EMPLOYEE WAS OPERATING AN EIMCO 935 LHD SCOOPTRAM. HE SET THE PARK BRAKE AND WALKED BEHIND THE SCOOPTRAM TO HELP PIN THE CONVEYER CHAIN OF AN EIMCO CONT MINER. THE PARK BRAKE FAILED ALLOWING THE SCOOPTRAM TO ROLL, PINNING THE OPERATOR BETWEEN THE REAR OF THE SCOOP TRAM AND THE END OF THE BOOM OF THE CONTINUOUS MINER.
EE WAS A PASSENGER ON A WALLACE MANTRIP WHEN HIS SEAT CUSHION SLIPPED. HE GRABBED THE SEAT CUSHION AND WHILE DOING SO, HE PLACED HIS HAND ON THE EDGE OF THE FLOOR WITH HIS FINGERS EXTENDED UND ER THE FLOOR. THE VEHICLE HIT A BUMP IN THE ROADWAY AND THE BOTTOM OF THE VEHICLE CONTACTED ROADWAY, MASHING THE 2 MIDDLE FINGERS OF RIGHT HAND.
AT THE END OF THE SHIFT, EE AND OTHER WERE RETURNING TO THE SHAFT BOTTOM TO CAGE OUT OF THE MINE. HE LOST CONTROL OF THE VEHICLE HE WAS DRIVING AND RAN INTO THE MINE RIB. THE LEFT SIDE OF THE VEHICLE WAS PUSHED BACK SEVERAL INCHES BY THE IMPACT. THE DASH AND STEERING WHEEL PINNED EE INT HE DRIVERS SEAT CAUSING HIS INJURIES. IT TOOK APPROX. 1 HOUR TO FREE HIM FROM VECHILE.
AT END OF SHIFT, EE & CO-WORKER WERE RETURNING TO SHAFT BOTTOM TO CAGE OUT OF MINE. CO-WORKER LOST CONTROL OF VEHICLE HE WAS DRIVING & RAN INTO MINE RIB. EE, WHO WAS A PASSENGER, WAS THROWN OF F FRONT OF VEHICLE & ROLLED ON MINE FLOOR FOR SEVERAL FEET, CAUSING HIS INJURIES.
TWO EES WERE REPLACING ROLLERS ONS TEPS OF THE CRYSTALLIZER PLANT MANLIFT ELEVATOR. THE ELEVATOR WAS INADVERTENTLY STARTED BY ANOTHER EE, BECAUSE THE ELEVATOR WAS NOT PROPERLY LOCKED OUT. ONE STEP CAUGHT EE MASHING HIM BETWEEN THE STEP AND CEMENT PEDESTAL CAUSING HIS INJURY. REQUIRED STITCHES.
EMPLOYEE WAS OPERATING MOBILE BRIDGE CONVEYER WHILE KNEELING IN SEAT WITH HIS FEET OUTSIDE OF THE OPERATOR'S COMPARTMENT. HE TURNED THE UNIT TO THE LEFT, AND HIS RIGHT FOOT WAS MASHED BETWEEN THE SIDE OF THE CAB AND THE MINE RIB, BRUISING HIS FOOT.
MECHANIC WAS SENT TO GAUGE ROLLER MILL SPACING. THE FEED WAS SHUT OFF THE MILL. THE MACHINE WAS SWITCHED OFF. THE MECHANIC PLACED GUAGE BETWEEN THE ROLLERS PRIOR TO ROLLERS STOPPING. HIS FINGE RS (SMALL, RING & MIDDLE) OF IS RIGHT HAND WERE DRAWN BETWEEN THE ROLLERS CAUSING THE 3 FINGERS TO BE CRUSHED. THIS INJURY RESULTED IN THE FIRST JOINT FINGER BEING LOST. THE REST SHOULD HEAL.
EMPLOYEE WAS RIDING ON A PERSONNEL VEHICLE FROM THE MINE SHAFT TO HIS ASSIGNED WORK AREA. THE VEHICLE HIT A POT HOLE IN THE FLOOR, STRAINING HIS LOWER BACK AND BRUISING HIS TAIL BONE.
