used a pry bar for leverage to check the chain tension. The bar slipped catching finger between the bar and the chain sprocket. EE was seen at ER. X-ray showed a fracture to middle finger tip, EE was referred to ortho doctor.
Operator had finished cleaning windows on the small water truck as EE was dismounting with EE's right foot to lower step EE's left foot slipped off the top step. EE let go of the grab bar as the slip caused EE's body to twist landing on EE's left side.
Employees were installing a high voltage cabinet on the shovel carbody. A skid steer was being used to hoist the cabinet sitting on a wood pallet into position. The injured employee was on top of the carbody reaching over to align the cabinet, when the injured employee signaled to lower the cabinet it bounced pinching the fingers between the mounting ledge and the cabinet.
EE was switching the Y09-10 switch when it failed startling them. Employee dove to the ground hitting a knee on rock. Employee was seen by orthopedic doctor on 2/3/20 diagnosed with contusion given a knee brace and taken off work.
EE operating west starter machine when hanger bar came out. Pushed emergency stop before machine jammed. EE asked coworker help reverse machine. EE put right hand on sheet feed platform to watch. As machine rotation reversed sheet guide pulled EE's right middle finger amputating the tip of middle finger distal phalange from 3/4-to full fingernail up.
EE had finished eating lunch & was walking when EE got a sharp pain to left groin. EE was taken to ER via ambulance, taken off work & referred. EE was seen 9/20/19 & released with restrictions. Follow-up on 9/27/19 OCC Med provider determined there was no mechanism of injury & was released to work full duty & given a referral to PCP for personal illness.
EE was cutting weeds when EE stepped back with left leg. EE said felt knee hyperextend and felt a pop. EE was treated on site by First Responder ice applied, but did not want further treatment. On 8/15/2019 EE reported to work and said EE wanted to be taken in.
EE got into the excavator and started it up and turned to the right to place EE's lunch behind EE. The high voltage alarm went off loudly near EE's right ear. EE was seen and released to work with restrictions.
While preparing to perform side frame/track repairs on a 4100A shovel. When the relief valve was ejected from the jack. Employee was struck by a pressure relief valve in the lower left abdominal/waist area. The injury resulted in the death of the employee.
EE was getting off a 6' ladder. EE was at the second to the last rung and stepped off thinking EE was at the bottom. EE fell landing on right shoulder and hit left elbow.
EE called supervisor on 2/20/19 to report that on 2/19/19 after clocking out EE was walking to the employee parking lot and rolled right ankle. EE was seen at the ER, fractured fibula, taken off work, referred to orthopedic doctor.
Haul Truck driver was being loaded when the first dipper released into the truck a large boulder struck the bed jarring the employee. EE was taken to the ER and was given time off work.
EE reported on 8/6/18 that they had hurt their knee on or around 6/18/18 between 7:30 a.m. - 11 a.m., they stated that they felt the knee pop while in a kneeling position. On 8/7/18 EE was taken to the doctor to be seen. EE was released back to work with restrictions.
EE was installing flexible rubber hose on the discharge side of the pump. When the hose slipped and struck EE on the lower right leg; sweeping both EE's feet from under EE causing EE fall to the ground into a sitting position. EE was taken to the ER to be seen. EE was diagnosed with a fracture to EE's right ankle.
EE was trying to remove a railroad tie when left index finger was smashed. The employee was taken to clinic steri-strips and glue were applied to the left index finger and ee was released without restrictions.
Electrician was re-energizing a 480V breaker when an arc flash occurred. Site contacted MSHA as the extent of the injuries were unknown at the time of incident. After arriving at UNM, injuries were determined to be less severe. Employee was diagnosed with 2nd degrees on 3% of body.
EE was walking back to office at Warehouse when they tripped and fell. EE caught themselves on hands and knees. EE was brought to safety to be checked by first responder; refused further treatment. On 12/7/2017 EE was taken to clinic for reported right ankle swelling. EE was given an ankle brace and given a full duty release back to work.

EE was loading cathodes when one got hung up. EE reached in to try and pick up the sheet when the cathode weight shifted smashing the right little finger. EE was taken to GRMC. Fracture to right little finger, stitches and a finger splint.
EE was installing some bolts on a pipe flange. As EE repositioned self, EE felt a pop in the right knee with instant pain. EE was taken to the ER. EE was taken off work and referred to orthopedic doctor.
EE was removing rocks from the tongue of the trailer to hook it up to the tow vehicle. EE picked up a rock and the tongue moved in an upward direction, smashing the employees thumb between the rock and the tongue on the trailer. EE was seen at the ER and diagnosed with a fracture to the thumb. Released with restrictions.
EE was working on cell furniture install. EE went to lift the copper "round bar" that was inside the jumper frame to place it on the hangers. The bar slipped out of EE's left hand pinching right ring finger between the round bar and the capblock. EE was taken to the ER x-ray positive for fracture to right ring finger. EE given restrictions and released back to work.
EE was retrieving chock from pickup bed. EE was standing on the bumper and slipped causing EE to fall to the ground. Landing on left side. EE complained of left wrist pain. EE was taken to the ER.
EE was closing gate at the scrape yard & lost footing and fell. EE fell backwards to the ground landing on the left side of body. EE was taken to the ER & was given 4 days off.
EE was assisting in removal of gantry pin from the 24 shovel. A spacer, along with a hydraulic ram was being used. As pressure was being applied with the ram the spacer came out of place striking the EE in the right shoulder. EE was taken to GRMC x-rays negative for break. Diagnosed with Contusion to right shoulder. Placed on restrictions for 3 days; released back to work.
EE was exiting forklift when rolled left ankle. EE was taken to the GRMC ER diagnosed with a sprain. Taken off work and referred to personal doc.
EE noticed door open on a PSA gunk pumper. EE hit it to close it. The cable holding the door struck the employee's right hand. EE was taken to GRMC. EE was diagnosed with a fracture to right thumb and hematoma. EE was released back to work with a full duty release.
MayDay at 0136 hours for communication pole struck by lightning resulting in fire. A blade operator noticed the fire and called the MayDay. The Fire Brigade was dispatched. The fire was extinguisher at 0238 hours and a fire watch set up. MSHA was notified by site Safety at 0138 hours.
EE was changing a lip protector on the #4 Loader. While attaching the plow the bolts spun cutting through leather glove resulting in a laceration to the employee's right forefinger. EE was taken to the ER received four stitches and released back to work full duty.
EE was attempting to tighten a bolt with a pipe wrench. EE positioned self in a low stance (kneeling). EE started to pull the wrench causing it to come loose on the fitting and swivel around smashing thumb between the wrench handle and a steel guard plate. EE was taken to ER given four stitches, diagnosed with fractured thumb and taken off work until seen by Ortho.
On 2/26/16 EE was washing the 6-blade. EE bent over to remove the steam cleaner hose from the ripper. As EE stood up EE felt a pull in the lower back. EE did not report incident until 2/28/16 EE did not want medical treatment. On 2/29/16 EE stated back was feeling sore and wanted to be seen. EE was seen at GRMC placed on restrictions. On 3/4/16 EE was taken off work by doc.
EE was torquing a nut when he caught his finger between the RAD gun arm and the swivels and the inner part of the wheel. Taken to ER to be checked. Fractured left index finger, 5 sutures and returned to work with restrictions.
EE was attempting to recover a valve and small piece of 4" HDPE pipe from under a rock. The EE attempted to lift a large rock with his left hand and pull the pipe with his right hand. The rock slipped pinching his right middle finger. EE was taken to the ER, received 3 stitches and diagnosed with a fractured finger tip and taken off work.
