As an articulating truck was backing towards the drill pad at 5 mph, driver failed to see a dozer, striking the rearend. Initially, both operators stated they weren't injured. However, the next morning, the dozer operator complained of neck and shoulder pain. EE was taken to the clinic and placed restrictions. Currently ongoing.

Torque arm for the backstop on Sag2 Conveyor seized, causing the arm to swing around and grazed employee's right arm. EE was taken to hospital and taken off work until cleared by PCP. Currently ONGOING.

Employee was being tasked trained on a service truck when they hit an icy patch on the haul road and began to slide. When they hit dry ground, they fishtailed and then rolled over. Employees were able to exit the truck and call a MAYDAY, and were taken to the ER, where they were examined and given one day off work.

As haul truck was backing into the shovel, dipper made contact with the bed, jarring the haul truck operator. Initially operator refused medical attention, but after about 3 hours, requested to be taken to hospital. ER referred to PCP, PCP took operator off work. Currently ongoing.

When entering the employee parking lot on 10/07/21, employee hit a pot hole and twisted EE's back. EE reported the condition at the morning tailgate. On 10/12/21, employee requested time off work to seek chiropractic treatment, and time off 10/13/21 to see employee's PCP about pain back EE claims is from hitting the pot hole in EE's personal vehicle.

At the start of shift August 14th, employee reported having back pain as a result of hitting a hole on the road to the crusher the previous shift. It had been raining on and off all day. Employee was taken to the ER, and given a full release for 8/17/21.

While driving down the tailing ramp, water truck hit a rut, causing front tires to jerk, and caused pain in driver's shoulder.

Employee was spotting under shovel, did not stop in time after hearing shovel horn, and hit the dipper with the truck bed. Employee reported to the supervisor at end of shift, and refused medical treatment. Start of shift 03/19/21, employee requested medical treatment, was given restrictions the site is accommodating.

Coolant line on haul truck failed, spraying coolant on deck. Employee downed the truck, and as they stepped out on the deck, waiting for the mechanic, stepped on non skid resistant area, slipped and fell. EE was taken to the ER and referred to PCP, given restrictions which can not be accommodated. F/U 9/20/18.

Mechanic was helping install tracks back on a drill, watching the track and idler for clearance, EE heard a pop, then felt something hit above left eye. EE didn't realize was hurt until EE wiped face and saw blood. After EE got home, felt something there, went to PCP and a small piece of metal was removed. PCP gave EE a day off work.

Employee was rolling up window on haul truck when ee felt a pop in shoulder. EE requested an ice pack, then later requested further medical attention. EE was taken to the ER, given restrictions, and referred to a specialist. CURRENTLY ONGOING.

Employee was operating a pickup, came to a stop, and was exiting the cab. When employee put their foot on the ground, the pickup began to roll backwards, and the door knocked them to the ground. The left front tire ran over their left ankle. Employee is currently off work.

The first loader bucket in a haul truck caused a loud noise, driver noticed window was spidered. As driver was taken by pickup to office to complete report, they told supervisor they had some back pain. Was take to ER and released. On 12/5, went to PCP, taken off until 12/20 appt. Ongoing.

After a rain event, haul truck left the stockpile, began to slide, doing a 180, and came to a stop. 2nd truck came off the dump, attempted to stop, slid sideways into 1st truck. 2nd driver was seen at ER on 10/12 and fully released. Oct. 15 employee complained of back spasms, went to PCP, and taken off work. CURRENTLY ONGOING.

While utilizing a forklift to move the drive pulley of the coarse ore conveyor, the crew decided to re-rig the load. Then as they lifted the load, it shifted, made contact with the employee, causing ee to hit a structure beam.

Employee claims seat bottomed out driving over transition from dump to haul road. Employee was taken to the ER, and given a full release to next shift. On 9/12/16, company received medical documentation removing employee from work due to event as of 9/1/16. Currently Ongoing.

Operator reported the haul truck was riding rough on 08/18. On 8/24 employee called work requesting to be seen by a physician for symptoms EE attributed to the rough riding haul truck. EE was given restrictions company cannot accommodate and referred for PT. Currently Ongoing.

Employee was operating a haul truck towards the shovel pit, hit a pot hole hidden by mud, jarring employee's back. CURRENTLY ONGOING

Employee was assisting moving cable, had hooked the cable to the front bumper of the pickup. Pickup driver put the vehicle in reverse, it rolled forward making contact with the ground employee's knee. Employee felt a pop. Initially no restrictions, referred to orthopedic, surgery on 10/5/16. Currently ongoing.

In employee parking lot, employee thought personal vehicle was out of gear, turned the key, it lunged backwards, striking the vehicle to the right. Injured employee was leaning against the rear of the vehicle on the right, waiting for carpool, was struck by parked car, causing back pain.

Employee was boarding a haul truck, activated the switch to raise the boarding ladder, and got hand pinned between the handrail and switch, bruising hand. Was taken to the ER, no fracture, but told to follow up with PCP. Currently Ongoing.

Employee reported rough ride on haul truck, mechanic observed bed had too much movement. Truck was downed, and driver reassigned to another truck. EE initially refused treatment, but about 2:20 p.m. requested to be taken to personal physician. EE was given restrictions company cannot accommodate, CURRENTLY ONGOING.

743HT driver passed a blade, stayed near the windrow, saw the 716HT approaching, so stopped. 716HT driver thought there was room between the 743 and blade to continue, but grazed the rock deflectors. 743 stated he was fine, went to the ER after shift, given a RX muscle relaxer, off work until clears D&A test.

Employee reported smelling an odor in the haul truck and feeling ill. She was taken to the ER, diagnosed with an acute headache, and taken off work.

Employee was attempting to dump at the stockpile when employee claims the bed came down while still partially loaded. Employee was taken to the ER, given a Rx for muscle relaxant that the company's D&A policy does not allow employee to work while using.

The shovel dipper opened on the dovetail of a haul truck, jarring an employee. He was taken to the ER and taken off work until 11/24/15.

Employee was operating a forklift downhill, hit a rock, causing the wheel to go into a little ditch, and laid the forklift on its side. The employee's foot was caught under the cage, fracturing his right big toe.

Haul truck operator had his window open as he pulled out of the shovel pit, dust came in the cab and got in his eye. He rubbed before flushing. Was taken to the ER, diagnosed with scratched cornea. Counseled to keep windows closed.

While being loaded, load shook the haul truck and EE felt back pain. She initially refused treatment beyond ice pack and ibuprofen. While on days off, EE went to PCP due to continued pain, was prescribed medication that company's D & A policy does not allow her to work. Currently ongoing.

As man van headed out to distribute drivers to the haul trucks, it hit a pot hole, causing one employee to hit his head on ceiling of man van, causing pain. He was taken to the ER, and was released to his next shift. On 7/20/15, employee requested to see his PCP, and was prescribed physical therapy for his neck.

Truck driver was attempting to unload when the haul truck rolled forward. He then attempted to lower the bed from mid-way up, which caused the bed to fall, jarring the employee. He requested medical attention, was taken to the ER and taken off work until 7/5/15.

Driver was being loaded when a large rock struck the bed of the truck, jarring the driver. He requested medical attention, was taken to the ER, diagnosed with cervical strain and taken off work until 6/7/15.

Haul truck driver was dumping load, when the bed lowered unexpectedly, jarring him. He was seen by an 1st responder and continued to work. 5/31 he requested medical attention, after time in ER, was sent home, referred to PCP, started days off. Did not see PCP until 6/5, released for 6/6, worked full shift, SDO 6/7 called off 6/8, currently ongoing.

On 5/22/15, employee requested to leave early stating his shoulder hurt, and he was not sure if he slept wrong, or if on 5/21/15 He reached to turn on his windshield wipers he hit a pot hole. No medical records were provided until 8/10/15 stating this was due to work, when bill was received by company.

Employee put his seatbelt on when he mounted the forklift. As he pulled the seatbelt from right to left, he felt a pop in his right shoulder blade area. He requested medical attention, was taken to the ER and taken off work for 2 days.

When haul truck operator dumped his load and let the bed down, the bed seemed to hit hard, jarring the employee. The employee traveled back to the shovel and then reported pain in his neck, was taken to the ER and taken off work until 4/20/15.

Employee was riding in the back of a man van at the end of shift. The van hit a pot hole, causing the employee to be bounced. He came in the next shift and reported pain, but worked through the shift, seeing his PCP the next day; was taken off work for 2 days scheduled work days.

On 1/19/15 a supervisor reported while driving a pickup he hit some known bumps and bottomed out, and he alleged he experienced lower back pain. He went to his PCP 1/21/15, was given a full release to work. Employee then notified company he needed off due to personal health issues. On 4/22/15, company was notified employee alleges he's been off since 2/5/15 due to back injury.

Employee was installing an impeller on a pump. He had his hand on the base, motioned to the forklift operator to lower the forks, and his pinky finger was beneath the fork. He was taken to the ER, diagnosed with a closed distal fracture and placed on restrictions. Currently ongoing.

Loader operator attempted to push a loose wire back into the squirrel cage of the A/C unit and caught his fingers in the fan blades. He was taken to the ER, the lacerations required 4 stitches to close, and he was taken off work.
